Ductal carcinoma of the pancreas. Rationales for total pancreatectomy.

In order to evaluate total pancreatectomy as a surgical procedure for ductal carcinoma of the pancreas, a histopathological analysis was made on 18 resected specimens with special regard to the pattern of cancer growth in the pancreatic tissue. In five of them there was no lymphatic involvement or extrapancreatic invasion, but cancer extended continuously to the tail along with the pancreatic ducts and reached the end of the ducts in three cases. All 11 patients treated with Whipple's procedure died of recurrence, while of four total pancreatectomized patients, one with continuously invasive cancer to the end of the pancreatic duct has been living more than eight years postoperatively. We believe that total pancreatectomy for this type of "intraductal spreading cancer" without invasion beyond the pancreas is indicated as a radical procedure.

Malignant melanoma producing serotonin.

A case of malignant melanoma, the tumor of which seems to produce serotonin, is presented. Neither hyperserotoninemia nor carcinoid syndrome was observed in the clinical course. An autopsied sample of the metastatic lung tumor contained a significantly higher concentration (1.35 microgram/g wet weight tissue) of serotonin than that of normal lung (below 0.5 microgram wet wt.). Tryptophan hydroxylase activity, the key enzyme of the biosynthesis of serotonin, was substantial in the same tumor sample when assayed by a sensitive radioisotopic method, supporting the productivity of the amine of the tumor. The ability of melanoma to produce serotonin could be attributed to its neural crest origin.

Primary adenoid cystic carcinoma of the esophagus: report of a case.

A case of primary adenoid cystic carcinoma of the esophagus is reported. A 51-year-old male patient had a tumor in the lower third of the esophagus which was incidentally found during an examination for cholelithiasis, and resected successfully. The tumor exhibited a polypoid appearance covered by normal esophageal epithelium, localized entirely in the submucosal layer, and morphologically identical to adenoid cystic carcinoma in the salivary glands. The patient is still alive and well three and one-half years after surgery. This seems to be a typical case of adenoid cystic carcinoma of the esophagus arising from the submucosal esophageal gland.

Carcinoembryonic proteins in gastric carcinoma metastatic to the liver.

Two cases of liver metastases from gastric carcinoma are described in which the simultaneous occurrences of alpha-fetoprotein (AFP), carcinoembryonic antigen (CEA) and carcinoplacental alkaline phosphatase (CPALP) were demonstrated in the sera and tumor tissues. AFP was detected not only in the tumor tissues but also in the liver tissue adjacent to the tumor, while the other 2 carcinoembryonic proteins were not detected in the non cancerous liver tissues. The characteristics of CPALP in Case 1 were almost similar to the Nagao isoenzyme, based on enzyme tests involving L-leucine, L-phenylalanine and EDTA inhibitions, heat-stability and Michaelis constant, except for electrophoretical slower moving, while that in Case 2 were identical to variant type CPALP (Warnock).

Demonstration of argyrophil granules in small cell carcinoma of the lung.

The Grimelius silver nitrate stain has enabled us to demonstrate the presence of tumor cells with argyrophil granules (argyrophil cells) in small cell carcinoma of the lung. Of the 22 tumors, 11 showed varying numbers of argyrophil cells. The occurrence of the cells differed in frequency among the subtypes of small cell carcinoma. The fusiform cell type showed the cells more frequently than the other types. Both tumors with numerous argyrophil cells belonged to the fusiform cell type. The number of positive cells seen under the light microscope did not correlate with the number of cells containing neurosecretory granules under the electron microscope, nor with the amount of either ACTH or serotonin in the tumor extracts. The demonstration of these cells in a pulmonary carcinoma may be of help in making correct histological diagnosis.

Production of several substances by medullary carcinoma of the thyroid.

Two patients with the typical morphology of medullary carcinoma of the thyroid are reported. Biochemical analysis of the tumor tissue demonstrated the presence of calcitonin, ACTH and MSH, although symptoms referable to these hormones were hardly manifested. Serum CEA levels were elevated and CEA immunofluorescences in the tumor cells were positive. Moreover, in one patient, serotonin concentration was raised in the tumor tissue, and gastrin level was also elevated in the peripheral blood. These results suggest that the tumors may produce several substances.

A cytologic study on small cell carcinoma of the esophagus.

The cytologic picture of small cell carcinoma primarily arising from the esophagus was studied with 7 cases which were confirmed by histologic examination. Cytomorphologic characteristics of small cell carcinoma of the esophagus are as follows: the arrangement of groups of tumor cells is irregular and overlapping with indistinct cell boundaries. The cytoplasm is small, or sometimes absent. The nuclei are round, oval or occasionally spindle shaped. Nuclear borders are thin. The chromatin of finely granular pattern has increased and is evenly distributed.

Coexistence of bilateral pheochromocytoma and pancreatic islet cell tumor: report of a case and review of the literature.

A 14-year-old Japanese male with a previously undescribed combination of bilateral pheochromocytoma and an islet cell tumor of the pancreas is presented. The combination of endocrine neoplasms in this patient overlaps multiple endocrine neoplasia (MEN) Type 1 and Type 2. A total of 14 reported cases of MEN overlapping Type 1 and Type 2 is reviewed. Of the 14, 7 patients with acromegaly developed a paraganglioma(s), 2 patients with Sipple syndrome had a pituitary adenoma, and in the other 5 patients, an intestinal carcinoid or a pancreatic islet-cell tumor occurred in association with either a thyroid medullary carcinoma or a paraganglioma(s). We believe that the occurrence of MEN overlapping Type 1 and Type 2 is more than a fortuitous association, and can be explained on the basis of the neuroectodermal origin.

Mechanisms of late recurrence after radical surgery for gastric carcinoma.

Factors involved in late recurrence of gastric carcinoma were investigated in 25 patients with late recurrence who died five or more years after radical surgery and 129 with early recurrence who died within two years after surgery. In the patients with late recurrence, the important routes of cancer spread in the initial phase of the recurrence were contiguous invasion and lymphatic spread. Whether early or late recurrence occurred had a high correlation with the following four conditions: (1) the amount of residual cancer left at the time of surgery; (2) the spread of the residual cancer; (3) the rate of cancer proliferation; and (4) the resistance of the host. To prevent late recurrence, it is necessary not to leave any cancer tissue in these routes at the time of gastrectomy, as well as to employ adjuvant chemo- and immunotherapy for the inhibition of cancer proliferation and elevation of the host resistance.

Argyrophil cell carcinoma (apudoma) of the esophagus. A histopathologic entity.

In a series of 79 cases of primary esophageal carcinoma resected at The Center for Adult Diseases, Osaka, there were six tumors with specific histopathologic features valid for the diagnosis of argyrophil cell carcinoma. Of the 6 tumors, 3 were studied electron microscopically and assay for ACTH content was performed on 4 tumors. Clinically, the ages of the 6 patients ranged from 56 to 71 years; two were women and four men. Four of the 6 patients died with widespread tumor recurrences within 9 months of operation. Microscopically, the 6 tumors were composed largely or almost entirely of small, spindle-shaped cells resembling those of oat cell carcinoma of the lung, and were characterized by the arrangement of tumor cells in solid sheets or anastomosing cords, the presence of argyrophil tumor cells, and the deposits of amyloid. Electron microscopically, the three tumors contained neurosecretory-type granules. Using bioassay or radioimmunoassay ACTH activity in the tumor tissues was detected in 3 out of the 4 tumors determined. From the light and electron microscopic characteristics and the assay evidence, it seems reasonable to conclude that the 6 tumors are endocrine polypeptide producing tumors (apudomas) that arise from argyrophil cells normally found among the basal cells of the esophageal mucosa, and that they represent a distinct histopathologic entity clearly distinguishable from other types of esophageal carcinomas.

Argyrophil cell carcinomas (apudomas) of the uterine cervix. Light and electron microscopic observations of 5 cases.

Of a series of 97 invasive carcinomas of the cervix, 5 were found to have argyrophil tumor cells, and 3 of these 5 tumors were studied by electron microscopy. The ages of the 5 patients ranged from 36 to 49 years, with a mean age of 42.4 years. The morphologic features of these five tumors were well consistent with those described on a variety of endocrine polypeptide neoplasms such as thyroid medullary carcinomas, carcinoids, pancreatic islet-cell tumors, and oat cell carcinomas of the lung. Microscopically, the 5 tumors were characterized by the formation of solid-sheets, ribbons, streams, and rosettes. They were characterized electron microscopically by the presence of neurosecretory-type granules, the abundance of intracytoplasmic microfilaments, the absence of tonofibrils, and the paucity of desmosomal attachments. On the basis of the microscopic, electron microscopic and cytochemical characteristics, it is suggested that the tumors are a specific type of cervical carcinoma derived from the argyrophil cells, normally found among the linings of the endocervical glands and the cervical squamous epithelium. We believe these 5 tumors should be regarded as an endocrine tumor, another member of apudomas.

Argentaffin granules in endocrine cells in human duodenal mucosa.

An attempt was made to gain ultracytochemical characteristics of the endocrine cells in the human duodenal mucosa. Bodian argyrophil and Fontana-Masson argentaffin reactions were applied to the blocks of the tissue. The ultracytochemistry showed that the secretion granules of all morphological types of the endocrine cells were precipitated with both the argyrophil and argentaffin silver grains in varying intensity. The endocrine cells containing uniformly round secretion granules tended to show a heavy precipitation with the argyrophil silver grains in contrast to a light precipitation with the argentaffin silver grains. The endocrine cells containing pleomorphic secretion granules were heavily precipitated with both the argyrophil and argentaffin silver grains. The findings of the present study suggest that the duodenal endocrine cells represent a single cell type in respect to 5-hydroxytryptamine metabolism, but differ in their metabolism of other polypeptide hormones.
