Intravenous cidofovir treatment for recalcitrant warts in the setting of a patient with myelodysplastic syndrome.

Cidofovir is an acyclic nucleoside phosphonate with broad-spectrum activity against DNA viruses, including human papilloma virus (HPV). However, data on the efficacy of cidofovir in an immunosuppressive setting remain contradictory. We report for the first time on the promotion of the healing of recalcitrant warts in a patient with myelodysplastic syndrome with intravenous cidofovir treatment.

Cutaneous T cell lymphoma and classic Hodgkin lymphoma of the B cell type within a single lymph node: composite lymphoma.

Composite lymphomas are defined as two unrelated, morphologically and genetically distinct lymphomas occurring at the same point in time within the same tissue or organ. Since their original definition, several composite lymphomas have been reported exclusively based on morphological grounds. However, with the application of immunohistological and molecular biological techniques it has become evident that many so called "composite" lymphomas do not fulfil the necessary criteria, because they merely represent two different morphological phenotypes of the same malignant clone. This report describes the manifestation of a true composite lymphoma within a single cervical lymph node, which is composed of a cutaneous T cell lymphoma and a classic Hodgkin lymphoma of B cell type--a very rare finding indeed.

[The vacuum sealing technique. A new procedure to cover soft tissue defects after resection of leiomyosarcoma].

The vacuum sealing technique is a new, simple to use procedure for traumatic and chronic soft tissue defects, burns and soft tissue infections. We used this technique for the first time for temporary closure after the resection of deep infiltrating leiomyosarcoma. Radical excision of the tumor with partial resection of the forearm flexor muscles and safety margin of 3 cm led to a large skin defect (12 x 16 cm). For temporary closure of this defect we used the vacuum sealing technique for 7 days. Because of histologically proven incomplete resection of the tumor, we did a second excision and used the vacuum sealing technique a second time for 7 days. After this time period the granulation of the wound ground was excellent, and we closed the defect by split skin transplantation from the thigh. Bacterial infection or other complications were not seen during use of vacuum sealing technique. The vacuum sealing technique induced optimal local conditions for skin grafting: it facilitated granulation tissue production and maintained a clean wound bed without the necessity of changing wound dressing before skin grafting. This technique is painless and does not impair mobility.

Adamantiades-Behçet's disease: interleukin-8 is increased in serum of patients with active oral and neurological manifestations and is secreted by small vessel endothelial cells.

The serum levels of several cytokines were determined in 94 patients with Adamantiades-Behçet's disease (ABD), aged 36.1+/-11.0 years, during the active stage (n = 75) and the inactive stage (n = 19) of the disease. A group of 75 healthy individuals matched for age and sex served as controls. Cytokine levels were determined using commercially available ELISA kits. Of the 75 patients with active disease and 19 with inactive disease, 38 (51%) and 4 (21%), respectively, and 23 healthy controls (31%) were found to have detectable levels of interleukin 8 (IL-8) in their serum (P < 0.05). Also, increased IL-8 serum levels were found in patients with active disease (median 12 pg/ml, P = 0.010) compared to patients with inactive disease (< or = 10 pg/ml) and to healthy controls (< or = 10 pg/ml). In particular, patients with oral aphthous ulcers (n = 51, 34 pg/ml) and neurological features (n = 4, 71 pg/ml) exhibited increased IL-8 levels. In contrast, there was no correlation between disease activity and the serum levels of IL-1alpha, IL-1beta, tumor necrosis factor alpha (TNF-alpha), soluble intercellular adhesion molecule-1 or basic fibroblast growth factor (bFGF). In a second set of experiments, the involvement of dermal microvascular endothelial cells in IL-8 secretion was investigated. Immortalized human dermal microvascular endothelial cells (HMEC-1 cells) were maintained for 4 h in vitro with serum from 18 ABD patients or with IL-1beta, a known stimulator of IL-8 synthesis, TNF-alpha or their combination at five- to tenfold higher concentrations than those found in the serum of ABD patients. Increased IL-8 secretion was found after incubation with ABD patients' serum (median 20 pg/ml), but IL-1beta, TNF-alpha and IL-1beta + TNF-alpha failed to induce IL-8 secretion by HMEC-1 cells (< or = 1-1.2 pg/ml) in biologically relevant concentrations. Our study showed increased IL-8 serum levels in ABD patients with active oral and neurological manifestations. Human microvascular endothelial cells may, at least partially, be responsible for the enhanced IL-8 secretion in the active stage of the disease.

Increase in sensitization to oil of turpentine: recent data from a multicenter study on 45,005 patients from the German-Austrian Information Network of Departments of Dermatology (IVDK).

Contact allergy to oil of turpentine was reported to have become rare. However, the evaluation of standardized data of 45,005 patients tested 1992-1997 in 30 Dermatological Centers associated with the German-Austrian Information Network of Departments of Dermatology (IVDK) showed an increase in positive patch test reactions to turpentine from 0.5% during the years 1992-1995, up to 1.7% in 1996 and 3.1% in 1997. In particular, 17,347 patients tested in 1996-1997 were evaluated in detail by comparing 431 individuals with positive patch test reactions with the rest of the group found negative to turpentine. Using the so-called MOAHLFA index, the following characteristics were shown. Turpentine allergy (a) was found to be significantly less frequent in men and in patients with occupational dermatitis, (b) showed no difference in its association with atopic dermatitis, (c) patients with turpentine allergy had significantly less symptoms of the hands, more symptoms of the legs or in the face and (d) were significantly more often aged over 60 years. Also, patients sensitized to turpentine had increased rates of additional sensitizations. The definite reason for the increase in turpentine sensitization in the population tested here is not clear. Therefore, a detailed exposure analysis is necessary; the new increase in turpentine allergies may be due to popular topical remedies or household chemicals.

Occupational contact dermatitis due to 2-chloracetophenone tear gas.

2-Chloracetophenone (CN) is widely used as tear gas by police and civilians for self-defence. It may affect the eyes, respiratory system and skin, sometimes causing serious injuries. Both irritative and allergic contact dermatitis have been described. We report three police officers who experienced accidental escape of CN from their professional tear gas canisters. All of them showed localized dermatitis at the site of contact to CN, while widespread lesions appeared after 4 days in one case. Patch tests with the original involved tear gas dissolved in acetone (at 0.1-0.0001%) indicated an allergic reaction in two patients and an irritative reaction in the third. Occupational contact dermatitis due to CN seems to occur among police officers more often than is generally known. Infrequently, extensive health problems may be caused by CN when lesions spread over the integument. Therefore, an improvement of safety measures in occupational CN gas use is needed, especially aiming at avoidance of accidental leakage of canisters.

Twenty-eight cases of juvenile-onset Adamantiades-Behçet disease in Germany.

BACKGROUND AND OBJECTIVE: Adamantiades-Behçet disease is a rare entity at a juvenile age. We aimed to enlighten epidemiological and clinical characteristics of juvenile-onset disease in Germany. METHODS: Data from the German Registry were used to compare clinical and epidemiological features of patients with juvenile-onset (</=16 years) and adult-onset (>16 years) disease diagnosed according to the criteria of the International Study Group. RESULTS: Twenty-eight (17%) of 168 patients of the German Registry exhibited the onset of the disease and 8 (5%) of them the complete symptom complex at a juvenile age. Juvenile-onset disease was characterized by an increase in familial cases (25 vs. 8% in patients with adult-onset; p = 0.047). The frequency of diagnostic signs was similar between the two study groups. In juvenile-onset disease, delayed development of the complete symptom complex (median value 35 months vs. 12 months after onset; p = 0.014) and lower prevalence of severe complications (9 vs. 29%; p = 0.042) were detected. CONCLUSIONS: The major clinical features of juvenile-onset and adult-onset disease in Germany are comparable, but in juvenile-onset disease, the course is delayed and patients experience less severe complications. In addition, there is a higher rate of familial occurrence of the disease in patients with juvenile-onset.

[Acute generalized exanthematous pustulosis, induced by nystatin].

Despite its widespread use, the antimycotic nystatin rarely causes allergic reactions. We observed a case of acute generalized pustular exanthem with arthralgia and fever, after the oral administration of nystatin. A causal relationship was demonstrated by positive Intracutaneous testing of the drug. Immunohistology revealed positive staining for C3 in the test area, whereas circulating immune complexes could not be identified. These findings are compatible with a type-II allergic reaction. Systemic prednisolone treatment led to a complete remission of the clinical symptoms within 11 days. In two test subjects who had never received nystatin intracutaneous testing was initially negative. After 3 weeks, both developed itching erythematous papules at the injection sites. Therefore, intracutaneous testing of nystatin involves the risk of sensitization.

[Standardized rapid hyposensitization with purified hymenoptera venom in wasp venom allergy. Prospective study of development of tolerance and side-effect profile].

In Germany, insect allergies cause about 10-40 death per year. Starting a specific immunotherapy (SIT) and its maintenance may be lifesaving for the allergic patients. Performing a prospective study, we exactly documented effects and side effects during the induction phase of SIT using a purified aqueous yellow jacket venom preparation in 20 patients allergic to yellow jacket venom (clinical degree II-IV). Indication for the therapy was defined individually for each patient and was based on a score incorporating the following criteria: a) clinical symptoms, b) in-vivo- and in-vitro testing, c) risk of re-sting. SIT was started under inpatient conditions following a standardized 8 day protocol. At the end of the induction phase all patients tolerated the maintenance dose of 100 micrograms of yellow jacket venom without complications even though all showed local side effects (threshold doses 0.01-10 micrograms, median value 0.2 microgram). 40% of all patients showed subjective side effects (median value of threshold dose 30 micrograms, range 0.2-60 micrograms) and 30% showed objective side effects (median value of threshold dose 55 micrograms, range 20-80 micrograms); however, all side effects were clinically mild. When objective systemic side effects were seen, the cumulative daily dose was 39-210 micrograms (median value 135 micrograms), exceeding 100 micrograms in 83% of patients. Our prospective study confirmed that the chosen protocol for SIT is effective in inducing tolerance to the venom preparation. However, using a purified aqueous yellow jacket preparation did not result in reduction of side effects.

[Allergies and pseudoallergic reactions to anesthetics. The clinical symptoms, risk factors and the diagnostic possibilities].

Nearly all drugs currently used during the course of general anaesthesia may lead to hypersensitivity reactions of various types. There may be an acute type I allergic reactions or to a more or less severe pseudo-allergic reactions, in rare cases with lethal outcome. Routine preoperative testing appears of little predictive value, in spite of the high frequency of so-called risk factors (atopy, other kind of allergy) among the evaluated group of patients. Careful allergological testing should be performed 4 to 6 weeks after any incidents of suspected drug intolerance, in order to discover the underlying causative agent. Skin testing is of diagnostic value for neuromuscular blockers and intravenous narcotics. RAST- and RIA-tests and/or mediator releasing tests may also used additionally. Together with all other administered drugs, all the routinely used neuromuscular blockers (suxamethonium, vecuronium, pancuronium, alcuronium, atracurium, mivacurium) should be tested, since they often represent the cause for such reactions. For other classes of drugs (for example, volatile anesthetics and opioids) the clinical relevance of skin testing still remains uncertain. For less severe incidents seen during general anaesthesia such as pruritus, or exanthema, skin testing seems to be less relevant.
