[Comparison of nasal mucociliary clearance before and after operation in chronic sinusitis].

Nasal mucociliary clearance is a first line of defence mechanism of the respiratory system against inhaled particles. It transports particles depositing into the nasal passages to the pharynx to be swallowed or expectorated, thus protecting the respiratory system from injury. Clearance function is affected by many factors such as the chronic inflammation of the paranasal sinuses. A group of 13 healthy volunteers (controls) and a group of 17 patients suffering from chronic sinusitis was investigated in the Department of Otolaryngology of Taichung Veterans General Hospital between June and October of 1990. A radioisotope method was used to measure the velocity of nasal mucociliary transport in the participants. This involved the use of radioisotope Tc99m combined with albumin particles and measurements were made in one nasal chamber at a time. The sinusitis patients were then surgically treated and they all improved. In all 25 tests were made in the control group and the mean velocity of their mucociliary transport was found to be 4.56mm/min. In the patients 28 measurements were made and their mean velocity was found to be 2. 67mm/min before the operation and 4.03mm/min after the operation. The difference between pre- and post-operative velocities was statistically significant as was the difference between the pre-operative velocities of the patients and those of the controls. The difference between controls and patients after the operation was not significant.

[Functional endoscopic sinus surgery].

Treatment of chronic paranasal sinusitis (CPS) by functional endoscopic sinus surgery refers to removal of the pathologic tissue which interferes with the physiologic function of the sinuses in the ostiomeatal complex under direct vision by use of endoscopes to restore the ventilation and drainage of the sinuses. While the ventilation and drainage of the sinuses become normal, the lesions of sinus mucosa will recover spontaneously and the function of the sinuses will be restored. From May to August 1988, functional endoscopic sinus surgery was performed in 47 cases based on the operative methods described by Dr. Stammberger. We excised the uncinate process of middle meatus and entered the infundibulum. The pathologic tissue in the frontal recess was removed first. The anterior ethmoid sinus and posterior ethmoid sinus were cleared depending on the excisable extent of the lesion and the sphenoid sinus wound be opened if necessary. Middle meatal antrosomy was finally performed to enlarge the obstructed ostium of the maxillary sinus. Seventeen of them met the criteria: 1. a history of the disease for more than 3 months and not improving or recovering after medical treatment 2. no past history of surgical treatment for CPS 3. the etiology is not dentigerous or septal deviation. Because one patient lost to follow up, sixteen patients constituted the materials of this study. About 80% of patients were improved or asymptomatic. No serious complication occurred in all patients and the most common one was the adhesion between the middle turbinate and the lateral nasal wall.

Etiologic studies of the 1983 and 1984 outbreaks of epidemic diarrhea in Guangxi.

Studies are reported on two outbreaks of epidemic diarrhea in China involving 19,007 patients. The first outbreak occurred in the northern part of the Guangxi Autonomous Region and extended south from April through September 1983. In June 1984 a second outbreak of 6,570 cases occurred in Guanyang, a county in the northern part of Guangxi. 125 fecal samples from the two outbreaks were cultured for bacteria; all of the samples except one were negative. Rotavirus particles were detected in 44.1% (15/34) of stool specimens examined by immune electron microscopy. The two outbreaks appeared to be caused by non-group-A rotaviruses, based on the pattern of electrophoretic migration of the RNA genome segments in polyacrylamide gels and on the lack of a detectable common group antigen(s) when tested by an enzyme-linked immunosorbent assay for group A rotaviruses. Complement-fixation tests also indicated that 37.5% of convalescent-phase sera from patients produced antibody to the isolated rotavirus. The total molecular weight of the RNA segments in the new Guangxi strain was calculated to be 10.69 X 10(6), with the 11 genome segments having molecular weights (X 10(6)) of 2.19, 1.83, 1.56, 1.53, 0.82, 0.81, 0.60, 0.46, 0.35, 0.30, and 0.24. This study also reports comparative detection rates for direct and immune electron microscopy and for PAGE. PAGE proved to be the most sensitive detection method for these new rotaviruses.
