The extensor carpi radialis longus muscle flap for anterior elbow coverage.

A muscle group resection for treatment of malignant fibrous histiocytoma of the distal arm exposed a 15 cm length of radial nerve. The soft tissue defect and radial nerve were covered with an extensor carpi radialis longus muscle island flap. The proximal dominant vascular pedicle enabled extensive mobilization and transposition of this muscle to cover the defect while preserving normal elbow, radial nerve, and hand function. Because of its ease of elevation, extensive arc of rotation, and its expendability, the extensor carpi radialis longus muscle flap should be considered as an option for reconstruction of defects about the elbow joint.

Acute free perforation as a presenting sign of regional enteritis. Case report and collective review of the literature.

Regional enteritis rarely presents as free peritoneal perforation. A case of such a manifestation is described and the literature is reviewed. Resection of the diseased segment is mandatory, for it is associated with the least postoperative morbidity and mortality. Satisfactory results are achieved with cutaneous double-barrel ileotransverse colostomy and subsequent reanastomosis or closure but primary anastomosis can be accomplished safely with construction of a "T-vent" (cutaneous transverse colostomy with ileotransverse colostomy). Perforation of an area of regional enteritis, although uncommon, should be considered in the differential diagnosis of the acute abdomen with peritonitis

A new irrigating solution for intraocular surgery: TC Earle Solution.

Electrolyte solutions--Plasma-lyte, TC Medium 199, TC Medium 199 plus additional bicarbonate, TC Earle Solution, and glutathione bicarbonate Ringer Solution--were evaluated for effectiveness in maintaining corneal thickness and endothelial viability. The TC Earle Solution was shown to be the most effective commercially available solution; little difference was demonstrated between TC Earle Solution and the experimental GBR Solution. Glucose and bicarbonate were found to be essential ingredients in the efficacy of the TC Earle Solution.

In vitro evaluation of intraocular infusion fluids: effects on the lens and cornea.

Glutatione-bicarbonate-Ringer solution (GBR), GBR deficient in adenosine and glutathione, and a newly proposed glucose-phosphate-Ringer solution (GPR) have been found to maintain lens function and corneal endothelial viability with approximately equal efficacy during prolonged periods of incubation. Currently used intraocular irrigating solutions do not perform as well in maintaining the function of borderline decompensated lenses and in sustaining endothelial viability.

Isolated hypogastric artery aneurysms.

Although isolated hypogastric artery aneurysms are among the rarest of all aortoiliac aneurysms, they are among the most devastating. Two cases, one with rupture directly into the urinary bladder, are presented and the typical clinical triad of compressive symptoms, pelvic mass, and previous aneurysm is described. Intraoperative management is predicated upon sac obliteration with minimal tissue dissection. An aggressive approach to asymptomatic or minimally symptomatic aneurysms is urged due to the very high mortality following rupture.
