Prenatal sonographic appearances of Klippel-Trenaunay-Weber syndrome.

A case of prenatal Klippel Trenaunay-Weber syndrome is presented, with a description of the sonographically observed disease progression in utero. The appearance of a complex thoracic multicystic mass in association with progressive unilateral lower limb enlargement was strongly suspicious of this disorder. Colour-flow Doppler studies of the cutaneous haemangiomata in utero were not of diagnostic assistance in this case. The prenatal detection of the large cutaneous haemangiomata was of critical importance in minimizing delivery trauma for the mother and fetus.

Doppler flow velocity waveform analysis in postdate pregnancies.

This study examines the efficiency of Doppler flow velocity waveform analysis in the evaluation of the postdate pregnancy. Seventy women whose pregnancies were at or beyond 41 weeks' gestation were assessed by calculating the systolic/diastolic ratios from umbilical and uteroplacental arteries. Waveform ratios from pregnancies associated with fetal compromise and abnormal neonatal outcome were similar to those from pregnancies in which the outcome was normal. These findings indicate that the pathophysiology of placental insufficiency in postdate pregnancies differs from that observed in cases of fetal growth retardation at earlier gestational ages. Doppler flow velocity waveform analysis is unlikely to be of benefit in the routine assessment of the postdate pregnancy.

Oxytocin induced second trimester uterine rupture.

A case of oxytocin induced rupture of an unscarred uterus during the second trimester is presented. While rupture of the uterus by oxytocic agents in the second trimester has previously been documented, this case is unusual in that fetal parts were palpable through the cervical os, delaying diagnosis and definitive management. Digital palpation of fetal parts through the cervix does not preclude the diagnosis of fetal extrusion secondary to uterine rupture.

Fetal and maternal Doppler flow velocity waveforms in normal sheep pregnancy.

Abnormalities in placental vascular resistance have been shown to have an important association with intrauterine growth retardation in both human and sheep pregnancies. In this study we have assessed impedance to blood flow through the ovine placental circulation by Doppler flow velocity waveform analysis. As gestation advanced from 66 to 136 days, a significant decrease in impedance to flow was observed in both umbilical and maternal placental arteries. The greatest decrease in impedance was observed between 66 and 109 days' gestation in umbilical arteries and between 66 and 80 days' gestation in maternal placental arteries. In these normal pregnancies, no significant correlations could be found between Doppler flow velocity waveform analyses and fetal or placental growth. We conclude that Doppler flow velocity waveform analysis is a valuable and non-invasive method to assess impedance to blood flow through the placental circulation in pregnant sheep.
