[Bone transplantation and AIDS].

The necessary obligation to carefulness at homogeneous transplantation of bone in concern of the AIDS epidemiology are being presented. According to the actual state of the HIV epidemiology a single examination of femur head donors contains an almost unmeasurable small risk of a virus transmission. Prior condition to an examination of the donor and to the usability of bones is the informed consent of the donor. Also important is the education of the recipient about the remaining risk, a provable documentation and a continuous observation of the HIV epidemiology.

[Intermediate-term results with the Mecron screw cup acetabulum--a follow-up study].

ISSUE: The short term result of the cementless implanted titan screw cups (type Mec-Ring) has been very encouraging. The study was aimed to show the survival rate in the middle term. METHOD: The survival rate of 111 implanted acetabular titan screw cups (type Mec-Ring) was determined in a retrospective study. 85 patients (= 76.5%) could be followed up after 6.7 years (5-8.5 years). RESULTS: 38 Implants had to be removed during the follow up period (34.2%). Taking into account the radiologic loosening in addition 22 cups were defined as failure (19.8%). Overall 36 (32.4%) cups showed excellent or good clinical results with 85 points according to the Harris-Hip-Score. 15 of the 22 radiologic loose implants showed only little or no pain. Considering the removed implants the probability of survival is 69% after 7 years. Defining the radiologic loose implants as failure the survival rate is 54%. CONCLUSION: X-rays have to be taken frequently to determine bone loosening early with subsequent revision operation. The survival rate is to low compared to other types of fixation. Therefore acetabular titan screw cups (type Mec-Ring) should not be used any more.

[Surgical treatment possibilities in aseptic and septic loosening of endoprostheses of the hip].

There is a permanent increase of replacement of endoprosthesis of the hip. The improvement of operation procedure and the application of spongious bone make it possible to reconstruct extreme defects of bone tissue of acetabulum and femur. The bacterial infection of the supporting tissue is the most serious complication in endoprosthesis. Even in early diagnosis and consequent therapy the infection often is not curable until endoprosthesis is removed. The resulting condition, which is similar to that seen after Girdlestone's operation, will mostly ensure an improved functional capacity of the hip joint.

[Spondylitis in the aged].

The spondylitis of the old patient is often late recognizable because of superposition with senile degeneration. The treatment is conform to the classical rules of musculoskeletal infections, after securing the diagnosis. The high risk of immobilisation in the age can be reduced by the operative therapy, but on the other hand must be an acceptance of the operative risk. Last named is very small, if the team is good acquainted with his work, accordingly short length of operation-time and without big histological lesions.

[Periarthropathia humeroscapularis].

Degeneration of the rotator cuff causes the painful periarthritis humeroscapularis. Especially the supraspinatus tendon is afflicted. 90-95% of patients with acute affection of supraspinatus tendon get free from symptoms with conservative treatment. Impingement syndrome of the rotator cuff will also get free from symptoms with conservative treatment. When the symptoms persist in spite of treatment, the operative enlargement of the acromiocoracoidal passage is indicated. Periarticular soft tissue calcification often becomes painful during spontaneous absorption of calcium deposits. Conservative treatment is in the foreground.
