[Negative pressure pulmonary edema. Post-obstructive lung edema after use of a laryngeal mask].

A 31-year-old male patient was scheduled for surgical treatment of a distal radius fracture under general anaesthesia using a laryngeal mask airway and spontaneous ventilation. The intraoperative course was uneventful. During recovery, the patient strongly bit the laryngeal mask, producing a complete obstruction of the artificial airway in combination with very forceful inspiratory efforts until the mask was removed. In the recovery room the patient developed dyspnea and desaturation. A portable chest radiograph demonstrated bilateral alveolar infiltration suggesting pulmonary edema. The patient was transferred to the intensive care unit where drug therapy (furosemide and hydrocortisone) and treatment with the continuous flow continuous positive airway pressure (CF-CPAP) system led to complete recovery within 24 h.

Sonographic demonstration of atypical congenital anomalies in the fetus of a diabetic mother. A case report.

Uncontrolled diabetes, particularly in early pregnancy during the stage of organogenesis, increases the risk of teratogenesis. A case of mixed congenital abnormalities in a fetus demonstrated ultrasonographically during the second trimester of pregnancy in an uncontrolled insulin-dependent diabetic mother is presented. Necropsy of the abortus confirmed the findings.

Genuine septation of the gallbladder. A case report.

Septation of the gallbladder is rarely of any consequence except in differentiating it from a disease of the organ. To date all radiographically demonstrated septa have been considered simple foldings of the wall. A case of genuine septation of the gallbladder after cholecystojejunostomy is presented.

Ultrasound diagnosis in intussusception of the bowel. A case report.

Ultrasound examination is at present a subsidiary aid in the definition of bowel lesions. There are, however, echographic features suggestive or diagnostic of intussusception. These should be borne in mind during ultrasound examination of the patient with a tender abdominal mass or intermittent diffuse abdominal pain.

The effect of water deficit on body temperature during rugby.

Sweat losses, water deficits and changes in rectal temperature were measured in 13 first-league players during a rugby match. Changes in plasma volume, serum electrolyte and blood glucose levels were also determined. Rectal temperatures were markedly elevated after the match. Both temperatures reached levels which are known to be associated with an impairment of physical, mental and psychological function, and could have caused a deterioration in performance during the second half of the match. Body temperatures were high enough to have predisposed to aggressive behaviour. Increases in rectal temperature were related to water deficit. The small volumes of fluid ingested by the players just before and during the match suggest that they are unaware of the importance of preventing dehydration. Recommendations are made to reduce the risk of hyperthermia. Glucose and electrolyte supplementation is not required just before and during rugby.

The concomitant problems of young stutterers.

This investigation was designed to survey a sample of 650 speech-language pathologists providing services in elementary schools and to obtain information on the concomitant problems of stutterers in their caseloads. The results of the survey revealed that 32% of all the young stutterers were free of any other problems. The remaining 68% were reported as having some other type of problem (articulation, language, voice, emotional disturbance, neurological handicap, etc.) in addition to their stuttering. Results also present the type of therapy received by each subgroup. Limitations of the survey method and clinical and research implications are discussed.
