Application of impediometry to rapid assessment of liquid culture media.

The impedance method provides as unique opportunity to determine microbial activity and kinetics. Since the metabolic processes depend on the nature and quality of the culture medium, impediometry allows the assessment of liquid culture media. Impedance microbiology represents an approach to quantitative microbiology. We investigated the influence of pH, composition and variation of the amounts of industrially made dry media, overheating during the dissolving or sterilisation processes, and qualitative differences between batches of the same culture medium. Using glucose broth as an example, we showed that impediometry allows quantitative, microbial assessment of culture media. Inaccurate preparation of the culture medium could be detected quickly by the use of impediometry. The method is very simple to perform, requires no sample preparation, allows rapid assessment of liquid culture media, and interprets results automatically with the aid of a microcomputer.

[Reference values for spirometry exemplified by FVC and FEV1 parameters--comparison of normal values of the Pathophysiology of Respiration Section and the European Society for Coal and Steel].

Reference values published by the Section Pathophysiology of Breathing in the Society for Pulmology and Tuberculosis of the GDR and reference values of European Community for Coal and Steel (EGKS) were compared on example of FVC and FEV1. It was found a relative well accordance of reference values of FVC and FEV1 in younger and middle-aged men of a height from 160-200 cm (between -5 to +15%). In women with a body height of 150 to 200 cm a well correlation for FVC and FEV1 is also given (-10 to +10%). Reference values are showing a more important difference with increasing age and independent of sex. There are problems concerning the reference values in male and female lower than 155 cm body height.

[The use of impedance measurements in medical microbiology].

The versatility of the impedance method makes it a valuable tool in clinical microbiology. Impedance can be used to estimate total number of bacteria or to determine bacterial susceptibility to antimicrobial agents. The impedance method provides a unique opportunity to determine microbial activity and kinetics. Impedance microbiology represents a different approach to qualitative and quantitative microbiology. The method is very simple to operate, requires no sample preparation, allows rapid detection of bacterial growth, and interprets with microcomputer results automatically.

Carcinoid of the cervix: natural history and implications for therapy.

Carcinoid tumors rarely occur in the cervix. We present a patient with FIGO stage IB disease who developed brain metastases 4 years after initial diagnosis and treatment. The clinical course of a collected series of patients with carcinoid of the cervix is reviewed. Even patients with low stage lesions frequently die of disseminated disease. Adjuvant chemotherapy may have a role in the treatment of such patients.

[Effects of nitrate and ascorbic acid on carcinogenesis in the operated rat stomach].

In the last few years, a great deal of attention has been paid to the hypothetical relationship between high nitrate intake, N-nitroso-compounds and gastric cancer. This long term study deals with the effect of nitrate and ascorbic acid contained in drinking water on the development of cancer in operated stomachs of rats without additional application of a carcinogen (tumor model). A total of 147 F1-Hybrids of WELS/Fohm X BD IX--rats were subjected to stomach operation (Gastroenterostomia antecolica anterior, truncular vagotomy and pyloroplasty). Carcinomas in stomach occurred within 33 weeks (post operationem). The surgical procedures altered the gastric flora, resulting in an increase and predominance of nitrate-reducing bacteria. The maximal tumor rate (10 of 13 rats) occurred in rats with gastroenterostomy and additional nitrate exposition. It may be possible, the nitrate is a precursor of gastric carcinogenesis. But we could not give any significant evidence of this fact. The addition of ascorbic acid to the drinking water lead to a suppression of the development of tumors. Our experience with the tumor model "operated stomach" underline the multifactorial genesis of stomach cancer in rats.

[Helpful adjustment--helpless accommodation. Physicians and the administration of Lower Saxony during the extermination of psychiatric patients at the time of national socialism].

After a brief introduction describing the development and organisation of the National Socialist extermination programmes to eliminate patients suffering from mental diseases, a description is given of the way these programmes were translated into reality in the then province of Hannover. The provincial government implemented these programmes hesitatingly but nevertheless as ordered, whereas the administrative decrees were complied with in a widely different manner by the individual psychiatric hospitals. These differences in the daily routine on-the-spot realisation of the extermination programmes suggest raising the question of professional ethics in psychiatry, as outlined by the author.

[Acute respiratory failure in a medical intensive care unit. Incidence and prognosis].

Acute respiratory failure had occurred in 89 of 1594 patients in a medical intensive care unit (5.6%), 26.8% of all patients (332) on long-term mechanical ventilation. Compared with the other chronically ventilated patients those with acute respiratory failure averaged a lower age, the proportion of women was higher and the duration of ventilation longer. The death rate was significantly higher (78.7% compared with 58.3%). The important prognostic factors included the underlying disease, additional abnormal organ function, severity of pulmonary gas exchange abnormality, and advanced age. If there was septicaemia, peritonitis, liver cirrhosis with bleeding oesophageal varices or polytrauma with acute renal failure the death rate was over 80%; after hypovolaemic shock, pancreatitis or postoperative pulmonary failure it was less than 65%. Patients who had abnormal function of at most one other organ in addition and an inspiratory arterial pO2 difference below 250 mm Hg, measured 12 hours after onset of mechanical ventilation, had a relatively favourable prognosis with a death rate of 33%, while in the other groups of patients it was 86-100%.

The actin gene in yeast Saccharomyces cerevisiae: 5' and 3' end mapping, flanking and putative regulatory sequences.

The 5' and 3' flanking regions of the yeast actin gene have been sequenced and the ends of the actin mRNA were determined by the single-strand nuclease mapping procedure. The mRNA starts with a pyrimidine residue 141 (or 140) nucleotides upstream from the initiation codon. The actin gene lacks a typical "TATA" box 30 base pairs upstream from the mRNA start site but it contains a region homologous to the canonical sequence 5'-GGCTCAATCT-3' which is found in several eukaryotic genes 70 to 80 bp upstream from the mRNA cap site. Judging from the S1 nuclease mapping, there are two populations of actin mRNA terminating 98 and 107 nucleotides downstream from the stop codon. The 3' termini are preceded by three AATAAA sequences found in most eukaryotic polyadenylated mRNAs.

[The problem of multiple malignomas].

It is reported on three female patients who fell ill with a second and third tumour, respectively, within 4 years. The cases in question were spontaneous neoformations, since risk factors, such as radiotherapy and applications of cytostatics were not used causally.

[QKD-time in thyroid-gland function diagnosis].

In altogether 205 patients with euthyroidism, hyperthyroidism and hypothyroidism the QKD-time was determined. With the help of this examination should be tested whether by means of this simple functional test of the cardiac circulation indirectly is possible also a judgment of the function of the thyroid gland. Essentially in the calculated mean values a differentiation between euthyroidism, hyperthyroidism and hypothyroidism could be made statistically. Since between the individual groups considerable overlappings of the individual values occur the exactness of the test is very much restricted in the preliminary diagnostics of diseases of the thyroid gland. We are of the opinion that the QKD-time is insufficient for the judgment of the function of the thyroid gland.

[Hepatogenic ascites].

In the paper the intrahepatic (hydrostatic pressure and colloid-osmotic pressure) and extrahepatic (hepatogenic aldosterolism and portosystemic collateralisation) factors effective in the development of an ascites were described and the frequency was tested on own clinical material. The hydrostatic pressure relations are presented as the most essential genetic factor in the development of ascites for hepatogenic reason.

[Immunosuppressive treatment of progressive chronic polyarthritis].

After description of forms and possibilities of the immunsuppression the results of this therapy are discussed in 47 own patients with clinically and paraclinically ascertained rheumatoid arthritis. The patients received the sum of 4.0 mg trenimon in combination with altogether 4.0 mg cyclophosphamide by means of 20 infusions with physiological saline solution as carrier substance. Under this treatment in 93.6% of the cases the findings of the examination of joints or the clinical stage improved. As to side effects incompatibilities on the side of the gastro-intestinal tract as well as leucopenias were observed. 18 patients could be after-examined after 6 years. Whilst in every case the clinical stage deteriorated, in the majority the degree of activity remained unchanged. All the results are compared with the results reported in literature. The authors are of the opinion that, when other forms of treatment have no success and contraindication are strongly taken into consideration, the progressing of the rheumatoid arthritis can be retarded by means of immunsuppressives. The pathophysiological processes can, however, scarcely be influenced causally.
