Oral ranitidine for prophylaxis against Mendelson's syndrome.

The effect of the new H2 receptor antagonist ranitidine on gastric pH was studied using a double-blind technique in 36 patients undergoing elective surgery, 18 of whom were given 150 mg of ranitidine orally the night before and on the morning of surgery. The incidence of gastric residue pH higher than 2.5 (p less than 0.01) was significantly greater in patients given ranitidine than in 18 untreated control patients. The mean volume of gastric aspirate in the treated group as 6.7 ml (range 2 to 20 ml) compared with the control group 15.6 ml (range 2 to 44 ml). The higher potency, longer duration of action, and fewer side effects of ranitidine compared with cimetidine suggest that this drug may have clinical advantages over other H2 receptor antagonists.

Marcus Gunn Syndrome associated with an unusual oculo-cardiac reflex.

The Marcus Gunn Syndrome is discussed. It is postulated that an atypical oculocardiac reflex is easily initiated in these patients so that precautions must be taken for its prevention and early detection. Marcus Gunn first described the syndrome in 1883. His patient had a congenital ptosis of the left upper eyelid. Paradoxical lid retraction was evoked by movement of the jaw. Consequently the syndrome also became known as the 'jaw-winking' syndrome. Other varieties of the Marcus Gunn syndrome have also been described. The pathogenesis of this syndrome seems to be the presence of abnormal central connexions between the trigeminal and oculomotor cranial nerves which supply the external pterygoid and the levator palpebrae superioris muscles. The importance of recognition of the oculocardiac reflex in patients undergoing eye surgery under anesthesia is well known to anaesthetists and to eye surgeons. The occurrence of this reflex in patients undergoing surgery on the eyelids has been reported previously. This paper describes a patient with the Marcus Gunn Syndrome who underwent three operations on the left upper eyelid and who exhibited unusual manifestations of the oculocardiac reflex.

Complications associated with surgery in the flank position for urological procedures.

The flank position is frequently used for certain urological surgical procedures and it may lead to several complications. a retrospective study was undertaken on charts of patients in the flank position who underwent urological surgical procedures during 1977 and 1978. Hypotension and post operative pulmonary complications occurred in 33.3% and 19% of these patients respectively. Various recommendations are made to prevent these complications from occurring.

Doxapram in respiratory failure. A case presentation.

The value of Doxapram in managing patients in respiratory failure due to an exacerbation of their chronic pulmonary disease is still not widely accepted. Controlled clinical trials are clearly necessary to determine whether Doxapram will improve the morbidity and mortality in these patients. However, it does seem that Doxapram is the most suitable of the respiratory stimulants available and this drug should be seriously considered when managing these difficult patients.

Infantile lobar emphysema. A case report.

A case of infantile lobar emphysema is presented. The period of weaning the infant from the ventilator is discussed, together with some suggestions for the future management of similar cases. Respiratory therapists were used to keep constant surveillance on ventilator function and inspired oxygen concentration. The use of dummies or pacifiers by infants may be detrimental in some instances. Excess humidification may increase airway resistance.
