Phagocytosis in patients with sickle cell disease.

Twenty sickle cell disease (SCD) patients, homozygous for sickle hemoglobin, were studied during asymptomatic periods. After clinical evaluation, several peripheral phagocyte parameters were evaluated. These were: ingestion rate, percentage of phagocytes with ingested particles, nitro-blue-tetrazolium (NBT) reduction rate, candidacidal activity; as well as serum levels of total haemolytic complement activity (CH50); third and fourth complement components (C3 and C4). Our data show a significant decrease of all studied phagocytosis parameters in the presence of sickle serum which indicate that asymptomatic SCD patients have basically deficient phagocytosis due to serum factors. Nine of the same patients were also evaluated during and after a painful crisis. Our results indicate that a painful crisis is not associated with further abnormalities of phagocytosis in SCD patients.

[Surgical therapy of intestinal complications in abdominal typhus].

At our clinic 185 patients with severe typhoid fever were treated. In 51 cases severe complications required a surgical therapy: 24 times bleeding of small or large intestine, 14 times perforation of the bowel and 13 times acute typhoid cholecystitis. Lethality of typhoid peritonitis with perforation is nowadays--inspite of specific therapy with antibiotics--still between 40% and 50%; lethality at our clinic was 37.5%. Dangerous complications are bleeding and re-perforation, which require an aggressive surgical procedure.

Susceptibility to environmental pollutants among minorities.

Susceptibility to environmental pollutants involves both biological and nonbiological factors. Individuals belonging to minority groups are much more likely to be subject to a number of these factors. This paper examines biological susceptibility of minorities to environmental pollutants and provides specific examples of susceptibility resulting from: genetic makeup; occupation; other factors such as compromised health status, exposure to mixtures of pollutants, substance abuse, and unemployment; and social inequality of access to health care, education, and communication skills. Recommendations are made for specific actions and for additional studies.
