The synthesis and secretion of collagen by cultured sea urchin micromeres.

Circumstantial evidence in several previous studies has suggested that sea urchin embryo micromeres, the source of primary mesenchyme cells which produce the embryonic skeleton, contribute to the extracellular matrix of the embryo by synthesizing collagen. A direct test of this possibility was carried out by culturing isolated micromeres of the sea urchin Stronglyocentrotus purpuratus in artificial sea water containing 4% (v/v) horse serum. Under these conditions the micromeres divide and differentiate to produce spicules with the same timing as intact embryos. Collagen synthesis was determined by labeling cultures with [3H]proline or [35S]methionine and the medium and cell layer were assayed for collagen. The results indicate that by the second day in culture micromeres synthesize and secrete a collagenase-sensitive protein doublet with a molecular weight of about 210 kDa. Densitometry indicates a 2:1 ratio of the respective bands in the doublet which is characteristic of Type I collagen. The doublet is insensitive to digestion with pepsin. This differential sensitivity is characteristic of collagen. Over 90% of the collagen synthesized by micromeres is soluble in the seawater culture medium. On days 2-4 in culture, collagen accounts for 5% of the total protein synthesized and secreted. Additional collagenase-sensitive bands are noted at 145 and 51 kDa. The relationship of the described collagen metabolism to previously characterized collagen gene expression in sea urchin embryos is discussed.

Post-cesarean analgesia using a subcutaneous pethidine infusion.

A pethidine infusion administered subcutaneously using a syringe pump was evaluated in 118 women following cesarean section. One hundred three women (87%) were satisfied with their postoperative analgesia. The mean length of infusion was 18.8 h, and the mean total dose of pethidine was 402 mg. Twenty-seven patients (23%) reported no severe pain postoperatively, the remainder had on average 2.6 h of pain which they described as severe. Of 38 women who had had a previous cesarean section, 19 (50%) considered the pump to be an improvement on the analgesia given the previous time. There were no cases of respiratory depression. This mode of analgesia has advantages over conventional intramuscular bolus injections. It was judged acceptable to both patients and ward staff.

Changes in coagulation and fibrinolysis during pregnancy: evidence of activation of coagulation preceding spontaneous abortion.

In order to monitor physiological changes in coagulation and fibrinolysis that occur during normal pregnancy, blood samples were collected in each trimester of pregnancy from 17 volunteers. Control samples were collected from 12 non-pregnant female volunteers. As pregnancy advanced there was a rise in the basal levels of fibrinopeptide A, cross linked D-dimer fragment and the B beta 15-42 fragment and an increase in the in vitro rate of fibrinopeptide A generation. These results were consistent with an increased activation of coagulation during normal pregnancy, compensated for by a concomitant rise in fibrinolytic activity. In two patients who spontaneously aborted, evidence of uncompensated activation of coagulation could be detected before the manifestation of any clinical signs. In a second pregnancy in one of these patients similar changes were observed, but were reversed by heparin treatment and the pregnancy progressed to full-term delivery of a normal infant.

A prospective study of children with first acute symptomatic E. coli urinary tract infection. Early 99mtechnetium dimercaptosuccinic acid scan appearances.

Between 1985 and 1987 102 children, age 0-14 years, presented with a first acute symptomatic E. coli urinary tract infection. Investigations included early 99mtechnetium dimercaptosuccinic acid (DMSA) scan (which was performed at a median of 27 days), ultrasonography, micturating cysto-urethrography and indirect voiding radionuclide cystography using 99mTc DTPA. Follow-up DMSA scan was carried out after 6 months. Twenty-one of 102 of initial DMSA studies showed diminished uptake of radionuclide and 12 showed cortical scarring. Twenty-nine patients had significant vesicoureteral reflux (VUR). The finding of diminished uptake on the initial scan was significantly associated with fever, systemic upset, length of symptoms and a peripheral blood leucocytosis, (p less than 0.05). In addition the finding was associated with fever and loin pain in the older child. Both diminished uptake and scarring were more common in refluxing kidney units. We propose that, in children with UTI, diminished uptake on early DMSA scan localises infection in the renal parenchyma.

The health visitor and the prisoner's child.

This paper comments on the relevance to health visitors of research carried out to establish the number of children in England and Wales whose fathers' are imprisoned, the impact of this on the children concerned and the need for intervention by health and social welfare agencies. Men received into prison during two separate three month periods were surveyed, a sample of wives interviewed and health visitors and teachers questioned. The findings suggest that in one year more than one hundred thousand children have a father sent into custody. Because of the current concentration of crime in areas of high deprivation, imprisonment of a father is a relatively normal occurrence in some districts, frequently adding pain, confusion and worry to the child's social, economic and educational disadvantage. Health visitors and teachers reported disturbed behaviour, ill health and problems at school. The role of the health visitor can be crucial in some of these cases yet frequently she is unaware of the family's predicament.

Surgical management of bleeding gastric ulceration.

In a nine-year period from 1977 to 1985 sixty-one patients underwent surgery for bleeding gastric ulceration in the Western Infirmary, Glasgow. Nineteen patients were treated by partial gastrectomy, twenty-two had undersewing of the ulcer plus vagotomy and drainage and twenty had undersewing alone. Mortality in the three groups was 26, 45 and 10 per cent respectively. All groups of patients were similar in terms of age, severity of haemorrhage, delay before surgery and grade of surgeon performing the procedure. Out-patient follow-up (mean: 37 months) of patients treated by undersewing alone revealed that 73 per cent were symptom free. Treatment of bleeding gastric ulceration by undersewing alone is effective and should be considered in patients who require surgery.

Pharmacokinetic study of the interaction between rifampicin and ketoconazole.

This study assessed the potential pharmacokinetic interaction between rifampicin and ketoconazole, two drugs used to treat the increasingly common combination of Mycobacterium tuberculosis and Candida albicans infection in AIDS patients. The peak plasma rifampicin concentrations in six healthy male subjects were not altered when taken in conjunction with ketoconazole. However, the peak plasma ketoconazole levels were significantly diminished when taken in conjunction with rifampicin, compared to when taken alone (P less than 0.015). The mean area under the curve (AUC) for ketoconazole was significantly diminished when taken with oral or intravenous rifampicin (P less than 0.001).

Reactogenicity and antigenicity of rhesus rotavirus vaccine (MMU-18006) in newborn infants in Venezuela.

The reactogenicity and antigenicity of the rhesus rotavirus vaccine, strain MMU18006, developed at the Laboratory of Infectious Diseases (National Institute of Allergy and Infectious Diseases, National Institutes of Health) were examined in a double blind, placebo-controlled study of 40 newborn infants in Caracas, Venezuela. The children were observed for the first few days after birth in the hospital nursery and by home visits for 10 days after vaccination to detect any adverse reactions. No reactions could be attributed to the vaccine. Serologic responses to the vaccine were evaluated in paired sera obtained at birth (cord blood) and 4 weeks after vaccination. Serologic responses to the vaccine were not observed by complement fixation, neutralization or a rhesus rotavirus VP7 epitope-specific competition assay. However, such responses were found in 9 of 14 tested infants by an immunoglobulin A-specific enzyme-linked immunosorbent assay. Seventeen of the 20 vaccinees also shed rhesus rotavirus vaccine in stool during the postvaccination period.

Narcissism and the use of personal pronouns.

In this study we explored the relationship between narcissism and the individual's use of personal pronouns during extemporaneous monologues. The subjects, 24 males and 24 females, were asked to talk for approximately 5 minutes on any topic they chose. Following the monologues the subjects were administered the Narcissistic Personality Inventory, the Eysenck Personality Questionnaire, and the Rotter Internal-External Locus of Control Scale. The monologues were tape-recorded and later transcribed and analyzed for the subjects' use of personal pronouns. As hypothesized, individuals who scored higher on narcissism tended to use more first person singular pronouns and fewer first person plural pronouns. Discriminant validity for the relationship between narcissism and first person pronoun usage was exhibited in that narcissism did not show a relationship with subjects' use of second and third person pronouns, nor did the personality variables of extraversion, neuroticism, or locus of control exhibit any relationship with the subjects' personal pronoun usage.

Diagnosis of lung disease in acquired immune deficiency syndrome: biopsy or cytology and implications for management.

One hundred and twenty consecutive bronchoscopic examinations were carried out on 80 patients with acquired immune deficiency syndrome (AIDS) between January 1982 and December 1986. Ninety one paired biopsy and cytology specimens from 72 of these patients were analysed. There was no significant difference between biopsy and cytology in diagnosing Pneumocystis carinii pneumonia (0.95 greater than p greater than 0.1). In 10 cases P carinii pneumonia was diagnosed by biopsy but not cytology and in seven cases by cytology but not biopsy. Nineteen patients had multiple infections or Kaposi's sarcoma. Biopsy was more useful than cytology in the diagnosis of other infections (n = 20) and Kaposi's sarcoma (n = 2) with positive cytological correlation in only three of the infections. Biopsy and cytology together have a diagnostic yield of 78.3%. We conclude that all patients presenting with respiratory disease who have, or are in a high risk group for, AIDS should be examined by bronchoscopy at an early stage with both cytology and biopsy.

Detective quantum efficiency of imaging systems with amplifying and scattering mechanisms.

We have analyzed the influence of stochastic amplifying and scattering mechanisms on the transfer of signal and noise through multistage imaging systems in terms of multivariate moment-generating functions. Stochastic amplification of photon noise by one stage of an imaging system is shown to constitute an effective signal to the next, while the underlying photon-noise component is unaffected by a subsequent scattering process. In the case of stationary, photon-limited inputs, these considerations then lead to useful expressions for the noise power spectrum and detective quantum efficiency for multistage image systems. The application of these results to the analysis of radiographic screen-film imaging systems is discussed.
