Tumor spread and DNA content in human renal cell carcinoma.

Local and distant tumor spread was evaluated and compared with DNA content analyzed by flow cytometry of eight samples from each of 71 renal cell carcinomas. Twenty-six tumors were homogenously diploid while 45 tumors contained at least one aneuploid tumor sample. Diploid tumors generally respected the surrounding tissues and only three of 26 tumors (12%) had evidence of local tumor invasion. In contrast, 33 of 45 patients (73%) with aneuploid tumors had local invasion (p less than 0.001). Local metastases in lymph nodes and adrenal was found only in patients with aneuploid tumors. However, distant metastases appeared in about the same frequency in patients with diploid and aneuploid tumors (35 and 29%, respectively). Patients with diploid tumors had significantly more often solitary metastases and, most interestingly, the occurrence of lung metastases was a characteristic feature for patients with aneuploid tumors (p less than 0.02). The diploid primary tumors with distant metastases generally were devoid of local invasion while all aneuploid tumors with distant metastases had local invasion and mostly also local metastases. Thus, different characteristics of tumor spread were shown for diploid and aneuploid tumors and the pathways for spread with distant metastases might also differ between these tumors.

Deoxyribonucleic acid content in metastatic renal cell carcinoma: clinical implications.

Tumor DNA content was analysed in 26 patients with metastatic renal cell carcinoma. The primary tumors were homogenously diploid in ten patients (38%). In 16 patients (62%) an aneuploid DNA content was found in at least one of eight tumor samples analyzed. Patients with diploid tumors survived significantly longer (P less than 0.001) than patients with aneuploid primary tumors (median 32.5 and 6 months, respectively). The DNA content of the metastases seems to give additional prognostic information. The results indicate that DNA content is a useful prognostic predictor in renal cell carcinoma. Patients with homogenously diploid tumors might be treated aggressively with nephrectomy and excision of apparent metastases, particularly solitary, since these patients seem to have a chance for prolonged survival time. Aneuploidy in metastases, solitary and multiple, indicates that nephrectomy is not meaningful.

Prognostic factors in renal cell carcinoma.

Various prognostic factors were retrospectively studied in 81 patients with renal cell carcinoma. Among 16 studied variables 3 were statistically strongly associated with survival. These were clinical stage, tumour cell type and histopathological grade. Some other variables were statistically associated with survival, e.g. tumour size, palpable mass, sedimentation rate and haemoglobin value. In patients surviving more than 10 years, the majority of tumours (28/35) were confined within the renal capsule. Tumours spreading outside the renal capsule dominated in patients who survived less than 10 years (32/46). Patients with renal vein involvement only had longer survival times than those with other local spread.

DNA content in ulcerative colitis. Flow cytometric analysis in a patient series from a defined area.

Sixty patients with ulcerative colitis of more than nine years' duration and 13 patients with a shorter disease duration were examined over a three-year period with flow cytometric DNA analysis and conventional histologic investigation of biopsy specimens obtained by colonoscopy. The patient series included all patients with ulcerative colitis from a colonic cancer surveillance program in a defined area. There were five patients-one who developed colonic cancer, one with high-grade dysplasia, and three with low-grade dysplasia. None of these showed aneuploidy (abnormal DNA stem lines). There were six patients with aneuploidy in one to six locations in the colorectum. On consecutive examinations, there was a high reproducibility of the abnormal DNA pattern for ploidy levels and location in the bowel. In these patients, there was no obvious relationship with definite dysplasia. It is concluded that flow cytometric DNA analysis in ulcerative colitis may become a valuable complement to evaluation of dysplasia in cancer surveillance, but assessment of the cancer risk in individual patients with aneuploidy needs further investigation.

Renal cell carcinoma in a solitary kidney: late nephrectomy after 35 years and analysis of tumor deoxyribonucleic acid content.

We report a case of renal cell carcinoma in a solitary kidney, which was palpable for 35 years and verified histopathologically 31 years before nephrectomy. Deoxyribonucleic acid analysis of paraffin embedded and new surgical samples showed a similar aneuploid deoxyribonucleic acid content with an index of 1.3. The unusually long survival of this patient is discussed.

Parenteral superoxide dismutase plus catalase diminishes pancreatic edema in sodium taurocholate-induced pancreatitis in the rat.

Scavengers of toxic oxygen reduction products have been reported to reduce the inflammatory reaction in some models of pancreatitis. In a blinded study, the effect of parenteral pretreatment with superoxide dismutase plus catalase was compared with placebo on pancreatitis induced in rats by infusion of 0.25% or 2% sodium taurocholate into the hepatopancreatic duct. The degree of inflammation was assessed by macroscopic examination of the pancreas, dry/wet weight ratios of pancreatic specimens, amylase activity in plasma and peritoneal exudate, the weight of the exudate, and its content of total protein. All parameters were indicative of a more severe inflammation in rats given the higher concentration of sodium taurocholate. The only significant effect of the superoxide dismutase plus catalase treatment was a moderate reduction of the dry/wet weight ratio, i.e., pancreatic edema, in rats given 2% sodium taurocholate. Our results indicate that toxic oxygen reduction products, available for interception by parenterally administered superoxide dismutase plus catalase, are of only minor importance in the pathogenesis of sodium taurocholate-induced pancreatitis in the rat.

Ulcerative colitis. Cancer surveillance in an unselected population.

An unselected series of patients with chronic ulcerative colitis from a defined catchment area underwent endoscopic and histologic cancer surveillance from 1977 to 1985. At the end point of the study, which included a total of 93 patients, there were 38 patients with total colitis of more than 10 years' duration. There was one case of colonic carcinoma, two cases of high-grade dysplasia, and no death due to colorectal cancer. We conclude that in an unselected group of patients with ulcerative colitis, the risk for colorectal dysplasia and cancer is low and that a surveillance program is reliable and can be performed at a community hospital.

Morphological studies of bronchial mucosal biopsies from asthmatics before and after ten years of treatment with inhaled steroids.

To examine the influence of inhaled steroids on the bronchial mucosa, biopsies from six patients with severe bronchial asthma were studied before and after ten years of daily treatment with inhaled steroids. Biopsies from six healthy subjects were also examined. In the biopsies taken from the asthmatic patients before treatment there was a significant increase in inflammatory cell numbers compared with the biopsies from the control subjects. In all patients scanning electron microscopy showed a reduced coverage by cilia. Squamous cell metaplasia was seen in two patients. After ten years of treatment the number of inflammatory cells was significantly reduced compared to that before treatment and was not different from the control biopsies. Most of the epithelial cells showed a ciliated surface. Small focal areas with non-ciliated cells could still be seen in four patients. In the two patients with squamous cell metaplasia before treatment, small areas of metaplasia could still be seen. Despite the absence of inflammation and reduced epithelial damage during treatment all patients still had bronchial hyperresponsiveness.

Local recurrence and survival after radical resection of rectal carcinoma.

The results of surgical treatment in 99 cases of rectal carcinoma operated on with curative intent by 26 surgeons were retrospectively analyzed and the resected specimens were reexamined according to the Astler-Coller staging system. No chemotherapy or radiotherapy had been given preoperatively. Local recurrence appeared within 5 years in 37% of the patients (within 2 years in 29%), with highest rate in stage C2 tumours in the lower third of the rectum. The recurrence rate did not differ between operations with abdominoperineal excision (n = 83) or with anterior resection (n = 16). All recurrences were treated with chemotherapy, radiotherapy or operation. Progressive disease without evidence of local recurrence was found in 11 patients. The overall 5-year survival rate was 50%. The respective rates for Astler-Coller stages A, B1, B2, C1 and C2 were 86, 59, 52, 33 and 27%.

Prognostic value of DNA content in colorectal carcinoma. A flow cytometric study with some methodologic aspects.

The DNA content of 37 colorectal carcinomas was studied prospectively by flow cytometry. The DNA content from the same tumors was also studied in disintegrated paraffin sections. The methods gave similar information on the DNA content and the correlation was 0.94. In eight patients tumor imprints were prepared and the DNA content was analyzed with static cytometry. A correlation coefficient of 0.83 between flow cytometry on fresh tumor tissue and static cytometry was found. Sixty-two percent of the tumors were aneuploid and had significantly higher S-phase values than diploid tumors. Histologic grade was not related to ploidy level, whereas Dukes' C tumors often were aneuploid. When the clinical course of patients was analyzed by log rank test (mean follow-up, 30.4 months), the patients with diploid tumors showed a clear advantage in terms of survival; only two patients in this group developed progressive disease. It is concluded that the DNA content is an independent prognostic predictor of colorectal carcinoma and that DNA can be analyzed by several methods. Of these, the method using disintegrated paraffin sections appears most attractive.

Flow cytometric DNA analysis of renal-cell carcinoma. A study of fine needle aspiration biopsies in comparison with multiple surgical samples.

The flow cytometric (FCM) DNA analysis of fine needle aspiration (FNA) biopsy material was compared with the DNA analysis of multiple surgical biopsy material from 44 renal-cell carcinomas. Twenty tumors were heterogeneous with respect to their DNA content. Eleven of the 17 tumors that had both diploid and aneuploid tumor cell clones in the surgical specimens gave a diploid DNA content in the aspiration biopsies; the other 6 cases showed aneuploidy in the aspirate. The fine needle aspirates from 18 homogeneously diploid tumors and 9 tumors with an aneuploid DNA content in all eight surgical samples revealed DNA indices similar to those found in the surgical samples. These findings show that aneuploid clones of renal-cell carcinoma can remain undetected by the use of FNA biopsy as a method for obtaining samples for FCM DNA analysis.

Gardner's syndrome with gastric and ileal polyposis. Case report.

A case of Gardner's syndrome with multiple colorectal carcinomas and polyposis of duodenum, ileum and stomach is reported. The patient was treated with colectomy, mucosal proctectomy and ileoanal anastomosis with ileal reservoir. The rationale of saving ileum and the anal function in presence of ileal adenomas and intramucosal rectal cancer is discussed.

DNA content and prognosis in renal cell carcinoma. A comparison between primary tumors and metastases.

Deoxyribonucleic acid (DNA) content was retrospectively determined by single-cell cytophotometry in primary tumors and corresponding metastases from 32 patients with renal cell carcinoma. In 15 of the primary tumors a diploid/near diploid and in 17 an aneuploid DNA content was found. A diploid/near diploid DNA pattern was revealed in 10 metastases and 22 were aneuploid. By comparing the DNA content in the primary tumors with their metastases, 13 of 32 showed a clear divergency, which might illustrate tumor cell heterogeneity of renal cell carcinoma. The DNA pattern showed a close correlation to morphologic grading. A correlation between DNA content in the metastases and survival time was found. Patients, with diploid/near diploid metastases survived significantly longer than those with aneuploid DNA contents (mean, 31.1 and 11.5 months, respectively; P = 0.004). In contrast to this, no correlation was found between DNA content in the primary tumors and survival time.

Prognostic value of deoxyribonucleic acid content in metastatic renal cell carcinoma.

The deoxyribonucleic acid content of tumor specimens from 23 patients with metastatic renal cell carcinoma was analyzed prospectively by flow cytometry and static cytophotometry. Of the primary tumors 10 (43 per cent) were homogeneously diploid or near diploid in 8 samples studied from each tumor and 13 (57 per cent) had an aneuploid deoxyribonucleic acid content in 1 to 8 samples. At the end of followup 9 of 10 patients with diploid or near diploid primary tumors were alive, compared to only 1 of 13 with aneuploid primary tumors. Patients with homogeneously diploid or near diploid tumors survived significantly longer compared to those with aneuploid tumor deoxyribonucleic acid content (p less than 0.001). After excision of solitary diploid or near diploid metastases 4 patients had no evidence of disease. In 3 of these patients the primary tumors were diploid or near diploid, whereas 1 had 1 aneuploid and 7 diploid or near diploid samples in the primary tumor. In 10 other patients 28 metastases revealed concordance in deoxyribonucleic acid content with the primary tumors. Our results indicate that deoxyribonucleic acid content might be a useful prognostic discriminator with implications for the clinical management of patients with metastatic renal cell carcinoma.

Prognostic significance of the DNA content in renal cell carcinoma.

The DNA content in renal cell carcinoma as an indicator of prognosis was studied retrospectively in 55 patients without distant metastases at operation. Two groups of patients were selected, differing with respect to survival. Thirty-three patients survived for at least 10 years and 22 succumbed to the disease within four years. DNA measurements in morphologically identified tumor cells were performed in histological sections by single cell cytophotometry. The tumor cells of the surviving patients had a DNA content comparable to that of normal cells. A diploid/near diploid DNA content was the dominant feature in 32 of these 33 tumors. The remaining patient had a tumor with a tetraploid/near tetraploid DNA value. In contrast, all tumors from the non-surviving patients had abnormally increased DNA content, indicating a high degree of aneuploidy in these tumors. The results suggest that DNA content may be superior to other clinical and microscopical parameters as a prognostic indicator.

Intestinal permeability as assessed with polyethyleneglycols in birch pollen allergic children undergoing oral immunotherapy.

Twenty-four children with rhinoconjunctivitis due to birch pollinosis were treated in a double-blind manner with enteric-coated capsules containing either a high dose of a birch pollen preparation (n = 11) or placebo (n = 13). The permeability of the small intestine was analysed at three different occasions with a mixture of differently sized polyethyleneglycols (PEG 400 and PEG 1000), before the start of oral immunotherapy (OIT), at the moment of maximum allergen dose, and after 3 months of therapy which was at the beginning of the pollen season. The actively treated children did not significantly change their permeability characteristics as determined from PEG recovery in the urine. By contrast, in the control group of patients the recovery of larger PEG molecules was decreased after 3 months of therapy, possibly due to the commencing pollen season. In addition, small bowel biopsies were taken at the time of maximum allergen dose from two children openly treated with OIT. Both specimens were normal.

DNA content in renal cell carcinoma. A comparison between flow and static cytometric methods.

Different flow and static cytometric techniques for analysis of DNA content were compared using imprints, fresh and paraffin-embedded tissues from renal cell carcinomas. All methods discriminated between diploid-/near diploid and aneuploid samples and gave comparable results concerning modal ploidy levels (r = 0.744-0.970). For retrospective investigations of renal cell carcinoma, flow cytometry using paraffin-embedded tissues seemed to be the method of choice in determination of DNA content.
