[Preliminary data on post-pancreatectomy diabetes mellitus treated by islet-cell autotransplantation].

The present study describes the first islet autotransplant program in Romania, and the first 3 cases of subtotal pancreatectomy for chronic pancreatitis combined with islet autotransplant. The primary objective was to pain relief by pancreatic resection, but also to preserve the endocrine function by islet autotransplant. Extensive distal pancreatectomy is effective in relieving pain, but should be limited to patients with small duct disease, in whom more conservative methods have failed, because of severe metabolic consequences. Islets were prepared by Liberase digestion of the excised pancreas, and infused unpurified into the portal vein in one case and in the omental pouch and peritoneum in two cases. All patients were relieved of pain, have achieved insulin independence, and positive C peptide levels, but one patient died of a acute bronchopneumonia 60 days post-transplantation. The mean islet yields were 2100 islet equivalents/Kg body weight. Islet autotransplantation can be considered a useful therapeutic option serving to prevent the occurrence of surgically-induced diabetes. The results have indicate that the omental pouch is a viable site for islet autotransplantation, that can accommodate a large tissue volume, is easy to access to implant, and the IBMIR (instant blood mediated inflammatory reaction) may be less severe.

Time-related variations of some biochemical parameters in patients with short- or long-lasting post-traumatic coma.

The variation in time of some blood biochemical parameters was investigated in 100 patients with acute head injury and coma of variable duration, in parallel with the clinical course. The purpose of this study was to establish possible correlations between the severity of the trauma (assessed according to the coma classification proposed by C. Arseni) and the variation of the parameters studied. The results show complex correlations between the biochemical events in the patient and the degree of brain function impairment initially and after some time. Long lasting coma (over 14 days) is associated with complex biochemical alterations representing an additional element for defining the clinical condition.

[A study of the microelements in the dental plaque].

In a total of 49 patients, of which 7 were controls, and the other had chronic superficial parodonthopathies, deep chronic mixed parodontopathies and gingivitis, a quantitative analysis was done of microelements from the dental plaque. Neutron activation analysis, chemical and chromatographic methods were used for evidencing the presence of Fe, Mn, Zn, Mg, Co, Ca, Na, and K ions in the dental plaque. Significant changes were noted with regard to the Fe, Zn, and Ca contents of the dental plaque in patients with deep and mixed parodonthopathies. In the patients with gingivitis high Mn and Na concentrations were found and low Mg and K ion contents. Cobalt in the dental plaque was within normal limits in all the patients. Changes of the microelements in the dental plaque are useful for clinical laboratories in view of detecting at an early stage the development of pathological processes which occur in the buccal cavity.

Acupuncture treatment of discopathic pain.

The authors present the effects of acupuncture in 50 patients showing discopathic pain. It is concluded that acupuncture treatment favours the reduction of drug consumption, and, implicitly, of iatrogenic pathology.

[Gastric resection with preservation of the pylorus with a variant of pyloroplasty].

The authors present the case of a female patient with gastric ulcer of the small curvature of the stomach who showed a particular neuropsychical lability, in whom emergency surgery was performed before it was possible to obtain data concerning the behaviour of the gastric acidity. In an attempt to avoid the postoperative dumping syndrome the authors performed a medio-fastric resection, removing the area with the ulcer and maintaining the antro-pylorus. For the obtention of a good drainage of the remaining stomach a variant of the pyloroplastia technique was associated.

[Technic for surgical reconstruction in comminuted fractures of the head of the radius].

Osteosynthesis n comminuted fracture of the head of the radius is extremely difficult because of the small size of the bone fragments. The particularly rich innervation of the elbow explains the violent vasomotor reaction that accompanied injuries of the elbow, reaction responsible in the last instance of the onset of biological alterations the articulation. For this reason, the therapeutical solutions must have in view rapid restoring of the passive and active movements of the elbow. Reactions of the radius head is often followed by ossification around the radius stump, subluxation of the inferior articulation and valgus of the ulna. Osteosynthesis is only possible if there is a single detached bone fragment in posterioexternal position with respect to the radius head circumference. The authors describe an original technique, i.e. reducing the fractured bone fragments which are then fixed with a Kirschner rod, brought out percutaneously. A description is given of the operative procedure and postoperative care in the first eight patients operated. Very good results were obtained in 4 case, good results in 3 and poor results in one case.
