Image guided respiratory gated hypofractionated Stereotactic Body Radiation Therapy (H-SBRT) for liver and lung tumors: Initial experience.

To evaluate our initial experience with image guided respiratory gated H-SBRT for liver and lung tumors. The system combines a stereoscopic x-ray imaging system (ExacTrac X-Ray 6D) with a dedicated conformal stereotactic radiosurgery and radiotherapy linear accelerator (Novalis) and ExacTrac Adaptive Gating for dynamic adaptive treatment. Moving targets are located and tracked by x-ray imaging of implanted fiducial markers defined in the treatment planning computed tomography (CT). The marker position is compared with the position in verification stereoscopic x-ray images, using fully automated marker detection software. The required shift for a correct, gated set-up is calculated and automatically applied. We present our acceptance testing and initial experience in patients with liver and lung tumors. For treatment planning CT and Fluorodeoxyglucose-Positron Emission Tomography (FDG-PET) as well as magnetic resonance imaging (MRI) taken at free breathing and expiration breath hold with internal and external fiducials present were used. Patients were treated with 8-11 consecutive fractions to a dose of 74.8-79.2 Gy. Phantom tests demonstrated targeting accuracy with a moving target to within +/-1 mm. Inter- and intrafractional patient set-up displacements, as corrected by the gated set-up and not detectable by a conventional set-up, were up to 30 mm. Verification imaging to determine target location during treatment showed an average marker position deviation from the expected position of up to 4 mm on real patients. This initial evaluation shows the accuracy of the system and feasibility of image guided real-time respiratory gated H-SBRT for liver and lung tumors.

Long term follow up of serostatus after maternofetal parvovirus B19 infection.

BACKGROUND: Maternofetal parvovirus B19 infection may result in fetal hydrops or abortion. Chronic infection has been associated with long term complications (polyarthritis, persistent aplastic anaemia, hepatitis). In pregnancy maternal immunosuppression caused by a TH2 dominant response to viral antigens has been observed. There is little information on long term reactivity to intrauterine infection. AIMS: To assess the serological status in children and their mothers after maternofetal parvovirus B19 infection and development of fetal hydrops. METHODS: A total of 18 children and their mothers, and 54 age matched control infants were studied. Main outcome measures were parvovirus B19 DNA, specific IgM and IgG against the virus proteins VP1/VP2, and NS-1 in venous blood. RESULTS: Parvovirus B19 DNA and antiparvovirus B19 (IgM) were undetectable in all sera. A significant larger proportion of maternal sera compared to study children's sera contained IgG against the non-structural protein NS-1. Mean levels of VP1/VP2 IgG antibodies were significantly lower in the children than in their mothers (48 (36) v 197 (95) IU/ml). There was no history of chronic arthritis in mothers and children. Five women had subsequent acute but transient arthritis postpartum, which was not correlated with antibodies against NS-1. CONCLUSIONS: Serological evidence of persistent infection after maternofetal parvovirus B19 disease could not be detected. Increased maternal prevalence of anti NS-1 (IgG) and increased levels of antiparvovirus B19 (IgG) may reflect prolonged viraemia compared to fetal disease.

Congenital intrapulmonary bronchogenic cyst in the neonate--perinatal management.

Approximately 50% of all congenital lung malformations are pulmonary and mediastinal bronchogenic cysts (BC). Therefore, their diagnosis and management is of clinical importance. Usually asymptomatic in the first months of life, bronchogenic cysts are frequently clinically inapparent even adulthood. Early diagnosis and elective surgery can prevent late complications such as pneumothorax, pulmonary hypertension, and recurrent infections; prognosis after surgery is excellent. If mediastinal shifting is present, fetal thoracocentesis is indicated to prevent cardiovascular insufficiency. We report a case of a prenatally diagnosed intrapulmonary BC of the right lung. Following in utero thoracocentesis of the cyst and transient spontaneous regression postnatal onset of severe clinical symptoms due to rapidly developing hyperinflation and mediastinal shifting within the first days of life required early surgical intervention.

Parvovirus B19 infection in pregnancy.

Transplacental transmission of human parvovirus B19 (B19 virus) to the fetus is an important cause of intrauterine death, abortion, stillbirth, and nonimmune hydrops fetalis. Adverse outcome of pregnancy can occur after symptomatic and asymptomatic maternal infection. Only rare cases of congenital malformations and fetal disease in live-born infants have been associated with intrauterine B19 virus infection. Laboratory results obtained from paired maternal and fetal cord blood samples indicate that a reliable diagnosis of fetal B19 virus infection should be based on detection of parvovirus B19 DNA.

[Primary hyperparathyroidism in the 3d pregnancy trimester].

HISTORY AND FINDINGS: A 24-year-old pregnant woman had to be hospitalized in the 33rd week of pregnancy because of premature contractions and clinical signs of pyelonephritis. She had a history of nephrolithiasis. Laboratory tests showed a total calcium of 3.6 mmol/l, hypophosphataemia of 0.59 mmol/l and an increased parathormone level of 420 ng/l. Ultrasonography demonstrated a large parathyroid adenoma, confirming the diagnosis of primary hyperparathyroidism. COURSE AND TREATMENT: Despite several recommendations of conservative treatment in the literature it was decided to perform a parathyroidectomy, which was done without complication during the 35th week of pregnancy. A healthy, mature boy was born at the beginning ot the 41st week: at no time did he show any signs of hypoparathyroidism. CONCLUSIONS: Surgical treatment of hyperparathyroidism is a reasonable and possible choice even in the 3rd trimester, because it allows regeneration of the fetal parathyroid. However, this decision must be individualized, in relation to the period of pregnancy and any progression of the disease.

[Doppler ultrasound studies of breast tumors with the continuous wave technique].

The aim of the study was to ascertain the reliability of continuous-wave Doppler signals in the diagnosis of breast cancer. CW-Doppler findings in a series of 63 cases of palpable breast tumors with histologic confirmation (24 malignancies and 39 benign tumors) are reported. Depending on the parameters used -20 -24 of 24 malignant nodes (83%-100%) and 20 -24 of 39 benign lesions (51%-62%) were correctly identified. Even benign tumors showed significant differences in systolic peak and diastolic frequencies compared to the contralateral breast. The Resistance-Index (RI) did not allow the differentiation of lesions when measured on one side only. Comparing the Resistance-Indices of both breasts helped improving the specificity of CW-Doppler-sonography to 89.7% and the positive predictive value to 73.3%. The CW-Doppler-sonography with the relevant parameters of diastolic and systolic peak frequency and the RI is considered a useful tool in the diagnosis of breast cancer.

[Status of fractionated abrasion and vaginal endocervical cytology in diagnosis of endometrial cancer].

The predictive value of cervical cytology and uterine curettage in 150 endometrial carcinoma was investigated. Cytology proved negative in 56% of all carcinomas and in 42.2% of those in stage 2 or 3. Uterine curettage was found to have a false-negative rate of 6%, endocervical curettage of 5.3%. By contrast, we found a high false-positive rate of 70.8% for endocervical curettage. 13.2% of the 129 endometrial carcinoma stage 1 had a false-positive endocervical fraction. No residual disease was found in 4.7% of all 150 hysterectomy specimens. Negative cervical cytology seems to be a reasonably good predictor of actual cervical involvement, if a positive endocervical curettage is to be interpreted.

[Doppler ultrasound differentiation of benign and malignant breast tumors].

Before surgery we studied the blood flow in and around 59 breast tumours (35 malignant and 24 benign) and investigated a correlation with the histology. The most decisive factors were the maximum end-diastolic frequency B (probability greater than magnitude of z = 0.0009) and the mean frequency F mean (probability greater than magnitude of z = 0.0017). The maximum systolic frequency A, the resistance index Ri, the diastolic angle W and the pulsatility index Pi showed less significant differences between malignant and benign histological types. In a retrospective survey, we tried to confirm the definitive histology by our Doppler results. In this, we failed in 17.14%--of malignant cases and in 33.33% in the cases of benign tumours.

Analysis of upper extremity movement in four sitting positions: a comparison of persons with and without cerebral palsy.

The purposes of this study were to compare the arm movements of persons with and without cerebral palsy and to determine if the alteration of the seat angle of a chair affected the quality of those movements. Twelve subjects--3 men and 3 women with spastic cerebral palsy and 3 men and 3 women without any known anomalies that could affect arm movements--were studied. The number of movement elements constituting a reach was used to measure the quality of movements. The findings demonstrated significant differences in the number of movement elements used by the subjects with and without cerebral palsy regardless of position. No significant differences could be attributed to the seating positions. Implications are discussed in relation to the method used in the analysis of movements and the effect of the findings for research and treatment.

[Vertical tumor thickness as an estimate of actual tumor volume].

According to Cavalieri, the volume of any body-irrespective of its shape-can easily be estimated on the basis of the area of slabs of known thickness. We made use of this old principle in order to determine the correlation between the volume of skin tumors and their maximal vertical thickness. Investigations of a total of 30 benign and malignant tumors of the skin of various shapes showed a correlation coefficient of r = 0.77. Therefore, the widely used determination of the vertical thickness of malignant melanomas can be regarded as a good estimator of the tumor volume and seems to be appropriate for histological tumor staging. Together with high frequency ultrasound, Cavalieri's method allows a simple and rapid pretherapeutical in-vivo determination of the tumor volume. However, only prospective studies can reveal whether the determination of the tumor volume is of superior prognostic significance to the established techniques.
