Cell volume-induced changes in K+ transport across the rat colon.

The effect of cell swelling and cell shrinkage on K+ transport across the rat colonic epithelium was studied by measuring unidirectional fluxes, uptake and efflux of 86Rb+, a marker for K+. Exposure to a hypotonic medium stimulated the secretory, serosa-to-mucosa flux of K+, whereas exposure to a hypertonic medium inhibited the absorptive, mucosa-to-serosa flux of K+ in the distal, but not in the proximal colon. Neither manoeuvre had any effect on the uptake of K+ across the apical or the basolateral membrane. Cell swelling induced a sustained increase in the apical and basolateral K+ efflux from both colonic segments, whereas cell shrinkage reduced the efflux. Ba2+ (10(-2) mol l(-1)) inhibited the swelling-induced stimulation of the apical, quinine (10(-3) mol l(-1)) that of the basolateral K+ efflux in the distal colon. Incubation of the tissue in Ca2+-free buffer or La3+, which blocks Ca2+-influx into the epithelium, strongly reduced the basal K+ efflux across the basolateral membrane. The same was observed with brefeldin A, a blocker of the transport of newly synthesized proteins out of the endoplasmatic reticulum. Swelling-induced K+ efflux, however, was not reduced. In the presence of colchicine, an inhibitor of the polymerization of microtubules, swelling evoked only a transient increase in mucosal efflux, which, especially in the proximal colon, fell after 6 min to the level of the isotonic control period. These results demonstrate that the cell volume is involved in the regulation of transepithelial K+ transport across the rat colonic epithelium and suggest a role of the cytoskeleton in the control of a part of the volume-sensitive K+ channels.

Water-column, sediment, and in situ chronic bioassays with cladocerans.

In situ bioassays are becoming very popular for laboratory test validation and field extrapolation. Natural conditions, including environmental variables, affect the "behavior" of contaminants and, consequently, their toxicity. This work aimed to develop protocols for in situ chronic bioassays with cladocerans and to assess the ecological relevance of conventional protocols for laboratory testing (water-column and sediment). An aquatic system impacted with acid mine drainage was chosen for the study. At some stations that were contaminated with heavy metals, water-column bioassays did not reveal any toxicity, while sediment and, especially in situ bioassays, revealed a delay in reproduction and a reduction in fertility. Thus, both laboratory bioassays underestimated the actual toxicity of studied sites: sediment, besides being an important contaminant source, became significantly altered with sampling, transport, storage, and manipulation. Therefore, an extra caution is needed when interpreting laboratory results, since the extrapolation to natural systems could be a very problematic step.

Prognostic factors in infants with acute myeloid leukemia.

Little is known about the factors that affect treatment outcome in very young children with acute myeloid leukemia (AML). We therefore analyzed the prognostic impact of various presenting clinical and laboratory features by discrete age group in 299 children with AML treated in four consecutive clinical trials between 1980 and 1997. Differences in presenting features, as well as treatment outcome, were compared between children aged 12 months or less (n = 28) or 13 to 24 months (n = 28) and those more than 24 months of age (n = 243). Children in the two youngest groups (24 months of age or less) had similar presenting features and treatment outcome. Collectively, these 56 children were significantly more likely than the 243 older patients to have M4 or M5 leukemia (70% vs 30%), CNS leukemia (33% vs 22%), the t(9;11) (p22;q23) (18% vs 6%) or other 11q23 translocations (23% vs 3%), and less likely to have Auer rods (2% vs 54%) or the t(8;21) (q22;q22) (0% vs 17%). Among patients aged 24 months or less, two factors independently predicted a favorable prognosis: FAB M4 or M5 leukemia (relative risk of relapse, 0.4; 95% confidence interval, 0.2-0.9) and the t(9;11) (relative risk, 0.3; 95% confidence interval, 0.1-1.0). Leukocyte count and 11q23 translocations other than the t(9;11) lacked prognostic significance. Among older patients, a leukocyte count <50 x 10(9)/l and the presence of the t(9;11) conferred a favorable prognosis.

CNS involvement in children with newly diagnosed non-Hodgkin's lymphoma.

PURPOSE: To determine the frequency of CNS involvement at diagnosis of non-Hodgkin's lymphoma (NHL), to characterize its pattern of presentation, and to determine its prognostic significance. PATIENTS AND METHODS: We reviewed the records of 445 children (1975 through 1995) diagnosed with NHL (small noncleaved cell NHL/B-cell acute lymphoblastic leukemia [SNCC NHL/B-ALL], 201 patients; lymphoblastic, 113; large cell, 119; other, 12). Tumor burden was estimated by serum lactate dehydrogenase (LDH) measurement and reclassification of disease stage irrespective of CNS involvement (modified stage). RESULTS: Thirty-six of 445 children with newly diagnosed NHL had CNS involvement (lymphoma cells in the CSF [n = 23], cranial nerve palsy [n = 9], both features [n = 4]), representing 13%, 7%, and 1% of small noncleaved cell lymphoma, lymphoblastic lymphoma, and large-cell cases, respectively. By univariate analysis, CNS disease at diagnosis did not significantly impact event-free survival (P =. 095), whereas stage and LDH did; however, children with CNS disease at diagnosis were at 2.0 times greater risk of death than those without CNS disease at diagnosis. In a multivariate analysis, CNS disease was not significantly associated with either overall or event-free survival, whereas both serum LDH and stage influenced both overall and event-free survival. Among cases of SNCC NHL/B-ALL, CNS disease was significantly associated with event-free and overall survival (univariate analysis); however, in multivariate analysis, only LDH had independent prognostic significance. Elevated serum LDH or higher modified stage were associated with a trend toward poorer overall survival among children with CNS disease. CONCLUSION: A greater tumor burden at diagnosis adversely influences the treatment outcome of children with NHL and CNS disease at diagnosis, suggesting a need for ongoing improvement in both systemic and CNS-directed therapy.

Entomological fauna from Reserva Biológica do Atol das Rocas, RN, Brazil: I. Morphospecies composition.

Atol das Rocas, the unique atoll in the South-western Atlantic, is located 144 nautical miles (266 Km) northeast from the city of Natal, NE Brazil and 80 nautical miles from Arquipélago de Fernando de Noronha, with geographic co-ordinates 3 masculine51'S and 33 masculine49"W. It's of volcanic origin and coralline formation. The reef is ellipsoid, its largest axis (E-W) is approximately 3.7 km long, and the shortest (N-S) is 2.5 km. Inside the lagoon, there are two islands: the Ilha do Farol and Ilha do Cemitério, which comprehend 7.2 Km2 of emerged area. The Atol das Rocas lodges 143,000 birds, mainly by Sula dactilatra, S. leucogaster, Anous stolidus, A. minuta and Sterna fuscata. Due to their remote location, the islands remain largely undisturbed by the human activities. Aiming to a first characterization of the entomological diversity and the general trophic niches of atoll's entomofauna, three collects were made (1994, 1995 and 1996) utilizing several methods for a wide sample. One thousand six hundred and six insect specimens were collected belonging to eight orders: 1. Coleoptera - 333 individuals of Dermestidae (Dermestes cadaverinus); Tenebrionidae (Phaleria testacea and morphospecies) and Curculionidae (one morphospecies); 2. Dermaptera - 50 individuals of Carcinophoridae (Anisolabis maritima); 3. Diptera - 281 individuals of Ephydridae (Scatella sp. and Hecamede sp.) and Hippoboscidae (one morphospecies); 4. Hymenoptera - 45 individuals of Formicidae (Brachymyrmex sp.); 5. Lepidoptera - 111 individuals of Microlepidoptera (one morphospecies); 6. Mallophaga - 18 individuals in birds (two morphospecies); 7. Orthoptera - 237 individuals of Acrididae (Schistocerca cancellata), Tridactylidae (one morphospecies) and Blattidae (three morphospecies); 8. Thysanoptera -531 individuals (one morphospecies). Also were collected 112 individuals of Arachnida. The taxa of the Order Araneae were represented by the families: 1. Miturgidae (Cheiracanthium inclusum); 2. Salticidae (two morphospecies) and 3. Segestriidae (Ariadna sp.); 4. Theridiidae (Achaearanea sp. and Latrodectus geometricus). For the Order Scorpionida, only samples of Buthidae (Isometrus maculatus) were collected. Through field observations, it was concluded the most insects are detritophagous and/or necrophagous. It is suggested that which the dimension of ecological niches of the insects are a function of the droppings, trash and corpses of birds. A low diversity in the entomofauna of atoll, with its 25 morphospecies, was ascertained.

Involvement of calmodulin and protein kinase C in the regulation of K+ transport by carbachol across the rat distal colon.

The cholinergic agonist carbachol stimulates the apical H+-K+-ATPase and apical as well as basolateral K+ channels in the rat distal colon. The effect of carbachol was tested in the presence of different inhibitors of the Ca2+ signaling pathway in order to characterize the intracellular mechanisms involved. Both carbachol-stimulated Rb+-efflux as well as carbachol-stimulated mucosal Rb+-uptake were dependent on the presence of serosal Ca2+. The Ca2+-calmodulin antagonist calmidazolium (10(-7) mol l(-1)) inhibited the stimulation of mucosal and serosal Rb+ efflux by carbachol. A similar effect had KN-62 (10(-5) mol l(-1)), an inhibitor of the Ca2+-calmodulin-dependent kinase II, suggesting the regulation of basolateral and apical K+ channels by this kinase. Staurosporine (10(-6) mol l(-1)), which potently inhibits protein kinase C, did not alter the effect of carbachol on Rb+ efflux, although the stimulation of apical Rb+ efflux by carbachol seemed to be less prolonged, indicating that protein kinase C is not involved in the regulation of K+ permeability. In contrast, mucosal Rb+ uptake, which is determined by the ouabain- and vanadate-sensitive K+ transport via the apical H+-K+-ATPase, was decreased to nearly one third of control values in the presence of calmidazolium. Both calmidazolium and staurosporine, but not KN-62, prevented the stimulatory action of carbachol on the H+-K+-ATPase, suggesting a synergistic control of this ion pump by both Ca2+-calmodulin and protein kinase C.

A Daphnia magna first-brood chronic test: An alternative to the conventional 21-Day chronic bioassay?

In this study a comparison was made of the results obtained in Daphnia magna chronic bioassays after first-brood release and after 21 days of exposure, using inhibition of normal reproduction and growth as effect criteria and EC10, EC20, EC50, no-observed-effect concentration (NOEC), and lowest-observed-effect concentration (LOEC) as statistical parameters. Test substances were sodium bromide (NaBr), 3,4-dichloroaniline (DCA), cadmium, and parathion. For NaBr, DCA, and cadmium, toxicity evaluated after the first-brood release was similar to toxicity evaluated after 21 days, using reproduction as end point. Parathion did not affect either reproduction or growth. Thus, LC50, NOEC, and LOEC were calculated using mortality as the endpoint for parathion. Results indicate that the period until release of the first brood is sufficient to predict the toxicity of some chemicals to D. magna. Values estimated on the basis of a logistic model (EC10, EC20, and EC50) were more appropriate than NOECs and LOECs for evaluating toxicity of the test substances. Furthermore, classic endpoints used for the evaluation of chronic toxicity (inhibition of normal reproduction and growth) may not be adequate to evaluate the sublethal toxicity of compounds that induce cumulative effects leading to mortality within the test period and causing no observable effects on the reproduction and growth of the species. Endpoints indicative of biochemical stress or effects on specific targets of the test compound may be useful in sublethal toxicity evaluation.

Discriminating the ecotoxicity due to metals and to low pH in acid mine drainage.

Several techniques have been developed to assess the ecotoxicity of contaminated watercourses. Most of these techniques involve chemical alterations of water samples, by diluting it or by adding chelating agents. These changes become particularly severe when assessing the toxicity of samples with very low pH and with high quantities of contaminants. Trying to overcome this problem, a novel toxicity test, specific for acid waters, was previously developed and field validated. The toxicity of acid samples is assessed using the survival time of Ceriodaphnia dubia. During this study, the novel test was applied to a field situation, where an aquatic system is seriously impacted with acid mine drainage. Its efficiency was tested and compared with two classical toxicity tests: the Microtox and the median lethal dilution with C. dubia. The survival time test was performed without adjusting pH and after adjusting pH to a fixed value (pH 2, 3, and 4). At pH 2 and 4 no acceptable results were obtained; at pH 3 it was possible to distinguish the toxicity due to pH from the toxicity due to other toxicants. The test conducted at local pH was able to discriminate toxicity sources only for highly contaminated samples. The toxicity evaluation of acid mine drainage samples was possible neither with the median lethal dilution test nor with Microtox.

Teleconsultation for cooperative acquisition, analysis and reporting of ultrasound studies.

We have developed a teleconsultation system to support cooperation between doctors at different institutions in the acquisition, analysis and reporting of ultrasound studies. All the relevant activities were supported by the application. The system enabled remote supervision of the ultrasound study as it was being performed, cooperative static image acquisition and analysis, as well as report writing. Cooperative analysis of previously acquired studies was also possible. The system is undergoing evaluation in five hospitals in Portugal.

Rheumatic carditis treated with high doses of pulsetherapy methylprednisolone. Results in 70 children over 12 years.

PURPOSE: To report the result of patients treated with IV methylprednisolone divided into three groups and compare their follow-up during the last 12 years. METHODS: Seventy children with active rheumatic carditis (76 episodes) in heart failure Class III and IV (NYHA) were studied. The diagnosis was based on modified Jones' criteria. After ruling out infections and strongyloidiasis, treatment with IV methylprednisolone bolus was started three times a week until the laboratory tests became negative. Patients were divided into 3 groups, according to the time of hospital admittance: Groups 1, 2 and 3, comprising of 40, 18 and 12 children, respectively. RESULTS: Eighteen children in Group 1 (45%) were in their 1st attack: 2 series of pulsetherapy were used in 10 (25%), 3 in 9 (23%) and 4 in 21 (52%). In Group 2, 14 cases (77%) were in their 1st attack: 2 series were used in 7 (39%), 4 in 9 (50%) and 5 in 2 (11%). The echocardiogram showed a flail mitral valve in 12 (66%) of these patients (1 death occurred after mitral valvoplasty). In Group 3, 6 patients needed 5 or more series of pulsetherapy and a flail mitral valve was present in 5 (41%). One child underwent mitral valve replacement while still in the active phase, after 8 series of pulsetherapy, and another died. The number of patients who needed 5 or more series was significantly higher in Group 3. CONCLUSION: There were variations in the presentation and evolution of the cases during these 12 year. The established pulsetherapy protocol continues to be useful to treat severe cases.

Higher frequency of glutathione S-transferase deletions in black children with acute lymphoblastic leukemia.

The genetic polymorphisms in human glutathione S-transferases (GST) M1 and T1 have been associated with race, disease risk, and outcome of some adult cancers. Also, there are racial differences in the incidence and characteristics of childhood acute lymphoblastic leukemia (ALL). Our objectives were to compare the frequency of the null genotype for GSTM1, GSTT1, or both in children with ALL to that in healthy controls, and to determine whether GST genotype was associated with treatment outcome and prognostic factors. We studied GSTM1 and GSTT1 genotypes in somatic cell DNA from black children and white children with ALL and in 416 healthy controls, using a polymerase chain reaction technique. Ninety of 163 (55.2%) white ALL patients and 14 of 34 (41.2%) black patients were GSTM1 null, frequencies not significantly different (P = .19) than healthy controls (53.5% in whites and 27.6% in blacks), although there was a trend toward more null genotypes in black ALL patients. Twenty-three of 163 (14.1%) white ALL patients and 12 of 34 (35.3%) black ALL patients were GSTT1 null, not different (P = .34) than the frequencies in healthy controls (15.0% in whites and 24.1% in blacks). However, the frequency of the "double-null" genotype, lacking both GSTM1 and GSTT1, was higher in black patients with ALL (8 of 34 or 23.5%) than in black controls (3.9%) (P = .0005), but this was not the case in white patients with ALL (10 of 163 or 6.1%) compared to white controls (8.0%) (P = .68). In stratified analyses, the GST double-null genotype was not associated with other characteristics that might differ between whites and blacks with ALL, such as age, T-lineage immunophenotype, presenting white blood cell count, DNA index, or insurance status. The null genotype for GSTM1, GSTT1, or both was not found to be a prognostic factor for disease-free survival or probability of hematologic remission; central nervous system relapse tended to be less common in those with the GSTM1 null genotype (P = .054). The double-null genotype for GSTM1 and GSTT1 is more common among blacks but not whites with childhood ALL. These data suggest that GST genotype, coupled with unidentified additional risk factors, may play a role in risk of childhood ALL in American blacks.

Suitability of test media containing EDTA for the evaluation of acute metal toxicity to Daphnia magna straus.

The acute toxicity of six metals [Hg, Cd, Cu, Zn, Cr(II), and Cr(VI)] to Daphnia magna neonates was evaluated using three different test media (Elendt M7, a complex medium containing EDTA; ASTM hard water and EEC, two simple media free of chelators). The EC50 values, at both 24 and 48 h, obtained for Zn, Cr(II), and Cr(VI) were similar in all of the media tested. Hg was more toxic in ASTM than in M7 and in EEC media. The toxicity of Cd and Cu was similar in ASTM and EEC media and higher when evaluated in M7 medium. Thus, M7 should be used only carefully for the toxicity evaluation of mixtures and effluents containing metals. It is recommended, however, that it be excluded from use in tests evaluating samples of unknown composition, or those known to contain Cu and Cd. For the metals tested in this study, a factor of five applied to each 48-h EC50 would be sufficient in order to attain the respective acute NOECs for immobility.

Predominance and characteristics of Burkitt lymphoma among children with non-Hodgkin lymphoma in northeastern Brazil.

The purpose of this paper was to define the histologic distribution, clinical features, and treatment response of childhood non-Hodgkin lymphoma (NHL) in northeastern Brazil. We reviewed medical records and histopathologic studies of 98 children treated for NHL from 1980 to 1987 at a major pediatric cancer center in Recife, Brazil. Treatment outcome was evaluated in relation to tumor burden (stage and serum LDH) and type of therapy (LSA2L2 vs other multiagent chemotherapy). There was a striking predominance of the small noncleaved cell (Burkitt) subtype, which occurred in 92 of the 98 children and adolescents diagnosed with NHL. Subsequent analyses focused on these patients. The majority (n = 84) had advanced (stage III/IV) disease at diagnosis. The abdomen was the most common site of disease (84 cases); jaw involvement was rare (three cases). Five-year event-free survival (excluding treatment refusals) was significantly better for patients with limited vs advanced stage disease (75 +/- 14% vs 42 +/- 6%; P < 0.04). Elevated serum LDH (>500 U/l) was associated with a poorer outcome (P = 0.008). The type of chemotherapy did not affect EFS (P = 0.95). Only 39% of patients are long-term survivors, reflecting the high rate of septic deaths (25% of patients) and parental refusal/abandonment of therapy (10%). Epstein-Barr virus (EBV) was detected in tumor cells from eight of the 11 cases studied. In clinical presentation, these cases resemble sporadic Burkitt lymphoma, yet in their apparent responsiveness to LSA2L2 therapy and association with EBV, they do not. Childhood NHL in northeastern Brazil is predominantly of the Burkitt subtype, and is associated with clinical features that appear to distinguish it from the endemic and sporadic forms of this tumor. These cases may represent a third or intermediate subtype of Burkitt lymphoma.

METIER (Modular Ecotoxicity Tests Incorporating Ecological Relevance) for difficult substances--III. Effects of medium renewal and use of a carrier on the bioavailability of parathion.

In this work we compared the effects of medium renewal and use of a carrier (ethanol) in the lethal toxicity of parathion to Daphnia magna Straus. Actual concentrations measured in test solutions with and without ethanol were not significantly different, indicating that there is no need for a carrier solution to assist dissolution of parathion, up to a concentration of 5 mg litre(-1). LC50 values at 48 h from bioassays with and without medium renewal were identical, indicating that in our experimental conditions, degradation of parathion was not a significant consideration. Similar 48-h LC50 values were obtained in all tests, with or without ethanol present. Thus, potential interactions between carrier and toxicant had no effect on the lethal toxicity of parathion to D. magna.
