Spot 14 protein interacts and co-operates with chicken ovalbumin upstream promoter-transcription factor 1 in the transcription of the L-type pyruvate kinase gene through a specificity protein 1 (Sp1) binding site.

In hepatocytes, the amount of the Spot 14 (S14) protein is closely related to the full expression of enzymes involved in the glycolytic and lipogenic pathways. In the present study we address the role played by this protein in the control of transcription of the L-type pyruvate kinase (L-PK) gene in primary hepatocytes. We show that human S14, which by itself does not bind to the L-PK promoter, physically interacts with the human chicken ovalbumin upstream promoter-transcription factor 1 (COUP-TF1) and induces the switch of this factor from a repressor to an activator. However, the enhancing activity of S14 and COUP-TF1 depends on the presence of a proximal GC-rich box (the L0 element) that specifically binds nuclear proteins from the livers of rats fed a glucose-rich diet. Moreover, the L0 element, which strongly binds dephosphorylated specificity protein 1 (Sp1), loses all affinity when this factor is phosphorylated by cAMP-dependent protein kinase. Mutations that affect binding of Sp1 and nuclear proteins to the L0 box also decrease basal transcription and impair glucose responsiveness of the promoter. These results therefore shed light on the mechanism by which the S14 protein, whose concentration rapidly rises after glucose intake, contributes to the full activity of the L-PK promoter.

Polarization measurements in high-energy deuteron photodisintegration.

We present measurements of the recoil proton polarization for the d(gamma-->,p-->)n reaction at straight theta(c.m.) = 90 degrees for photon energies up to 2.4 GeV. These are the first data in this reaction for polarization transfer with circularly polarized photons. The induced polarization p(y) vanishes above 1 GeV, contrary to meson-baryon model expectations, in which resonances lead to large polarizations. However, the polarization transfer Cx does not vanish above 1 GeV, inconsistent with hadron helicity conservation. Thus, we show that the scaling behavior observed in the d(gamma,p)n cross sections is not a result of perturbative QCD. These data should provide important tests of new nonperturbative calculations in the intermediate energy regime.

RT-PCR detection of the Na,K-ATPase beta3-isoform in human heart.

The Na,K-ATPase is a heterodimer composed of an alpha-catalytic and a beta-glycoprotein subunit. At present, three different alpha-polypeptides (alpha1, alpha2, alpha3) and two distinct beta-isoforms (beta1 and beta2) have been detected in human heart. The aim of the present study was to determine whether or not the beta3-isoform of the Na,K-ATPase can be detected in human heart. Using the highly sensitive method of RT-PCR, we here show that human heart expresses the beta3-isoform of the Na,K-ATPase. Given the differences in pharmacological properties of the nine different Na,K-ATPase isoenzymes (containing all combinations of the subunit isoforms), the study of beta3-isoform regulation in human heart may be of interest in understanding the altered response of human myocardium to digitalis therapy during heart failure.

Dynamical relativistic effects in quasielastic 1p-shell proton knockout from 16O

We have measured the cross section for quasielastic 1p-shell proton knockout in the 16O(e,e(')p) reaction at omega = 0.439 GeV and Q2 = 0.8 (GeV/c)(2) for missing momentum P(miss)</=355 MeV/c. We have extracted the response functions R(L+TT), R(T), R(LT), and the left-right asymmetry, A(LT), for the 1p(1/2) and the 1p(3/2) states. The data are well described by relativistic distorted wave impulse approximation calculations. At large P(miss), the structure observed in A(LT) indicates the existence of dynamical relativistic effects.

[Myocardial diverticulum of the left ventricle. Review of the literature apropos of a case].

Left ventricular myocardial diverticular rare, as they represent 0.05% of all forms of diagnosed congenital heart disease. The circumstances of diagnosis are as variable as the multiplicity of symptoms with which they are associated. Consequently, treatments (or very often conservative management) are also varied and a standardized procedure has not been defined in this field, apart from a common sense approach.

[Spontaneous dissection of coronary arteries. Apropos of 2 new cases with favourable course].

The authors report two new cases of spontaneous coronary dissection and discuss the real frequency of this disease as well as their generally reported poor prognosis. One case was a 32-year-old female migraine-sufferer treated by beta-blockers, who developed a rudimentary anterior myocardial infarction. The other case was a 43-year-old man who presented with a threatened anterior myocardial infarction without myolysis documented by cardiac enzymes. Both patients were smokers and both had an excellent outcome, clinically and on stress tests, with a follow-up on medical treatment alone of thirty months for the first case and fourteen months for the second case. These two new cases had a good prognosis, in contrast with the majority of cases published.

[Abdominal pain in children caused by lactose intolerance. Prospective use of the hydrogen breath test].

OBJECTIVES: We performed the breath hydrogen test in children consulting for abdominal pain in order to determine the minimal quantity of lactose required to prove malabsorption with least patient incomfort and to evaluate the effect of an adapted diet in children with a positive test. METHODS: The breath hydrogen test after lactose intake was performed in 109 children (51 boys, 58 girls, mean age 8.2 + 3.2 years, range 3 to 15) seen for unexplained abdominal pain. All these children had complained of abdominal pain daily for at least 6 months and no other cause could be identified. Children with acute or chronic diarrhoea or constipation were excluded. Nineteen children had been transferred from the surgery unit and 6 had been followed by the psychiatry unit. All these children drank milk regularly. RESULTS: The diagnosis of lactose intolerance was established in 83 children (76.1%) on the basis of a hydrogen peak in the breath after lactose ingestion. One-fourth of the children were of French origin and the others had at least one grandparent who was of Mediterranean or African origin. A lactose-free diet was proposed and led to the disappearance of abdominal pain in 24% and a clear improvement in 32%. CONCLUSION: The breath hydrogen test is a simple non-invasive test which allows a selection of children who could benefit from a lactose-free diet although it cannot be used to predict the effect of diet.

Flurbiprofen local action transcutaneous (LAT): clinical evaluation in the treatment of acute ankle sprains.

One hundred and thirty-one male and female outpatients, aged 18-70 yr, with acute pain in the ankle joint caused by a post-traumatic sprain, entered a multicentre, randomised, double-blind, parallel-group, study. The patients were assigned to a 40 mg flurbiprofen patch (n = 65) or a non-medicated (but otherwise identical) control (n = 66), 12-hourly over 7 days, and were assessed at entry and after 3 and 7 days treatment. On day 7, spontaneous pain (the prime efficacy parameter), as evaluated by the patient on a visual analogue scale in the physician's office, showed significant improvement in the 40 mg flurbiprofen patch group compared to control (change from baseline) (p = 0.039), a result corroborated by the evaluation of the periarticular oedema: a reduction of 77.4% was observed in the 40 mg flurbiprofen patch group, compared with 63.8% in the control group (p = 0.025). The other selected efficacy criteria showed changes with a trend in favour of the 40 mg flurbiprofen patch but without statistical significance. Two mild and local adverse events were reported by two flurbiprofen patch patients, but neither patients discontinued the treatment prematurely. Physicians and patients found the flurbiprofen patch to be efficacious and well tolerated. Compliance was excellent in both groups. The efficacy and tolerability of the 40 mg flurbiprofen patch are therefore confirmed in the treatment of acute ankle sprains.

Nosocomial infection surveillance in a vascular surgery unit.

An epidemiological nosocomial infection surveillance program was conducted in the Vascular Surgery Unit of Purpan University Hospital, Toulouse, France, involving 389 patients hospitalized between June 1 and November 30, 1988. The methodology and inclusion criteria used were those of the Centers for Disease Control of Atlanta. Twenty-six patients had 30 nosocomial infections according to these criteria, a prevalence of 6.7% and an incidence of 5.6%. These patients were hospitalized for 485 of a total of 4317 days; bed occupation due to infection was 11.23%. Of the 30 infections, the most common were 13 (43.5%) urinary tract infections and six (20%) operative wound infections. E. coli (8 isolates) and Staphylococcus aureus (7 isolates) were the most frequently encountered offending microorganisms. A case-control study showed that mean hospitalization time was increased by 11 days (p less than 0.001) in infected patients and that antibiotics were used four times as often in these patients (p less than 0.001). Urinary tract infection represented 50% of nosocomial infections in our study. The prevalence and incidence of wound infection was 20% and 8%, respectively. Nosocomial infection always occurred in patients already infected or who were debilitated. Nosocomial infections prolonged hospitalization by 57%.

[Continuous flow measurement of glycemia by the modified Trinder method].

An automatic continuous flow method on autoanalyser of the second generation is described. The peculiarity of this work was the presentation of a manifold glucose oxydase consuming little reagent compared with those at present in daily use. A study concerning the precision, the reproducibility and the contamination is presented. Correlations with a hexokinase technic together with the possibility of working at a wavelength of 340 nm are also studied.
