Preventing hairline elevation in endoscopic browlifts.

An endoscopic-assisted method of brow elevation using galea aponeurosis plication with direct suture fixation is presented. This technique does not elevate the hairline, nor is any skin or scalp resected. Dissection is easy because it is limited to the subgaleal plane of the forehead.

Endoscopic management of glabellar frown lines.

Resection of the corrugator muscles has been done for years as a routine adjunct to open coronal browlifts. The new endoscopic procedure described here permits endoscopic resection of the corrugator muscles through three 5-mm hairline incisions in those patients who do not need or want a browlift. The primary benefit is that of reducing the subconscious tendency to frown that some patients exhibit whenever concentrating or conversing. Although patients typically seek plastic surgery procedures to improve how they see themselves, these patients seek a reduction in the subconscious frown to improve how others see them and react to this unattractive facial expression.

Measurement of capsular contracture: the conventional breast implant and the Pittsburgh implant.

At present, there is no accurate, reliable method of experimentally measuring capsular contracture. This study had four goals: (1) to define the parameters of capsular contracture employing principles of biomechanics of soft tissues, (2) to develop laboratory techniques to measure the parameters, (3) to design an implant that mechanically impedes the process of encapsulation, and, (4) to test this implant against a conventional one. We have developed a breast implant (the Pittsburgh implant) with an altered surface topography. Its silicone shell is punctuated by projections 1 mm in height and 1 mm in diameter. Two techniques were devised to measure contracture. The first involved measuring the force deformation along a coronal axis. The second involved measuring hydrostatic pressures within the implant resulting from the injection of known quantities of saline. Measurements were performed in vivo on 36 animals. By both force and pressure measurements, the Pittsburgh implant showed less capsular contracture (p = 0.12 and 0.012, respectively). Histology revealed that the prototype surface alters the linear arrangement of myofibroblasts and redirects the laminar collagen into a waveform pattern. We conclude from this experimental study that an altered surface topography may serve as a means of rendering a capsule less mechanically effective. We feel that the proposed methods can be used in the laboratory to characterize the extent of capsular contracture.

Treatment of mallet finger due to intra-articular fracture of the distal phalanx.

When a mallet finger deformity results from an intra-articular fracture of the distal phalanx comprising more than one third of the articular surface, an accurate reduction of this fracture is necessary to prevent secondary degenerative arthritis. A technique for open reduction is described in which the distal interphalangeal joint is exposed by dividing the extensor tendon and permitting a precise reduction of the fracutre fragment. Elective division of the extensor tendon had not compromised the results.

Reducing the subconscious frown by endoscopic resection of the corrugator muscles.

Certain patients subconsciously frown whenever concentrating or conversing. This hyperactivity of their corrugator muscles gives them an unattractive mean or angry look. Prominent glabellar frown lines eventually result from making this facial expression repeatedly. If such patients undergo an open coronal browlift, their corrugator muscles are resected routinely. However, patients who do not need or want a browlift seldom agree to a coronal incision just for access to their corrugators. To address this problem, the author devised an endoscopic operation to approach and resect the corrugators through three 5-mm hairline incisions. In this initial series, the subconscious tendency to frown when concentrating or conversing was reduced to about 20% of the preoperative level. This resulted in a more pleasant facial appearance.
