[Isolated hyperthermic extremity perfusion with vindesine, dacarbazine and cisplatin in the treatment of malignant melanomas].

A three drug combination consisting of dacarbazine, vindesine and CDDP (DVP) was used for isolated extremity perfusion in 26 patients with stage I, II and III melanomas of the limbs. Dosages of 100 mg/l dacarbazine, 0.4 mg/l vindesine and 20 mg Cis-Platinum/l extremity volume have been tolerated without major side effects. All perfusions of bulky tumors or metastases resulted in more than 50% tumor-size reduction (8 patients stage III A, 6 patients stage III AB). In five stage III patients relapse occurred 2, 3, 5, 6 months after isolated DVP perfusion.

[5-S-cysteinyldopa determination in the urine. Its value for monitoring in melanoma patients].

In case of uncharacteristic patient complaints or slightly pathologic BSR or liver enzyme levels in patients, suffering from melanoma, the determination of 5-S-cysteinyldopa in the 24-h-urine may be of great diagnostic value. If the complaints and the pathologic values are early symptoms and signs of generalized metastases being clinically not yet manifest, the excretion of 5-S-cysteinyldopa in the urine is often increased. In such cases, a thorough and possibly invasive search for metastases is indicated.

Cysteinyldopa↗

[5-S-cysteinyldopa in the urine - a "tumor test" for malignant melanoma? Comparison with the usual laboratory examinations].

In a total of 1,828 determinations, urinary excretion of 5-S-Cysteinyldopa was studied over a period of three years in 384 patients treated for melanoma or with metastases of malignant melanoma. By serial investigations the excretion of 5-S-Cysteinyldopa was compared to the course of the disease. In the case of small and circumscribed metastases which could be eliminated by surgical treatment, the excretion of 5-S-Cysteinyldopa remained normal. When the disease became generalized, an increase of the urinary excretion of 5-S-Cysteinyldopa prior to the clinical manifestation of the metastases was observed in only four out of 26 cases. In the remaining cases, the increase of 5-S-Cysteinyldopa coincided with the manifestation of metastases, or the excretion of the substance became pathological when the metastases were already conspicuous. In five patients, the urinary excretion of 5-S-Cysteinyldopa remained normal inspite of widespread disease. Therefore, its diagnostic value seems to be similar to that of the "common" laboratory investigations the results of which are only pathological when the disease has already become generalized. Our investigations demonstrate that serial investigations of the urinary excretion of 5-S-Cysteinyldopa only rarely indicate melanoma metastases prior to their clinical manifestation. In cases of early metastasing melanoma, all common laboratory investigations are of limited value. BSR and GGT levels which become pathological very early in the course of the disease are so sensitive that slightly pathological levels may be ambiguous. In these cases, however, pathological levels of 5-S-Cysteinyldopa most probably will indicate a widespread disease.

[Familial malignant melanoma and atypical disseminated moles (BK-mole syndrome)].

Report of a family with malignant melanomas and multiple atypical pigmented moles in three siblings. A 29 year old caucasian woman had a nodular melanoma. Twelve years ago, at the age of 23, her elder brother had been suffering from a melanoma with a solitary cerebral metastasis. Following radical surgical treatment, he has been well since that time. Their sister died with 21 years after inadequate treatment and generalisation of a melanoma in her temporal region. All patients had multiple atypical nevus cell nevus-like pigmented moles with irregular bizarre configuration, especially in the upper parts of the trunk. These findings correspond to the BK-mole syndrome. This heritable syndrome is not frequent, but important because of the necessity to keep all relatives of BK-mole patients under continuous dermatological control and to remove each suspicious lesion early enough.

[Attempt at applying statistical methods to delimit melanoma types].

By means of the "multivariate variance analysis" ("MANOVA"; Friedrich and Olfert, 1977), we tried to differentiate the histologic types of malignant melanomas also by quantifiable criteria. The investigation depended upon data from 411 patients. Difficulties arose from incomplete data material. But in principle, the indication of this method is given in such problems. If the number of cases could be augmented, the method would be helpful as a support for diagnostic quality control.

[Accuracy of the clinical diagnosis in malignant melanomas].

Histologic findings and clinical data of 411 patients with malignant melanomas treated in 1952 up to june 1977 reveal an improvement of diagnostic accuracy. Within the last years, fewer patients were referred to the clinic with extensive tumor growth or with metastases. A change from the levels of invasion V and IV to III is evident. Further improvement to level II and I with a corresponding smaller tumor thickness is appearing in outlines.
