[Studies about the therapy of tuberculosis in East Germany (former GDR)].

On the basis of a questionnaire the therapy of 2285 patients with tuberculosis was evaluated for the years 1987-1989. The data of the GDR National Tuberculosis Register (9827 patients) were used for statistical comparison. 85% of the patients had a pulmonary tuberculosis, 947 patients (41% of all) excreted tubercle bacilli in the sputum. The average duration of treatment was 31.8 weeks, 94.7% of the patients were hospitalised for treatment for an average time of 12.4 weeks, the following outpatient treatment was 19.4 weeks in average. The initial phase (daily-treatment) in all cases of tuberculosis was 8.6 weeks, the follow-up phase was 23.2 weeks (all data are averages). The intermittent outpatient drug administration was made in 95% of cases under nurse control in outpatient departments. The initial treatment was started in 92.1% with 3 or 4 drugs (H/R/Z/S or E). Isoniacid and rifampicin were prescribed in 89.7% and pyrazinamide was used as a 4th drug. In the follow-up phase the combination isoniacid and rifampicin was employed intermittent in 75.7%. The delaytime for the evaluated cases was 38.8 days in average. The result of study shows, in most cases the patients were treated according to the recommendation of therapy. The clinical stay of patients is too long.

Public schools welcome students with disabilities as full members.

As noted in a parent newsletter, the inclusion of children and young adults with disabilities with other students in our schools will result in "an ever widening circle of people who believe that disabilities are a part of life, that people with disabilities are a part of [our] natural environment who should not be isolated, and that people with disabilities can have a positive effect on non-disabled people and the general community". All students can benefit from regular class activities even though the individual goals for some may be quite different. Such efforts are not always easy to implement, and depend on adequate support for both students and school personnel--but the benefits for all students are substantial. As one regular education teacher noted: "Above all, we want our kids to show concern for others, now and in their future days. Typical kids in integrated classrooms benefit because these goals are as much a part of their daily classroom experience as the 'academics'. In reality, I believe the academics are enhanced because of these goals." And this teacher observed: "I believe that all kids have a right to be in inclusive classrooms--to be educated with their peers. When everybody in a class is considered equal, some very great things will happen."

[Tuberculosis among immigrants in East Germany].

The number of new cases of tuberculosis decreased hardly during the last seven years in the GDR. This is especially seen in the age group of 20 to 29 years. The percentage of immigrants with tuberculosis in the age group named above increased from 24% (1982) to 41% (1988). Special attention is to pay to the case finding of tuberculosis among the GDR citizens at the age of 20-29 years as the number of new cases of tuberculosis did not change in the last years.

[Status, development and trends in the epidemiology of tuberculosis in East Germany since 1982].

The article is an epidemiologic review of the situation in the German Democratic Republic with regard to tuberculosis. Conclusions for the future strategy to fight tuberculosis are drawn. During the past few years the drop in the incidence of tuberculosis, especially of bacillary pulmonary tuberculosis, is no longer as marked as it had been previously. Today, tuberculosis is a disease of middle and advanced age, under present prevailing conditions. The strategy for the discovery of new cases of tuberculosis must be adapted to these facts. For surveillance of tuberculosis during the coming years it will be necessary to explore new methods of case finding and of the discovery of sources of infection, screening of high-risk groups and examination of patients with bronchopulmonary signs and symptoms.

[Position, development and trends in the epidemiology of tuberculosis in the GDR 1982-1987].

On the occasion of the commemoration of the 100th anniversary of the discovery of the tubercle bacillus by Robert Koch, in 1982, it could be stated that in GDR, like in other developed countries, tuberculosis is not any more a serious problem of public health but has changed to a much reduced endemic disease. After evaluation of the most important parameters of the tuberculosis problem, morbidity, mortality, and infection rate, conclusions are drawn for the future strategy of tuberculosis control. After the rapid decline of the incidence of new tuberculosis cases during the seventies, also during the last 5 years the epidemiology figures were favourable. But it has to be stated, that during the last years the decrease of the incidence of new cases of all forms of the disease was less pronounced, and with bacteriologically confirmed pulmonary tuberculosis a stagnation of the incidence occurred in the last four-years-period. Today, under our circumstances, tuberculosis is mainly a disease of middle and higher age. Therefore, the strategy of detection of new cases has to be adapted to and developed according to these conditions. New methods of case finding, detection of infection sources, screening or risk groups, and the investigation of patients with bronchopulmonary symptoms must be developed. Also during the next years, strict surveillance of tuberculosis remains mandatory.

[Lung damage caused by paraquat. 1. Histologic, histochemical and pathophysiologic findings in rat lungs following acute paraquat poisoning].

A single dose of 0.05 mMol paraquat per kg body weight produced severe lesions in alveolar epithelial cells of type II and the capillary endothelial cells of rat lungs. The maximum damage pattern was seen after 48 hrs. Our results are in contrast to the general opinion in the literature, that paraquat leads to selective lesion of alveolar epithelial cells of type II in the lungs. The detection of the damage of the lung cells by semiquantitative histochemical investigations and the PO2 measurement as indicator of respiratory insufficiency were parallel in the time course. The right ventricular systolic pressure showed normal values.
