Albumin as a tear supplement in the treatment of severe dry eye.

AIM: To determine the efficacy of using albumin tear supplements in the treatment of ocular surface disorders as a substitute for serum eye drops. METHODS: The effects of albumin on the viability of serum deprived conjunctival cell were observed in vitro. The ability for albumin to compensate for serum was demonstrated by measuring the activity of the apoptosis related enzyme, caspase-3. In an animal study, corneal erosions were inflicted in 40 Japanese white rabbits. Rabbits were treated with 5% or 10% solutions of human albumin, and the decrease in epithelial defect size was compared with saline control and 0.3% sodium hyaluronate. A clinical case series trial of 5% albumin drops was conducted in nine patients with Sjögren's syndrome with severe dry eye. RESULTS: The addition of albumin to serum deprived conjunctival cells inhibited caspase activity and increased cell viability, showing that albumin can compensate for some of the physiological properties of serum. Corneal erosions in rabbits healed significantly faster (p<0.05) in eyes treated with 10% albumin compared with control and sodium hyaluronate. Patients with Sjögren's syndrome used albumin drops showed statistically significant improvement in fluorescein and rose bengal scores, but not in tear break up time and subjective symptoms. No adverse effects of albumin were observed during the study. CONCLUSIONS: The use of albumin as a protein supplement in artificial tear solutions is a viable approach in the treatment of ocular surface disorders associated with tear deficiency.

A report of two cases treated with Pauwels' femoral osteotomy for advanced osteoarthritis resulting from a sequela of infectious coxitis in childhood.

Pauwels' femoral valgus osteotomy was performed on two osteoarthritic hips that developed as a sequela of infectious coxitis in the childhood. One of the hips was diagnosed as osteomyelitis of the ipsilateral femur before the operation and was treated with curettage and implantation of antibiotic-impregnated cement beads. The results of both hips were excellent and maintained for 9-11 years.

Increase of hybrids in yeast electrofusion by mitochondrial mutations.

Yeast mitochondria were found to affect the zeta-potential of protoplasts, resulting in electrofusion of membrane behavior. For modeling purposes, two fusion systems were investigated: (1) parent yeasts of Saccharomyces cerevisiae G706 (rho(+)) x O708-11-16A (rho(+)), with zeta-potentials of -10 to -27 mV and -28 to -45 mV, depending on MgCl(2) concentration in the medium, respectively; and (2) parent yeasts of S. cerevisiae G706-1 (rho(-)) x O708-11-16A (rho(+)), with zeta-potentials of -30 to -60 mV, depending on MgCl(2). Yields of the hybrids in system (2) were over 100-fold higher than those in system (1). Thus, regulation of yeast electrofusion was found to be possible by mitochondrial mutations.

Factors influencing joint-preserving operations in the treatment of the late stages of osteoarthritis of the hip.

Between November 1983 and December 1992, 136 hips (119 patients) with coxarthritis were operated on using joint-preserving techniques based on the rationale of Pauwels' osteotomy. The criterion for selection was a patient in whom the height of the joint space in the weight-bearing area of the hip was less than 1 mm. The mean age at operation was 48 years and the mean follow-up 109 months (60 to 171). Hips were categorised using Bombelli's classification of osteoarthritis, into atrophic and non-atrophic types. The endpoint was defined as that at which the height of the joint space became less than 1 mm again. The Kaplan-Meier curve showed that the rate of survival of the non-atrophic group was significantly better than that of the atrophic group. Cox's proportional hazard model indicated that the factors influencing the results of joint-preserving operations included Bombelli's classification, postoperative incongruence of the joint and the height of the joint space.

[Development of Sweet's syndrome during all-trans retinoic acid therapy for acute promyelocytic leukemia].

A 54-year-old woman visited our hospital because of gingival bleeding on May 31, 1998. After hematological and bone marrow examinations, she was diagnosed as having acute promyelocytic leukemia (APL) and given all-trans retinoic acid (ATRA) therapy starting on June 1. Anti-cancer drugs were administered for 5 days from June 12 because of an increase in the number of APL cells. The patient developed fever on June 20, and nodular erythematous eruptions appeared on June 23. Sweet's syndrome was diagnosed from biopsy samples of the eruption. ATRA was therefore discontinued, and prednisolone was started on June 29. The fever and skin eruptions improved rapidly, and complete remission was obtained on July 13. Sweet's syndrome due to ATRA may be a partial form of retinoic acid syndrome, in which the differentiated leukemic neutrophils increase and invade various organs. However, Sweet's syndrome must be considered regardless of the WBC count because in this case the syndrome occurred even when the WBC count was not high.

Middle-term results of anatomic medullary locking total hip arthroplasty.

We report the results of cementless, anatomic, medullary locking hip prosthesis application in our first consecutive series. We used the so-called Asian size of prosthesis with proportionally smaller stem sizes in both diameter and length. Forty-seven stems and sockets were analyzed with a mean follow-up of 69 months. The mean Merle d'Aubigné hip scores were 8 points preoperatively and 16 points at the final follow-up. Radiologically, the stems showed excellent stability without loosening. Stress shielding around the stems did occur in most cases but did not progress. Preoperative bone quality influenced the extent of stress shielding evaluated at the final follow-up: higher stress shielding was noted in poorer quality bones at the time of operation. There were problems with the sockets. The shallow socket and impingement at the protruded rim seemed to cause a high incidence of dislocation (13%). Massive polyethylene wear occurred in 5 sockets. These sockets were 48 and 46 mm in diameter with 26 mm heads. In conclusion, the stems of the anatomic medullary locking hip prostheses used in Japan showed satisfactory stability even in poor quality bones, but there were problems with the polyethylene liners. Our solution was to use larger sockets with 22 mm heads.

Tonic fiber in Mauremys (Clemmys) japonica jaw muscle.

The jaw muscles of a turtle [Mauremys (Clemmys) japonica (MCJ)], have been analyzed for the histochemical characteristics of their fiber types. Six jaw muscles of the MCJ have been analyzed for histochemical characteristics of their fiber types. In this study, fiber types in two groups of muscles are classified; 1) fast twitch glycolytic (FG), fast twitch oxidative glycolytic (FOG), slow twitch oxidative (SO), and tonic fibers, according to the system of Putnam et al. (1980); 2) reacted differently in similar histochemical tests and classified, respectively, as various types. Especially tonic fibers constituted low percentage in the MAMP (ca., 0.3%), MI (ca., 3.9%) and MPtp (ca., 1.0%) were found. These results suggested that the tonic fiber plays a small part of the function in jaw movement.

Quantitative morphology of dermal elastic fibers system of the human face during aging.

Human skin has various distributions and arrangements of elastic fiber (EF). Previous reports did not clearly show the distribution of EF in the face skin because of various contents during aging. In this study, a color image analyzer indicated distribution of elastic, oxytalan, and muscle fibers in human face skin. During aging the muscle fiber size and the content of the EF decreased in the modiolus and inferior labial regions of the human skin, and the ratio of the EF was lower than that of oxytalan fiber measured areas. That is, the dimension of oxytalan fiber may reflect the content of EF, and muscle has a role in the distribution of the EF in human face skin. In the deeper regions, small and large EF bundles were found near the sheath of gland and muscles. Therefore, face movement might be an important aspect to maintaining the EF content of human face skin.

Heel lifting as a conservative therapy for osteoarthritis of the hip: based on the rationale of Pauwels' intertrochanteric osteotomy.

Patients with osteoarthritis of the hip were treated with a conservative therapy of heel lifting. Orthoses were applied on 35 hips in 33 subjects and the cases were followed for 23 months on average. Dramatic pain relief was reported, but the time required to reduce or completely relieve pain increased according to the stage of osteoarthritis. The radiological results were not satisfactory. During the follow-up, only two hips showed improvement, 22 showed no change, and 11 deteriorated. The mechanism of heel lifting in relation to the hip joint was analysed, showing that pelvic obliquity was achieved and the trunk stabilized. In conclusion this simple orthosis was effective as a palliative therapy for osteoarthritis of the hip.

High-performance liquid chromatographic method for the rapid and simultaneous determination of sulfamonomethoxine, miloxacin and oxolinic acid in serum and muscle of cultured fish.

A rapid method for the simultaneous determination of sulfamonomethoxine (SMM), miloxacin (MLX) and oxolinic acid (OA) in serum and muscle of cultured fish by high-performance liquid chromatography has been developed. A Hisep shielded hydrophobic phase column (15 cm x 4.6 mm I.D.) and a mobile phase of 0.05 M citric acid-0.2 M disodium hydrogenphosphate buffer, pH 2.5 in 10 mM tetra-n-butyl ammonium bromide-acetonitrile (85:15) with ultraviolet detection at 265 nm were used. The recoveries of SMM, MLX and OA from serum and muscle samples were 72-101%. The detection limits of the three drugs were 0.05-0.1 microgram/ml or g of sample.

[Study on expression of proliferating cell nuclear antigen (PCNA) in ovarian cancer].

Specimens of ovarian cancers, which were obtained through laparotomy from 17 patients who received no preoperative chemotherapy and were examined histopathologically, were immunohistologically stained with anti-PCNA antibody to examine the ability to proliferate. The percent age of stained tumor cells per 1,000 cells was used as a PCNA-labeled rate. This rate was analyzed in relation to the clinical stage/histological type and the presence or absence of lymph node metastasis. 1. Tissue specimens were stained with anti-PCNA antibody in all cases, and the PCNA-labeled rate was 49.5 +/- 13.3%. 2. There were no significant differences in PCNA-labeled rates between different histological types or between clinical stages. 3. The PCNA-labeled rates was significantly higher in cases with metastasis than in those without metastasis (p < 0.01). 4. No metastasis was detected in the cases showing a PCNA-labeled rate of 50% or below. It was indicated that this PCNA-labeled rate, which can be determined with formalin-fixed specimens, is useful for evaluating the degree of biological malignancy of ovarian cancer.

Distribution of the macromolecular components of masticatory muscles during differentiation of the muscle fibers in the postnatal rat.

The distribution of collagen fibers of rat masticatory muscles during the postnatal period (two weeks), was investigated by electrophoresis and immunohistochemistry. At these stages, the myosin of rat masticatory muscles displays specific electrophoretic patterns. Comparison of the myosin patterns of these muscles allows their identification. 1) Analysis by SDS-PAGE indicated that one of three weakly reactive stainable proteins with lower mobility than the heavy chain of myosin disappeared from the temporal muscle on day 13, as compared with other masticatory muscles. However, in histochemical analysis of the muscle fibers, the reaction specific for succinic dehydrogenase (SDH) activity was strong, and the fibers on day 13 could be classified into two types with respect to SDH activity. By contrast, on day 0, the fibers were classified into two types with respect to myosin ATPase activity. 2) Immunohistochemical analysis indicated that the distribution of the components of the extracellular matrix in the epimysium (type I collagen), perimysium (type I collagen, fibronectin, and laminin) and endomysium (type III collagen, fibronectin, laminin, and tenascin) was related to the metabolic capacity on days 12 to 13. The variability in the types of myosin and in proteins of the extracellular matrix might be important during the development of rat masticatory muscles.

Joint-preserving operations for elderly patients with advanced and late-stage coxarthritis.

Forty-one hips in 38 elderly patients with osteoarthritis of the hips, operated on with joint preserving techniques at the age of 55 years or more, were analyzed clinically radiologically. Bilateral involvement was seen in 13 cases. Ten hips were operated on after this study, before the patients were 55 years old, or no operation has yet been done. The average age at operation was 59 years, and the average preoperative hip scores (Merle d'Aubigné) were as follows: pain 2.8; range of motion (ROM) 4.4; gait 3.4. Average preoperative hip flexion and abduction were 88 degrees and 20 degrees respectively. Mean follow-up period was 57 months, and mean final hip scores were: pain 5.1; ROM 4.6; gait 4.0. Significant improvement was seen in pain and gait scores but no improvement in ROM scores. Excellent and good scores were achieved in half of the cases. Failure was seen in four cases. Hip pain recurred in nine patients. Five out of these patients were classified as atrophic according to Bombelli's classification of osteoblastic response of osteoarthritis of the hip. The responsiveness of the femoral head influenced the clinical results of the joint-preserving operations.

Human neuroblastoma cells produce the NF-kappa B-like HIV-1 transcription activator during differentiation.

Human neuroblastoma GOTO and IMR32 cells differentiate into neural cells on treatment with 5-BrdU. Here we report that in association with differentiation induced by 5-BrdU, the transcription of HIV-1 was activated in both neuroblastoma cell lines. An NF-kappa B-like factor was also found to be produced in association with the neural cell differentiation of these cells. These results suggest that, during differentiation of neural cells, an NF-kappa B-like factor was produced, and that when the neural cells are infected with HIV-1 the production of NF-kappa B results in activation of transcription of HIV-1. Thus this NF-kappa B-like factor may be involved in the mechanism causing AIDS dementia.

[The clinical evaluation of UF-021, a new prostaglandin related compound, in low tension glaucoma patients].

We administered UF-021 isopropyl unoprostone (Rescula) ophthalmic solution, a new prostaglandin-related compound developed as an anti-glaucoma agent, to 13 low tension glaucoma patients for 24 weeks, and clinically evaluated the effect of the intraocular pressure (IOP) reduction, visual field change, and side effects of the agent. Intraocular pressure was reduced in 11 out of 12 patients who were studied for statistical analysis. In these 12 patients, significant IOP reduction was recognized in the mean IOP for 24 weeks after the administration of UF-021. No side effects were detected in any of the 13 patients. From these results, UF-021 could be used as a new anti-glaucoma agent for low tension glaucoma, and also safer than many other anti-glaucoma agents.

Determination of oxytetracycline in blood serum by high-performance liquid chromatography with direct injection.

A direct injection analysis by high-performance liquid chromatography has been developed for oxytetracycline in serum of animals and fish. A Hisep shielded hydrophobic phase column (15 cm x 4.6 mm I.D.) and a mobile phase of methanol-0.2 M oxalic acid (10:90, v/v, pH 7.0) with ultraviolet detection at 360 nm were used. The standard calibration curves in serum of chicken, hog, cattle and rainbow trout were linear over the range 0.1-20 micrograms/ml. The recoveries of oxytetracycline from all serum samples determined at two different concentrations (0.5 and 2.0 micrograms/ml) were 88-103%. The detection limit was 0.05 micrograms/ml for every serum sample.

A new model of plastic ankle foot orthosis (FAFO (II)) against spastic foot and genu recurvatum.

A plastic ankle foot orthosis (AFO) was developed, referred to as functional ankle foot orthosis Type 2 (FAFO (II)), which can deal with genu recurvatum and the severe spastic foot in walking. Clinical trials were successful for all varus and drop feet, and for most cases of genu recurvatum. Electromyogram studies showed that the FAFO (II) reduced the spasticity of gastrocnemius and hamstring muscles and activated the quadricep muscles. Gait analysis revealed a reduction of the knee angles in the stance phase on the affected side when using the FAFO (II). Mechanical stress tests showed excellent durability of the orthosis and demonstrated its effectiveness for controlling spasticity in comparison with other types of plastic AFOs.
