[Acute pulmonary infiltrates in hematologic malignancies in aplasia (author's transl)].

Extensive pneumonias are usually implicated as the sole cause for acute respiratory failure complicating severe neutropenia in hematologic malignancies and are often fatal. We report study of 11 patients investigated and treated in intensive care unit, using transtracheal aspiration, continuous positive airway pressure (CPAP) via face mask and granulocyte transfusions Two groups of patients emerged from this study. The first group with immediately diffuse pulmonary infiltrates, positive blood cultures, negative tracheal cultures, marked improvement in hypoxemia during CPAP, benefits from granulocyte transfusion without impairment in ventilatory status and may be considered as non-hemodynamic pulmonary oedema. The second group with localized pulmonary infiltrates, negative blood cultures and positive tracheal culture, slight improvement in hypoxemia during CPAP, gets no benefit from granulocyte transfusion, with additional impairment in ventilatory status and may be considered as acute extensive pneumonias.

[Type II familial hyperlipoproteinaemia : value of plasma exchange treatment (author's transl)].

A 13-year-old boy with homozygous type II familial hyperlipoproteinaemia with marked cutaneous and tendinous xanthomatosis has been followed-up for a 3-year period. Signs of unstable angina developed at the age of 14 years, followed by the onset of a posterodiaphragmatic infarction at 16 years-ion. The usual poor spontaneous prognosis, and the lack of efficacy of medical treatment on blood cholesterol levels, led to suggest the use of plasmapheresis. Treatment was interrupted after three months in spite of its efficacy (blood cholesterol levels reduced by 36.2 p. 100 by the 3rd month); in fact, marked alterations in ventricular function, as shown by non-invasive exploratory tests, risked to provoke an ischaemic accident from hypovolaemia during plasma exchanges. This method is nevertheless the only one currently available that can appreciably lower cholesterolaemia levels in this affection. The treatment, which is not devoid of risks, should be undertaken during the early stages, before ventricular function deteriorates due to progression of atheroma.

HLA-A3, B7 linkage disequilibrium in hemochromatotic patients with or without insulin dependent diabetes.

Sixty-six idiopathic hemochromatotic French patients were HLA-A, B typed. The previously known strong association with A3 was confirmed (RR = 10.6, P less 10(-9)) and our results indicate clearly that a gene implicated in idiopathic hemochromatosis (IH) determinism is located in the HLA-A region. The linkage disequilibrium between A3 and B7 was found to be far greater in IH patients than in controls. The authors have therefore hypothesized that this might be due to a selective advantage of this haplotype in IH. The A3, B7, Dw2 HLA haplotype has been shown to exert a protective effect against common insulin dependent diabetes (IDD). Thus the patients were divided into two groups according to the presence or absence of a definite IDD. B7 was found more frequently in IH patients without IDD but the difference is not significant. In this context, the strong linkage disequilibrium between A3 and B7 might be due to the protective effect of the B region of this haplotype against IH secondary IDD.

[HLA and erythrocyte genetic markers in a family with hereditary angioneurotic edema (author's transl)].

The study deals with a family of 22 members spreading over four generations; 14 members suffer from hereditary angio-neurotic edema; all of them have been typed for 30 antigens of the A and B loci in the HLA System and for ABO and Rhesus erythrocyte markers. There is no connection between the disease and any of the markers considered. As for the HLA system, there should be at least 7 chromosomic recombinants to account for the relationship with one of the haplotypes involved.

[Imported malaria: serological diagnostic and prevention of transfusional malaria (author's transl)].

Some particular aspects, epidemiological, clinical, serological of imported malaria are studied about 137 cases seen in the department of Tropical and Parasitologic Diseases of Cochin Hospital. The interest of studying several antigens, homologous and heterologous, in detection of fluorescent antibodies is discussed. In the case of any infesting Plasmodium species, the use of homologous antigen gives very superior titers. Systematic use of two antigens helps to detect possible antimalaria antibodies in blood donors and prevent transfusion given malaria.
