Apocrine cells in breast cyst fluid and their relationship to cyst type: a morphometric study.

The morphology of apocrine cells exfoliated in breast cyst fluid (BCF) was studied in 78 BCF samples obtained from 39 premenopausal patients with gross cystic disease who were bearing two simultaneously aspirated cysts. 57/78 samples showed cell clusters suitable for computer-assisted cytometry. This was performed on 5820 cells using a Leitz Texture Analysis System (TAS). We measured the surface areas of cytoplasm, nucleus and nucleolus; we also calculated the nuclear/cytoplasmic (N/C), nuclear/nucleolar (N/n) ratios and the nuclear roundness factor (RF). Cysts were divided according to the cationic pattern of BCF: Type I, K+/Na+ greater than 1.5; Type II, K+/Na+ less than 0.66. The cytometric analysis was made on 47 samples of Type I and 10 samples of Type II. At the light microscope, no difference was apparent between the apocrine cells coming from Type I or Type II cysts. Cytometric measurements showed significant differences for the apocrine cells aspirated from Type I vs. Type II cysts for the mean cytoplasmic area (97.13 +/- 24.28 S.D. mu2 vs. 59.66 +/- 14.90 S.D. mu2, respectively) and the mean nucleolar area (4.35 +/- 0.99 S.D. mu2 vs. 2.75 +/- 0.71 S.D. mu2, respectively). Our data do not allow the inference of apocrine changes in the epithelium lining the cysts simply from the cationic pattern of BCF. The significantly wider cytoplasm and nucleoli of the apocrine cells aspirated from Type I cysts could reflect different functional stages of these particular cells.

Ampullary somatostatinoma. An immunohistochemical and ultrastructural study.

Two new cases of ampullary somatostatinoma are reported. In one case the tumor is associated with an increase in somatostatin-positive cells in the adjacent duodenal mucosa. Both tumors show a predominant glandular pattern with many psammomatous calcified bodies. Such bodies seem to arise by calcium phosphate encrustation of intraluminal cellular debris. The neoplastic cells contain two distinct types of intermediate filaments: the first is located along the plasma membrane and reacts to keratin antiserum; the other, appearing as paranuclear aggregates, reacts to neurofilament antiserum. The neoplastic cells show signs of intestinal differentiation (microvilli, glycocalyceal bodies, filamentous core rootlets) as well as of neuroendocrine differentiation (secretory granules, whorls of neurofilaments).

[Campylobacter pyloridis and gastritis: endoscopic, histological and microbiological correlations].

Biopsy specimens from antral mucosa in 65 consecutive patients presenting for gastroscopy were assessed histologically and microbiologically for the presence of C. pyloridis. Spiral bacteria were found in 63% of cases by Warthin-Starry silver stain and in 72% by culture on blood agar plates with Skirrow's antibiotic supplement. A significant association between the occurrence of C. pyloridis and chronic gastritis was noted; the association is not influenced by the type of gastritis, superficial or atrophic, or the degree of activity of gastritis.

Multiple gastric carcinoids and pernicious anemia: report of a case.

A case of pernicious anemia associated with multiple gastric carcinoids is reported. The neoplastic growth was composed of enterochromaffin-like (ECL) cells, and ECL cell hyperplasia was observed also in hyperplastic polyps, inside the fundic glands and in small nests lying in the lamina propria (microcarcinoidosis). The possible relation between pernicious anemia and ECL cell hyperplasia is discussed.

Multifocal early gastric cancer: morphology and histogenesis.

69 foci from 27 multifocal EGC are compared morphologically with 90 solitary EGC. In the multifocal group there was a preponderance of gross types IIa and IIb, Small and Minute carcinomas, an intramucosal growth pattern and a well differentiated intestinal histotype. Although the differences in gross type and tumor size are related in some measure to the stage of neoplastic growth at the time of diagnosis, the higher frequency of well differentiated intestinal carcinomas in multifocal EGC probably reflects a different histogenesis. This hypothesis seems to be borne out by the greater extent of intestinal metaplasia and the larger number of dysplastic foci detectable in the surrounding gastric mucosa.

Benign and malignant mesothelial lesions of the pleura: quantitative study.

Benign and malignant mesothelial lesions of the pleura are quantitatively studied using a Leitz TAS image analyzing system. The Feulgen fast green staining method allows performing on the same cell measurements of the shape and size of the nucleus, the size of the nucleolus and the content of DNA. Numerical data are studied in a multivariate statistical analysis in order to obtain the probability of the morphometric diagnosis in each case. Results demonstrate that the most important measurements are the percentage of cells with evident nucleolus, the nuclear perimeter and the standard deviation of the DNA content, and that these measurements give a correct diagnosis in all cases.

[Radiologic aspects of the surface mucosa of the duodenal bulb. Correlation with the endoscopic and histological aspects].

Double contrast barium meal, endoscopy and histology of the duodenal bulb were blindly compared in 47 consecutive patients. The agreement in the judgement of normality of the mucosal surface between radiology and endoscopy was 78%. The agreement in the diagnosis of presence or absence of nodularity and thickened folds was respectively 91% and 87%. The findings of nodularity correspond to duodenitis with pyloric metaplasia or to heterotopic gastric mucosa, while the thickened folds correspond to duodenitis with pyloric metaplasia, Brunner's hyperplasia, lymphatic nodular hyperplasia or cystic glandular dilatation. The diagnostic errors in the radiological examinations in respect to endoscopy is quite important in the case of erosions.

Morphometric and topologic analysis of freeze-fractured interphase nuclei.

Interphase rat liver nuclei were studied by freeze fracturing followed by electron microscopic observations. This method permits information on the native organization of the nuclear components in the hydrated state to be obtained. Morphometric analyses, performed with a Leitz Texture Analysis System, gave precise information on the different nuclear components, based on the histograms of their size distribution in heterochromatin, interchromatin and nucleolar areas. The textural characteristics were analyzed by computer to determine the topologic distribution of the solenoid chromatin fibers, the nucleosome filaments and the ribonucleoproteins in the different nuclear domains.

Subarachnoid haemorrhage due to widely metastatising malignant ependymoma.

A case of a widely metastasing malignant ependymoma in a 35-year-old man is reported. The first and most impressive clinical feature was a massive subarachnoid bleed with rapidly developing spinal cord compression due to widespread metastases over the cord and the cauda equina. The cerebral ependymoma was finally shown on a CT scan. Subarachnoid haemorrhage caused by malignant spinal deposits seems to be exceedingly rare. Finally, some problems of classification concerning the malignant forms of ependymoma are discussed.

Small bowel adenocarcinoma and Crohn's disease: report of a case with differing histogenetic patterns.

This paper presents a case of Crohn's disease with associated small bowel adenocarcinoma. A polypoid area of gastric metaplasia was present and showed both fundic and pyloric features, the latter showing increasingly severe dysplasia and infiltrating adenocarcinoma containing many enterochromaffin cells, in which serotonin was demonstrated. At foci of granulomatous inflammation one observed a well differentiated sulfomucin secreting adenocarcinoma. Away from the metaplastic or inflammatory lesions there arose both a poorly and well differentiated sialomucin secreting adenocarcinoma. Multiple microfoci of adenocarcinoma and dysplastic changes of the ileal epithelium were present in the absence of any gross abnormality.

Early gastric cancer: a morphologic study of 41 cases.

The incidence as well as the morphologic features of early gastric cancer observed in 41 cases during the last 5 years are reported. The incidence of early gastric cancer was 9.11% among the total gastric carcinomas seen during the above period. As regards age and sex, the patients suffering with early as well as advanced gastric cancer revealed insignificant variation. With gastric biopsies, the diagnostic accuracy was 88.5%. There were multifocal lesion in 29.2% of cases with a total of 85 foci. Type IIc was the predominant macroscopic lesion, followed by type III. The antrum and the angularis of the lesser curvature were the main sites for early gastric cancer. The intestinal and well-differentiated types of early gastric cancer were the main histologic features seen. Diffuse type and submucosal lesions were more frequent in 6 (14.6%) cases observed with regional lymph node metastases. Our study revealed that there has been a gradual increase in the number of gastric biopsies, early gastric cancer, and the early:advanced gastric cancer ratio.

Radiologic diagnosis of early gastric cancer by routine double-contrast examination.

In 4538 double-contrast examinations of the upper gastrointestinal tract, 8 early gastric cancers (EGCs) were prospectively diagnosed before endoscopy. Four others were false negative cases, but retrospective analysis led to identification of the lesion in 3. The rate of EGC in radiologically diagnosed and verified cancers was found to be 10.6%. Gastric polyp rarely corresponded to EGC, whereas gastric ulcer more frequently corresponded to an EGC. Five of 12 EGCs were multifocal, with 21 satellite foci of carcinoma, of which 3 were probably radiologically identifiable as varioliform erosions. Routine double-contrast study appears valuable for detecting EGC, but the rate of false negative cases indicates that integration of radiologic, endoscopic, and cytologic data as well as accurate histopathologic study of the surgical specimen are needed to diagnose and characterize early gastric carcinoma.

Bone marrow histopathological and molecular changes of small B-cell lymphomas after rituximab therapy: comparison with clinical response and patients outcome.

This study correlates bone marrow changes after Rituximab (RTX) treatment with the clinical characteristics and outcome of 26 patients with small B-cell lymphomas. The percentage, phenotypic profile and clonality pattern of bone marrow lymphoid infiltrate were analysed before and after RTX treatment. Clinical, histological and molecular responses to RTX were correlated to the clinical outcome of the patients. Sixteen out of twenty-six patients obtained a complete clinical remission (CR). A favourable histology--follicular lymphoma (FL), hairy cell leukaemia (HCL) and marginal zone lymphoma (MZL)--was associated with a higher frequency of clinical CR and histological remission (HR), in comparison with mantle cell lymphoma (MCL), chronic lymphocytic leukaemia (CLL) and lymphoplasmacytic lymphoma (LPL). Two patterns of bone marrow HR were observed: 1) complete lymphoid cell disappearance (9 patients); or 2) nodular/interstitial T-cell infiltration (10 patients). Three histological persistence (HP) patterns were observed: 1) persistence of CD20+ small lymphoid cells in 1 patient with MCL; 2) loss of CD20 antigen expression in 4 patients with CLL; or 3) persistence only of clusters of monotypic plasma cells in 2 patients with LPL. CR and HR were strongly correlated. The percentage of lymphomatous infiltrate after RTX was higher in patients who subsequently died of the disease. Molecular response showed no correlations with the further clinical course in 12 patients achieving a complete clinical remission. In conclusion, bone marrow morphological and immunohistochemical analysis with a restricted panel of antibodies is useful to avoid 42% false positive and 85% false negative interpretations. Persistence of monoclonality after RTX might have a role in evaluating the molecular pattern of CD20-negative clones that can emerge after RTX as a tumoral escape to therapy.

[Laboratory diagnosis: correlated health and environmental risks].

Method description and initial results of a study to assess risks to health sector workers and environment due to chemical agents used and waste products generated in diagnostic clinical chemical laboratories, and image diagnostic testing. A survey was conducted of the methods and agents used and their toxicological classification, the number of workers exposed and an analytical profile of the waste produced. The assessment of risk to workers was based on cytogenetic tests (chromosome aberrations and micronuclei); the assessment of environmental risk from waste disposal was based on tests on plant systems.
