Approximate statistics of gapped alignments.

A heuristic approximation to the score distribution of gapped alignments in the logarithmic domain is presented. The method applies to comparisons between random, unrelated protein sequences, using standard score matrices and arbitrary gap penalties. It is shown that gapped alignment behavior is essentially governed by a single parameter, alpha, depending on the penalty scheme and sequence composition. This treatment also predicts the position of the transition point between logarithmic and linear behavior. The approximation is tested by simulation and shown to be accurate over a range of commonly used substitution matrices and gap-penalties.

Bridging the gap or damming the flow? Some observations on using interpreters/bicultural workers when working with refugee clients, many of whom have been tortured.

This paper will comment upon some of the dilemmas inherent in the task of using interpreters/bicultural workers when working with refugee clients who have fled to Europe, many who have been tortured. A high proportion of refugees may not speak European languages or share explanatory health beliefs. The author would argue that these differences should not act as barriers to obtaining health and counselling services and that employing interpreters/bicultural workers may help to bridge these gaps and improve service provision and delivery.

Successful treatment of severe Rh iso-immunization with immunosuppression and plasmapheresis.

Nine patients with severe Rh iso-immunization were treated by plasma exchange combined with immunosuppression. Apart from 3 abortions, previous pregnancy losses consisted of 7 intra-uterine and 5 neonatal deaths. Only 2 patients had had no previous pregnancy loss. Differences in the optical density of the amniotic fluid of 8 patients fell into the upper Liley zone. There was one intra-uterine death due to abruptio placentae but no neonatal deaths. When the outcome of the pregnancy immediately preceding the treatment pregnancy was compared to the treatment pregnancy, the fetal loss was reduced from 6 to 1. No adverse fetal effects were encountered.

Gaucher's disease in the black population of South Africa. A case report.

The diagnosis of the non-neuropathic form of Gaucher's disease was confirmed by haematological and enzymatic investigations in a Black girl. The aetiological relationship of this condition with Gaucher's disease in other populations is uncertain, but lack of expression of the enzyme defect in the white blood cells in our patient might be significant in this context. This case serves to emphasize that Gaucher's disease enters into the differential diagnosis of unexplained splenomegaly, irrespective of the ethnic background of the affected person.
