[Imaging analysis by computer tomography in chronic middle ear otitis diagnosis].

The resource to an image analyzer of coronal plates resulting from computer tomography (CT) of temporal bones suffering chronic otorrhea (tympanosclerosis and cholesteatoma) and its comparison with other plates of normal subjects, allowed the AA. to settle different clear cut densitometric profiles. Hereby a first attempt of objective assessment of radiological images of these pathologies could be contemplated.

Thalamic projections to the anterior suprasylvian and posterior sigmoid cortex: an HRP study of the "vestibular areas" of the cerebral cortex in the cat.

We have confirmed electrophysiologically the existence of an oligosynaptic vestibular projection to the cortex surrounding the rostral end of the anterior suprasylvian sulcus ( ASsS ). However, we failed to confirm a similar projection to area 3a in the posterior sigmoid gyrus. We studied the thalamic projections to each of these cortical regions by injecting small amounts of HRP in the cortex and looking for neurons retrogradely labeled throughout the thalamus. The exact location of the cortical injections was assessed cytoarchitectonically. The heaviest neuronal labeling after injections in the banks of ASsS was obtained in Po (including in this complex GMmc ). A moderate number of projections was found from VPi, VPm and VPl (the labeling in the latter being particularly prominent in a case injected in the lower bank of ASsS ), and also from VL. Occasional labeled neurons were found in the rostro-ventral part of LP. After injections in area 3a in the posterior sigmoid gyrus, which affected to a minor degree either area 3b or 4, many labeled cells appeared in the rostral and dorsal part of VPl, and in the central and lateral parts of VL. Fewer labeled cells were found in VPi, Po and LP. In most cases some occasional labeled cell was observed also in the intralaminar nuclei and in Vm.

Afferent Pathways↗

Chondrosarcoma of the temporal bone and otosclerosis.

Chondrosarcoma constitutes 6% of all primary bone tumors and 11% of malignant primary bone tumors. Nevertheless, in a review of the tumor registry of the University of Michigan covering a period of 50 years, there were only 3 cases involving the temporal bone. A case of a woman with a chondrosarcoma of the temporal bone that was partially resected by means of an infratemporal approach at the skull base is presented. This patient had previously undergone surgical treatment for otosclerosis of the other ear. Several considerations regarding survival factors in this type of tumor are dealt with in terms of the histological features, therapeutic options and anatomic location. The possibility that this lesion may originate from the persistence of the cartilaginous inclusions that some authors consider to be involved in the origin of otosclerosis is discussed.

Inner ear malformations: Mondini's dysplasia.

Advances in imaging techniques are enabling the detection of increasing numbers of inner ear malformations. Mondini's dysplasia, whether alone or in association with other malformations, is one of those most frequently encountered. We report 4 cases of Mondini's dysplasia treated by us, discussing recent embryological and genetic findings.

Osteoma of the promontory: a case report.

Osteomas of the middle ear are rare lesions. We report the case of a 20-year-old male with osteoma originating from the promontory coincident with tympanic membrane perforation. Histopathological diagnosis was performed by means of a superficial curettage during surgery, to avoid a cochlear fenestration. Postoperative closure of the air-bone gap after myringoplasty confirmed the presumed asymptomatic nature of these lesions.

[Image analysis of naso-sinusal pathology with computerized tomography].

Imaging analysis has been applied to study typical cases of nasosinusal diseases, in an attempt to determine its capabilities for CT scan interpretations. Thus, polyps with sinusitis, cerebrospinal fluid occupying sinuses, cysts and bone tumors may be compared in order to obtain objective results that may help in the differential diagnosis in these conditions.

Diagnosis, Differential↗

[Acquired stenosis of the nasopharynx].

A case of scarring stenosis of the nasopharynx occurring in the fifth decade without previous medical history is presented. Transpalatal resection with a prolonged stent, produced patent nasal cavities, but the different studies performed did not establish the etiological diagnosis. A final stage of rhinoscleroma was suspected.

[Inner ear diseases of probable autoimmune origin and its response to steroid treatment].

Cellular and/or humoral immune reactions may be involved in the development of some cochleovestibular dysfunctions long considered idiopathic. In a clinical study of 98 patients, 82 of them with cochleovestibular disorder, a battery of immunologic studies was used. Thirty-one patients received steroid treatment, of which 19 obtained good results; 5 of these patients had positive serum antibodies.

[Synthetic implants in the reconstruction of the ossicular chain].

Experience has revealed a higher rate of functional failure of biocompatible prostheses used in the surgical reconstruction of the middle ear than was reported initially. The cause of these failures was analyzed in a retrospective study of 135 patients who underwent middle ear reconstructive procedures: 47 of them involved implantation of plastic prostheses. Proplast or Plastipore, and had a sufficient follow-up. We evaluated the prosthetic material and middle-ear conditions for which they were implanted.

Cerebrospinal Fluid Otorrhea↗

[Tympanomastoid glomus tumor and persistent stapedial artery].

A case of tympanic glomic tumor associated with vascular malformation of the perifacial plexus is reported. The case and its possible relation with persistent stapedial artery are discussed. The embryological development of the middle-ear vessels and blood supply to glomic tumors were reviewed.

[Experimental autoimmune labyrinthitis: evaluation of various models in guinea pigs].

Evidence of hearing loss in humans with systemic autoimmune disease has suggested the possibility of isolated forms of deafness originated by a autoimmune mechanism. Several authors reject McCabe's (1) proposed clinical description based mainly on response to treatment and have attempted to develop an animal model of autoimmune deafness. A comparative analysis was made of several experimental models of sensorineural hearing loss in guinea pigs developed in our laboratory for the study of this problem.
