Evaluating the impact of methadone maintenance programmes on mortality due to overdose and aids in a cohort of heroin users in Spain.

AIMS: To assess the relationship between methadone treatment (MT) and overdose and HIV/AIDS mortality among heroin users resident in Barcelona city. DESIGN: All patients who started treatment in any treatment centre between 1992 and 1997 were included in a cohort the first time they were admitted for heroin addiction treatment. Follow-up controls were carried out every 9 months, on average, until 31 December 1999. Variables, both constant and varying over time, were fitted into Cox regression models. FINDINGS: The study recruited 5049 patients, which provided 23,048.2 person-years. Fifty per cent were in MT during the study period; of the total cohort 1005 patients died: 38.4% due to AIDS, 34.7% to overdose and 27% to other causes. Overall mortality decreased from 5.9 deaths per 100 person-years in 1992 to 1.6 in 1999. Globally, life expectancy at birth was 39 years, 38 years lower than that of the general population. The main factor for overdose mortality was not being in MT at the time of death [relative ratio (RR) = 7.1]; other factors were being a current injector at baseline and being HIV positive. For AIDS mortality, the main factor was the calendar year (RR for 1996 versus 1999 = 4.6), the next major factor was more than 10 years of heroin consumption, followed by not being in MT, being unemployed, then having a prison record. CONCLUSIONS: The observed mortality decline could be linked to the effectiveness of low-threshold MT. The life expectancy of heroin users increased by 21 years during the study period.

[Tobacco in Barcelona's metropolitan transportation: observation study].

OBJECTIVE: To describe the extent of no-smoking notices, consumption and sales of tobacco in Barcelona's underground transport installations. DESIGN: Descriptive, observation study. SETTING: Community. METHODS: A sample of 54 stations was selected at random. The presence of users smoking, staff smoking, notices about tobacco consumption and tobacco sales were evaluated. INTERACTIONS: Platforms, entrance hall and corridors and/or stairs were observed for at least 5 minutes at each station. MEASUREMENTS AND MAIN RESULTS: Users smoking were recorded in 87% of the observations, and cigarette stubs in 97.5% of observations, whereas company staff were only seen smoking in 5.6%. In 72.2% of the entrances there were tobacco sales-points. The users observed smoking were divided equally between the sexes, although the men were mainly middle-aged adults (64.3%) and the women were mainly young (60.6%). In 64.2% of the observations, some sort of notice was seen. CONCLUSIONS: Both notices and compliance by staff are high, although users habitually consume tobacco. Tobacco sales-points at stations are very common. The findings justify the adoption of specific measures designed to improve compliance with the regulations.

Intraoperative donor cholangiography.

Biliary drainage has long been called the Achilles' heel of liver transplantation, and biliary complications compromise the success of liver transplantation by increasing graft loss and the rates of a required second operation, morbidity, and mortality. One cause of complications is unrecognized anomalous biliary anatomy. We examined 73 intraoperative donor duct cholangiograms (IODDCs) to assess our ability to identify biliary anomalies intraoperatively. Normal anatomy was seen in 42% (31/73); some part of the right-sided biliary system drained into the left bile duct in 22% (16/73); trifurcated systems with a single branch point for the right posterior, right anterior, and left ducts appeared in 16% (12/73); low insertion of a right segmental duct to the hepatic duct was seen in 11% (8/73); and drainage of a right segmental duct into the cystic duct or into the hepatic duct at the cystic duct origin was noted in 8% (6/73). It was believed that the last group represented a condition that dictated extra caution in biliary reconstruction. The incidence of radiographic recognition of these anomalies was more than twice the clinical recognition in our patient population, implying that many such "problem" ducts usually go unrecognized. IODDCs facilitate training of transplant fellows. Costs are low, and morbidity is nil.

Antigenicity of basement membrane fractions obtained from rat seminiferous tubules.

A noncollagenous fraction of basement membrane (D-STBM) obtained from rat testes was submitted to sodium dodecyl sulfate-polyacrylamide slab gel electrophoresis (SDS-PAGE), and eight well-defined bands were detected. A cross-reaction with an antiserum against laminin was revealed by immunoblotting in five bands, with molecular weights ranging from 54 to 64 kDa. No further resolution of these components could be obtained by size exclusion and ionic exchange chromatography. Fifty-two percent of the rats immunized with D-STBM and adjuvants developed a mild multifocal damage of the testis. The lesions were characterized by foci of seminiferous tubules with different degrees of sloughing and/or atrophy of the germinal epithelium. Giant multinucleated cells were frequently seen, and mild interstitial mononuclear cell infiltrates were also detected. By immunofluorescence, deposits of rat IgG with a faint discontinuous linear pattern were observed along the walls of the seminiferous tubules. Circulating antibodies to D-STBM were detected by ELISA in 100% of the rats, whereas in a cross-reaction with laminin antibodies were detected in only 63%. All rats studied revealed a positive delayed type of hypersensitivity (DTH) response to D-STBM. None of the control rats injected with saline and adjuvants presented circulating antibodies to D-STBM or laminin or a positive DTH reaction to D-STBM. Some control group rats (10%) revealed few isolated seminiferous tubules with some degree of sloughing of the germinal epithelium.

[Transurethral resection using glycine irrigation].

An accident is described which occurred during a transurethral resection of a prostatic adenoma using a 1.5% glycine solution in a 68 yr old patient. The surgery was performed under spinal anaesthesia with bupivacaine. After 14 l of irrigation fluid had been used, visual disturbances, bilateral mydriasis and drowsiness appeared, with no haemodynamic disturbances. Three hours after the end of surgery, there appeared convulsions, with the blood sodium level at 118 mmol X l-1. Symptomatic treatment resolved the problem, and the clinical examination was normal 12 h after surgery. The role of hyponatraemia in the clinical state is discussed. The usefulness of regional anaesthesia in the early detection of this type of accident is recalled.

[Testicular neoplasms in cryptorchism].

The authors study the greater incidence of testicle neoplasias in cryptorchidic patients and the convenience of hormone or surgical treatment before puberty. These follows a discussion of the therapy to be applied depending upon whether the cryptorchidia is unilateral or bilateral.
