[Follow-up of chronic inflammatory bowel disease. A 10-year material].

During a ten year period, all patients (N = 641) with chronic inflammatory bowel disease were systematically followed, clinically and endoscopically by means of mucosal biopsies. The complexity of these disorders was obvious, and in 103 patients it was impossible to establish a diagnosis of either ulcerative colitis or Crohn's disease. Fulminant colitis occurred in 9.8% of the ulcerative colitis patients and only four of these had to have surgery. Total colectomy was necessary in only 58 patients. Twenty-one patients died, four from causes related to the inflammatory bowel disease. Six patients with ulcerative colitis were treated for adenocarcinoma of the colon. No malignancies were found in the patients with Crohn's disease. The biopsies showed only two patients with high grade dysplasia.

Effect of peroral antacid treatment in patients with acute upper gastrointestinal haemorrhage: a randomized controlled trial.

During recent years, it has been reported that antacids have a beneficial prophylactic effect in critically ill patients regarding upper gastrointestinal bleeding. In these trials rather complicated treatment schedules and high doses of antacids were used. The present study was designed to investigate the therapeutic efficacy of a more practical antacid treatment regimen in patients with haematemesis and/or melaena. Ninety patients, hospitalized for haematemesis and/or melaena within the past 24 hours, were included to double-blind treatment with antacids or placebo for 7 days. The dose, 20 ml liquid antacid (Balancid) with a neutralizing capacity of 105 mmol, was administered each second hour during waking hours. The groups were comparable regarding incidence of ulcer disease, age, sex, acid secretion, and usage of aspirin. Continuous bleeding or rebleeding was recorded in 15 of the 43 patients in the antacid group (34.9%), compared to 19 of the 47 patients in the placebo group (40.4%), the difference not being statistically significant. Furthermore, the requirement of blood transfusions was not significantly different in the two groups. In conclusion, the haemostatic effect of the antacid treatment regimen used in the present study was not significantly superior to placebo in patients with endoscopically verified acute upper gastrointestinal haemorrhage.

Localized telangiopathy of the stomach and duodenum diagnosed and treated endoscopically. Case reports and review.

During the period from January, 1976 to December, 1979 we performed 5000 upper gastrointestinal endoscopic examinations. Of these 650 were for gastrointestinal bleeding. In nine patients we found, endoscopically localized mucosal vascular abnormalities of the stomach and duodenum. Repeated endoscopic examinations were necessary in four cases before the correct diagnosis was made. Three telangiopathies (as we prefer to call them) were found incidentally, and in six cases endoscopy was performed because of gastrointestinal bleeding. In five of the six cases with bleeding, the telangiopathy (TAP) probably was the bleeding lesion. The treatment of choice is endoscopic, either by electrodestruction with mono-polar current, or by removal with a diathermy snare. Neither barium meal x-rays of the upper gastrointestinal tract in five cases, nor exploratory laparotomy in two cases, revealed the telangiopathies. There has been no rebleeding after the treatment, except for one case in which another TAP of the small or large bowel is implicated.

Intraluminal duodenal diverticulum. Report of a case diagnosed by endoscopy.

Intraluminal diverticulum of the duodenum is a very rare congenital anomaly arising in the second part of the duodenum in close relation to the papilla of Vater. 39 cases have been reported in our available literature. The clinical symptoms and signs are nonspecific, and characteristically the preoperative diagnosis has in all cases been based on the roentgenographic appearance of the lesion. In this paper we present a case diagnosed by duodenoscopy, the radiological studies including hypotonic duodenography revealing no pathology.

Endoscopy-related bacteremia.

A prospective study was undertaken to investigate the frequency of bacteremia after endoscopy of the gastrointestinal tract. In one out of 100 patients undergoing gastroscopy and in one out of 25 patients undergoing coloscopy, positive blood cultures could be demonstrated. In both cases anaerobe lactobacilli were recovered. No bacteremia was demonstrated in 25 patients undergoing endoscopic retrograde cholangiopancreatography (ERCP).
