Expectations as determinants of patient satisfaction: concepts, theory and evidence.

The apparent lack of conceptual agreement and the inconsistency in the approach to understanding expectations prompted this analysis of the literature in the field of patient satisfaction. A review of 18 journals over the last few years, as well as a number of relevant books, provided the evidence for the state of the current theory. An attempt has been made to distill the main definitions in use, to illustrate practical models of the relationship between expectations and satisfaction, to identify the influential personal and social variables, and to consider the special nature of health care. Some of the empirical methods and findings are then presented, with conclusions made about how the theory of expectations may be developed to assist in understanding patient satisfaction.

An overview of Spanish studies on appropriateness of hospital use.

This paper is an overview of hospital utilization review in Spain. Most of the hospital utilization studies have used the Appropriateness Evaluation Protocol as the review instrument. The studies, mainly retrospective, started in the late 1980s and used different adaptations of the medical-surgical version of the protocol. The level of inappropriate use detected ranges between 2.1 and 44.8% for admissions, and from 15 to 43.9% for inappropriate days of stay. The variability in the detected degree of inappropriateness may be due to the differences in the review instrument, the use of the override option, the patients included in the study, or the setting. The determinants of inappropriate hospitalization in Spain are mainly related to access to the different levels of care and to the conservative attitude of the physicians. Future utilization review in Spain should be followed by the development of specific interventions to correct the current pattern of overutilization.

Design and results of the nursing quality assurance program in Hospital de la Santa Creu i Sant Pau: an integrated effort.

This paper aims to describe and evaluate the results of the Nursing Quality Assurance (QA) Program in the Hospital de la Santa Creu i Sant Pau. The program was designed in 1987 as part of a global QA program involving all aspects of patients' care. The QA Program was organized at three levels: hospital, ward and joint medical-nursing. During the first 3 years of the study the following activities were carried out: (A) nursing process and results; (B) risk management; (C) patients' opinion; (D) utilization review; (E) ward nursing QA programs. Among other significant results, the program has detected that only 55.6% patients manifest that they sleep well at hospital, hygiene of dependent males (51%) is significantly worse than dependent females (81%) and patients' falls are more prevalent among oriented patients (73%) than disoriented patients (27%). Also a significative improvement has been demonstrated after implementing corrective measures in patients' information (7.5/10-8.1/10) and patients without supervision in emergency areas became from 35% to 0.02%. New nursing records were designed as a result of the QA assessment. The most important conclusion is that the Nursing QA Program is a valid instrument to evaluate general problems in the hospital and concrete problems in each ward.

Studies in utilization of drugs: a review of different methods.

Pharmaceutical expenses are 19% of social security expenses in Spain. Studies of prescription habits, prescription prevalence, therapeutic profiles, prescription compliance and incidence reports are thus required. Different methods to study utilization of drugs are discussed and examples of each method are provided. The number of registered pharmaceutical specialties varies between 21000 in Germany (2900 active substances) and 1000 (700) in Iceland. The thiazide consumption varies between 4.3 daily defined doses (DDD) per 1000 inhab./day in Spain and 42.9 in Sweden. Medication errors were caused by recording problems in 6.5% of all drug distributions.

The implementation of a quality assurance programme in the Hospital de la Santa Creu i Sant Pau in Barcelona.

The goal of quality assurance is to guarantee quality in medical care through the detection of problems with their subsequent evaluation and study, and through the implementation of corrective measures, when necessary. This article describes the criteria, methods, and results of a programme of quality assurance introduced in the Hospital de la Santa Creu i Sant Pau, Barcelona, Spain, in 1981. The programme covered the following aspects: evaluation of medical care services; process and outcome of medical care; use of available resources; and patient satisfaction. The results obtained over the last two years showed a substantial improvement in those areas which had been influenced by the programme, as well as the development of collective responsibility of the medical staff when faced with the problems of quality that have been detected.
