Measuring organizational readiness for nursing research programs.

Nursing research programs are important innovations for health services organizations (HSOs). Evidence supports the positive impact that successful programs have on both clinical practice and patient outcomes. In the current era of scarce health care resources, decision makers must be able to accurately judge HSO readiness for nursing research programs to facilitate their success. Judging innovation readiness is an important and complex process for decision makers. An Innovation Readiness Scale (IRS) was developed from research-based HSO contextual cues identified by nurse researchers as supportive of successful hospital-based nursing research programs. The IRS provides decision makers with a measure by which to judge HSO readiness for nursing research programs. Psychometric results are reported from a pilot and actual test of the IRS in two urban acute care settings involved in nursing research program innovation.

Health care system innovation: a model for practice.

Health care systems are undergoing dramatic and rapid change. Innovations occurring within these systems have an impact on all facets of a system's performance and reshape its services as well as provider and patient roles. In this era of innovation, an essential survival skill for advanced practice nurses is an ability to use a macrosystem perspective to effectively strategize for role development and provision of services within practice settings experiencing rapid innovation. The Health Care System Innovation Model described in this article provides a framework for such a macrosystem perspective.

An assessment taxonomy for designing nursing research programs.

Clinical nursing research programs are a common structural component within nursing service organizations. Descriptive literature identifies many design features that are important in the development and success of organizational innovations, among which is the need to design programs that are congruent with the overall organization. Two exploratory Delphi studies were conducted to identify and classify organizational attributes that clinical nurse researchers considered critically important to the success of nursing research programs. Nadler and Tushman's Congruence Model for Organization Analysis was used as a framework to develop an assessment taxonomy for classification of the 64 critically important attributes identified by clinical nurse researchers.

Managers' ability to perform: individual and collective assessment findings.

Changes in role responsibilities, organizational relationships and mix of professional and non-licensed personnel have increased the need for well prepared managers who can be effective in new work environments. Because such individuals are not readily available, identification of potential managers and development of incumbents are crucial. This article describes how four hospitals used the same unique assessment system to identify the strengths and limitations of 116 managers, the implications of their findings, and subsequent interventions. Suggested also are activities with potential for meeting hospitals' future needs for effective managers.

Instrumentation to describe subjective sleep characteristics in healthy subjects.

The purpose of this study was to develop and test the Verran and Snyder-Halpern (VSH) Sleep Scale, an instrument to subjectively measure sleep characteristics. Four major sleep factors and their associated characteristics were proposed for the Sleep Scale. Subjects completed three randomly ordered sleep questionnaires on three consecutive weekday mornings within the first two hours after arising. Scales included the VSH Sleep Scale, a sleep questionnaire and a sleep log. The VSH Sleep Scale had a reliability coefficient of .82 (theta). Construct validity was examined by factor analysis and correlations between Sleep Scale items and corresponding items on the two other study instruments. Scale validity also was assessed by the known groups method. Beginning support for the validity of the VSH Sleep Scale is provided.

Doctoral programs in nursing: an examination of curriculum similarities and differences.

In this exploratory study, eight doctoral programs in nursing (four PhD and four DNS/DSN/DNSc) were surveyed to examine curricular similarities and differences between research-oriented and professional-oriented programs. Content analysis was utilized to examine manifest content of 101 units of analysis derived from questionnaire items. These items reflected environmental input, curricular design, and outcome variables. Findings suggested that more curricular similarities than differences existed between the two program types. Curricular design variables reflected the most differences in the areas of program purposes and objectives. Several areas were identified for additional research in each variable category.

A model-based context assessment approach for the informatics nurse specialist consultant.

As the number of informatics nurse specialists (INS) has grown, they have become increasingly involved in information system (IS) consultation practice. Their combined clinical and informatics expertise makes these specialists highly marketable and successful IS consultants. As healthcare settings have shifted their IS development toward interdisciplinary-integrated clinical information systems (CIS) that support a patient-focused care continuum, INS have been required to broaden their consultation approaches beyond a domain-specific focus. This article illustrates a model-based context assessment approach that can be used by INS to expand their consultation repertoire.

Assessing health care setting readiness for point of care computerized clinical decision support system innovations.

Increasing interest in linking high-quality and cost-effective clinical practice processes to patient outcomes has enhanced interest in computerizing clinical practice guidelines to support clinician decision making. This article describes a Clinical Information Technology Innovation Model (CITIM) that was used to assess health care setting readiness for a clinical decision support system (CDSS) to decrease innovation risk and enhance the potential for innovation success. A case illustration shows CITIM application in the assessment of a hospital's readiness for a computerized knowledge-based system to support RN acute pain management decision making with patients who had coronary artery bypass graft surgery.

Performance-based staff development. A baseline for clinical competence.

Changes in health-care financing, complex technological advancements, and an expanding nursing knowledge base have made it increasingly difficult for health care educators to assess and enhance the development of professional nurses. Using a set of objective evaluation methods and tools, educators assessed the competency of novice and experienced staff. This information identified learning and practice needs that formed the bases for individualized orientation and clinical educational programs.

A framework for the development of nurse manager negotiation skills.

Effective negotiation skills have been identified as an essential component of the nurse manager role. Application of the negotiation process in today's dynamic and competitive health care environment is frequently difficult and stressful for the novice nurse manager. The authors describe a Conceptual Model for Negotiation that can be used as a management development framework for the novice nurse manager.

Health services organizations computer innovation. Ready or not?

Health services organizations (HSOs) have expanded into increasingly integrated delivery systems requiring more sophisticated health information systems (HISs) to support decision making. Computer innovations are required in most HSOs to support these HISs. Health services organizations decision makers are faced with many complex decisions during the computer innovation process. One of their most important decisions is judging HSO innovation readiness. Accurate judgments about HSO innovation readiness will have a direct impact on how successful decision makers are as computer innovators.
