[Prosthetic treatment of patients with an extensive posttraumatic maxillary defect with the use of dental implantation method].

Prosthetic care in cases of severe traumatic maxillary injuries remains topical. Alveolar maxillary processes atrophy make prosthetic treatment more difficult due to insufficient fixation and stabilization. At the stage of surgical treatment some corrective operations for a complete recovery of the disturbed functions and anatomic interrelations are made. Maxillary defects are rather variable in the form and size being dependent upon etiological factor, defect topography, terms of prosthetic treatment and time after operative intervention. Introduction of the method of dental implantation solves many problems of the appliance fixation and its adaptation in the place of the defect. A case is reported of rational prosthetic treatment after reconstructive-restorative operation on the upper jaw followed by 5 dental implants installation.

[Study of shearing strength between layers of light curing composite Esterfill PHOTO].

There was determined shearing strength of monolit samples of light curing material and samples received by modeling of further layers on surfaces with and without inhibited layer and also tensile strength of layers connected with the help of light curing adhesive. The study was performed on enamel, dentine and cutting edge masses from set of light curing material Esterfill PHOTO for prosthetic purposes. The received results showed that shearing strength between layers of enamel, dentine and cutting edge masses of Esterfill PHOTO varied between 22 and 28 MPa and tensile force divided by the nominal surface area did not depend upon the way of sample modeling. Distraction zone represented half of the cone turn out of substrate. More rigid material chopped in the process of distraction lesser volume off substrate. For prosthetic purposes it was recommended to use the equipment allowing to combine the final photopolymerization of the article with annealing under high temperature conditions or make annealing of the ready article after photopolymerization.

[Patient rehabilitation after mandibular reconstruction by revascularized rib autogenous graft with the use of dental implantation].

Deformations after mandibular resection can be eliminated by single stage defects replacement by reconstructive materials of different origin. Prior to microsurgical methods of jaws reconstruction the patient rehabilitation with defects of the jaws had very limited success due to a number of drawbacks of traditional techniques (transplant tendency to resorption and seizure, complexity in surrounding tissues recovery) that makes impossible the use of the dental implantation techniques. In case of interdisciplinary planning of the treatment dental implants can be successfully placed in reconstructed mandible providing the conditions for aesthetic and functional rehabilitation of patients. The article described a clinical case of carrying out microsurgical mandible reconstruction with consequent placing of 7 dental implants and rational prosthetic appliance making.

[Experimental substantiation of parameters and strength characteristics of new endodontic-endoosseous implant construction].

Postoperative tooth stump is not able to endure usual functional loads adequately that leads to enhanced resorption of bone tissues surrounding root and loss of the tooth. Based on math computer simulation of mode of deformation of biomechanical model "metal-ceramic construction--endodontic-endoosseous implant (EEI)--stump of operated tooth--bone tissue of the jaw" new domestic EEI parameters were elaborated which improved greatly biomechanical properties of tooth with operated roots. Strength characteristics of new EEI were studied. A set of instruments for EEI was developed.

Biomechanical Phenomena↗

[The use of the Periotest method for the clinical assessment of the efficacy of dental implantation].

The paper describes a modern diagnostic method: Periotest for assessing the stability of intraosseous implants. The mobility of implants after remote and immediate implantation is assessed in 65 patients with dentition defects. The study yielded complete information on the status of tissue complex, detected the early signs of impaired bone integration after implantation, and helped more reliably predict the results.

[Bone tissue density study of the mandible in patients after orthodontic treatment using implants].

Mandibular bone compactness has been studied by ultrasonic osteometry before and after implantation. Ultrasonic examinations were carried out in remote periods after orthodontic treatment (implantations) of patients with dentition defects. Analysis of the results indicates that 2-4 years after the implantation is the critical term, because masticatory loading of dentition is levelled and the unilateral chewing reflex is restructured after fitting with dentures.

Bone Density↗

[Evaluation of the bioelectrical activity of the masticatory muscles in patients depending on the period since implantation].

The results of electromyographic control used for evaluating the efficacy of orthodontic treatment are analyzed. Analysis of electromyographic parameters after orthodontic treatment of dentition defects with implants showed essential changes in the function of the masticatory muscles at different terms after implantation. Asymmetrical activity typical of unilateral chewing was observed, as well as high bioelectrical activity of the temporal muscles.

[A biomechanical study of dental implants using a method for 3-dimensional volumetric mathematical modelling].

Biomechanics of the dental implants introduced into alveoli immediately after tooth extraction has been investigated. The programme ANSYS has been used. Three-dimension volume mathematical models were calculated, with the help of which the tense-deformed state of the supportive biological tissues has been investigated in the area of direct implantation. On the grounds of the results obtained a conclusion has been made that there is an essential improvement of the load distribution under investigation of the inner bone modified biologically designed implants for direct implantation.

[The features of using intraosseous implants with intramobile elements].

Functional features of type Brånemark implants are compared with those of type IMZ implants. The problem of determining optimal stiffness characteristics of type IMZ implants has been solved by means of linear programming. The conclusion was made on considerable improvement in distribution of loads when type IMZ osseointegrated implants are used as supporting elements in the denture.

[The enhancement of the efficacy of patient orthodontic treatment based on the mathematical modelling of prospective implant designs].

Opportunities to increase prosthetic treatment efficiency by means of applying osseointegrated implants of traditional and prospective types are analysed. A technique of mathematical modelling of the interaction between implants and jaw bone is exposed. The numerical model and applied program, developed on the basis of finite element method, enabling to analyse a stress strain state of the bone and to determine extreme safe loads on implants are described. It is shown, that the most loaded zone is a layer of the compact bone directly contiguous to the neck of the implant. That is in good agreement with the results of clinical research, according to which just this zone has the highest percentage of complications. Methods for further optimisation of implants and prosthetic structures for the purpose to perfect the techniques of prosthetic treatment are discussed.
