Hormone serum levels and hormone receptor contents of endometria in women with normal menstrual cycles and patients bearing endometrial carcinoma.

Serum levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), prolactin (HPRL), 17 beta-estradiol (E2) and progesterone (P) were estimated in 46 subjects with normal menstrual cycles in whom hysterectomies were performed. Estrogen (ER) and progesterone receptor (PgR) levels in endometrial samples of these patients were estimated, and histological dating of the cycle day was carried out. Similarly, hormone serum levels and ER as well as PgR were estimated in 17 patients with endometrial carcinoma. No correlation between LH, FSH, HPRL and ER as well as PgR was noted in the normal subjects. Correlation between P and ER was observed in this group. Parallel variations between E2 and PgR were recorded in the normal females. In the carcinoma group no correlations between hormone serum levels and receptor contents were found, but ER and PgR correlated with each other. Receptor levels was highest in the well-differentiated group of endometrial carcinoma. The present experiments provide a rationale for progestagen therapy of carcinoma of the endometrium.

Endometrium

[Experiences using the suction curettage system of Hamann and Pockrand (author's transl)].

All curettages of a 8-week-period were done using the suction system of Hamann and Pockrandt. This test-collective includes 66 patients, their age was between 20 and 78 years; 6 of them had to be excluded: the surgical intervention was begun without general anaesthesia but continued with general anaesthesia is required by the patient. The test-collective was divided into 2 groups: in group A (30 patients) all suction-curettages were done in general anaesthesia, in the first 20 patients a conventional curettage with dilation of the cervix were performed additionally, in order to proof the effectiveness of the suction curettage. The histological findings of the materials of the conventional curettages had shown no more informations than that of the materials of the suction curettages. -- In group B (30 patients) the suction curettage were done in diazepampentazocin-analgesia; a third of them had specified no sensations, the others had stated a feeling of pressure or spasms. -- In each group the histological examinations were possible without any problems. The advantages of the suction curettage system are the rare necessity of cervix dilatation (7 of 60 cases), the possibility of the operation without general anaesthesia and the possibility of a quick performance of the curettage.

[Endogenous oestrogen state in women with postmenopausal bleeding (author's transl)].

The endogenous oestrogen state in 52 women (33 with endometrial carcinoma and 19 with benign conditions of the endometrium) was assessed ny means of histological examinations of vaginal swabs and of radio-immunological determinations of 17 beta-oestradiol. Considering their age, these women displayed an oestrogen activity far exceeding that found in previous studies. There was no difference between the oestrogen levels in women with carcinoma and the other patients. Furthermore, it was stiking that in the same group of women the examined parameters frequently showed different results. Consequently, a diagnosis based solely on the investigation of one parameter must frequently be misleading.

[A very large luteoma gravidarum].

A case of luteoma gravidarum detected during cesarean section is reported for the first time in German literature. The tumor described is the biggest of its kind so far. By means of this own observation details of origin, clinical behavior, histology and therapy of this odd tumor are discussed. Nevertheless a good deal remains enigmatical and further studies will be as interesting as actual.

[The value of cytological screening for neoplasms in young women].

The question of the incidence of positive or suspect cervical cytology-tests in young women of 30 years or less of age out of a total of 54,604 cervical cytology-tests is discussed. The results of clinical, cytological, and histological follow-up are reported. According to our experiences that 158 smears of group Pap. III and 99 smears of group of Pap IV and V were found in young women of 30 years or less of age this group of patients should not be excluded from consecutive cervical cytology-tests or national health-screening programs.

[Results of surgical radiotherapeutic management of cervix carcinomas during 1966-1968].

From January 1st 1966 until December 31st 1968, 593 women with carcinoma of the cervix uteri were admitted to and treated at the I. Universitäts-Frauenklinik of Vienna. 151 cases underwent a radical operation (Wertheim) resulting in a relative 5-years-healing of 85.43% and 442 women were exclusively treated by a combined radium-X-ray-therapy. From the 442 inoperable cases in 234 cases--which is 53%--a 5-years healing was reached. Of all the 593 cases together, 363 are living without relapse after 5 years, which is equal to 61.21% absolute healing.
