[Decomposition studies on the core function of working memory].

By means of decomposition we investigated using an experimental design (n = 160) the impact of combinations of processing and retention requirements on performance. Retention-during-processing spans (i.e., dual task requirements with equal importance of both evaluated components) were compared with the "reading" (final word) span of read sentences. In a correlational design, the relationships of these intentional spans and a unintentional indicator of working memory (the pronoun inference distance) were compared with fluid intellectual performances. The effects of processing on the span may not be restricted to articulatory suppression of rehearsal. While the substitution of retention of an additional processing-independent item improved the span, this did not hold for items resulting from processing. Thus, the span is determined by coordination of retention with processing, but neither by an activity without retention nor by retention per se. Coordination requires resources in addition to those required by the activities to be coordinated. Strategies of coordination differing in their efficiency result in different spans. Both unintentional processes and intentional ones contribute to the dual task performance of the retention-during-processing type. The best predictors for fluid verbal and nonverbal intellectual performance are the unintentional (pronoun) inferences and the complex intentional retention-during-processing span. The results are in favor of a working memory model in terms of a set of functions.

[Daily ultrashort chemotherapy and intermittent short-term chemotherapy with 4 drugs of communicable pulmonary tuberculosis treated for the first time. Results of a cooperative multicenter study].

Three short-course regimens, all comprising isoniazide (H), rifampicine (R), streptomycine (S) and pyrazinamide (Z), are compared in a randomized prospective cooperative clinical trial. The drugs are given daily in a 3-month regimen (3-HRSZ), twice a week in a 6-month regimen (6-HRSZ2), and in a further two-phase 6-month regimen the 4 drugs are administered 3 times a week for the first 3 months followed by the administration of HSZ twice a week (without R) for further 3 months (3-HRSZ3/3-HSZ2). The number of patients admitted to study is 80, 144 and 139 respectively. The 3-month regimen has been stopped because of a high rate of relapses. 17 p.c. of the patients admitted have to be excluded from analysis for various reasons, out of these 5.8 p.c. because of adverse reactions. Two thirds of the patients had heavily positive sputum cultures at the start. 300 patients completed therapy. At the end of therapy cultures were negative in 94 p.c., 100 p.c. and 99 p.c. respectively. The rate of bacteriological relapses is 19 p.c. in 3-HRSZ, 9 p.c. in 3-HRSZ3/3-HSZ2 and 3 p.c. in 6-HRSZ2, during a follow-up period of 3-4 years after completing therapy. The acceptability was good in all treatment groups. Adverse reactions like "flu" were rarely observed. Increased blood urea was common but in general without clinical symptoms. Elevation of ALAT and ASAT was relatively frequent but mostly transient and without clinical importance. The results served as basis for the new "Recommendation for Treatment of Tuberculosis" and are interpreted with regard to practical consequences and possibilities for further rationalisation of treatment.

[Problems concerning old-aged patients with pulmonary tuberculosis from the clinical point of view (author's transl)].

One fourth of all patients with bacteriologic positive tuberculosis, who had been admitted in the Robert-Koch-Hospital of Leipzig between 1974 and 1978, were older than 70 years of age. Most of them suffered from an extensive, contagious, cavitary disease with impressive clinical symptoms. The prevailing part of them could be treated by the so-called "Zweiphasentherapie". That means a daily initial treatment was followed by an intermittent period of chemotherapy. Most of them reached quickly sputum conversion. As yet the bacteriologic relapse is very low.
