Hypothalamic-pituitary-ovarian axis in uremic women.

Menstrual abnormalities are common with decreasing renal function and uremia. Maintenance hemodialysis therapy partially corrects some of the abnormalities. Since the pathophysiology of the menstural irregularities is unknown, the secretion of LH, FSH, estrogen, and PRL was studied under basal conditions and after provocative testing in 13 women undergoing chronic maintenance hemodialysis. Elevated LH concentrations were found in all six premenopausal (44.1 +/- 26.4 mlU/ml, normal follicular phase 4 to 30) and seven postmenopausal (246.1 +/- 171.4 mlU/ml, normal 35 to 200) women studied. FSH values were normal both in the premenopausal women (7.9 +/- 4.1 mlU/ml, normal 2 to 20) and in postmenopausal women (88.0 +/- 69.0 mlU/ml, normal 40 to 200). FSH and LH levels did not show typical ovulatory spikes in three of four women studied. Pulsatile hormonal release was absent. The pituitary response to LH-RH was adequate and prolonged, but the response to clomiphene citrate was variable. The study documents the existence of abnormalities at various levels of the hypothalamic-putuitary-ovarian axis.

Deficiency of T and B lymphocytes in uremic subjects and partial improvement with maintenance hemodialysis.

Lymphopenia of a group of uremic patients was associated with normal percentages of T cells but reduced percentages of B cells. Lymphocyte counts improved after a period of maintenance hemodialysis, although not to control levels, and B cell percentages returned towards normal. Uremia is therefore associated with depression of total T and B cell numbers, with a relatively more pronounced effect on B cells. A period of maintenance hemodialysis produces increase in numbers of both cell types and depression becomes nonselective.

Lymphocyte subpopulations in maintenance hemodialysis patients.

Lymphocyte counts and T,B and null cell proportions were analyzed in forty non-splenectomized, non-nephrectomized maintenance hemodialysis patients, and compared with those in controls. The patients were lymphopenic, but had normal T, B and null cell percentages, regardless of tehir primary renal disease. It is postulated that their lymphocyte suppression is non-selective and may be related to the state of controlled azotemia or to the dialysis procedure.

Long-term effect of dialysate glucose on the lipid levels of maintenance hemodialysis patients.

The long term use of a glucose free dialysate in a group of 15 maintenance hemodialysis patients is associated with a significant decrease in serum cholesterol in 50% of the patients and a significant decrease in serum triglycerides in 25%. The lipid changes appear to be independent of changes in body weight. Hyperlipidemia is believed to be an associated risk factor in the genesis of atherosclerosis in the general population and perhaps in maintenance hemodialysis patients. The long term correction or palliation of the hyperlipidemia of some maintenance hemodialysis patients by the use of glucose free dialysate may be beneficial.

An extracorporeal complexing hemodialysis system for the treatment of methylmercury poisoning. I. In vitro studies of the effects of four complexing agents on the distribution and dialyzability of methylmercury in human blood.

Almost all of the methylmercury (MM) in human blood is protein bound, primarily to sulfhydryl ligands. Sulfhydryl agents such as penicillamine, N-acetylpenicillamine, cysteine and N-acetyleysteine are capable of reversing the protein binding of MM when they are added to whole human blood. The magnitude of the protein binding reversal was similar for each compound as predicted by the determination of their relative affinities for MM in vitro. Concentration-dependent reversals of protein binding of MM in blood was observed at increasing sulfhydryl concentrations from 10-4 to 10-2 M. At 10-2 M, a 55- to 60-fold increase in non-protein-bound plasma MM was observed, when compared to blood with no added sulfhydryl agent. Both the complexing agent and the MM complex formed in blood were readily dialyzable using a Travenol 145 twin coil hemodialyzer. At cysteine concentrations of 10-2 M in whole blood, up to 44 percent of whole blood MM was dialyzed on a single pass at a dialyzer blood flow rate of 55 ml/min. Under the same conditions, up 50 94 percent of plasma cysteine was dialyzed. A system is presented for use in vivo on experimental animals. The potential advantages of this method over existing therapeutic regimens for MM poisoning are discussed.
