[Multilayer deep hernioplasty by E. Shouldice method in inguinal hernias].

The article gives a detailed description of steps of performing multilayer deep hernioplasty after E. Shouldice. In order to substantiate the possibilities to perform the procedure the structure of the transverse fascia in the ilioinguinal area was investigated in 20 corpses without hernias as compared with those having hernias. The data obtained give qualitative characteristics of the content of the collagen and elastic fibers of the transverse fascia. Their amount was proved to be 1.5-2 times less as compared with normal. Under analysis were results of surgical treatment of 786 patients with inguinal hernias. Long-term results of the surgical treatment were studied in 281 patients operated by the method of the multilayer deep hernioplasty after E. Shouldice. The method of surgical treatment of inguinal hernias was proved to be pathogenetic and highly reliable.

Histoprotective effect of antihypoxant olifen during experimental acute pancreatitis.

We evaluated the efficiency of perfusion with olifen in preventing oxidative stress at the early stage of acute pancreatitis. Transaortic perfusion with olifen prevented clinical and biochemical symptoms of acute pancreatitis, attenuated oxidative stress, reduced peritoneal exudation, and restricts the area of pancreatic necrosis to 6% tissue.

[Pharmacological analysis of morphofunctional changes in the adrenergic structures of the pancreas as affected by lesions of the endocrine and exocrine pancreas].

The adrenergic section of the vegetative pancreatic innervation has been examined, using morphological, histochemical and biochemical methods, during neurogenic injury, acute experimental pancreatitis and alloxan diabetes. It has been established that the development of those etiologically different processes was accompanied by catecholamine level reduction in the adrenergic structures of the pancreas which can be prevented by ganglioblockers. It is concluded that sympathetic nervous system participates in etiopathogenesis of a number of pancreatic diseases and that it is possible to prevent and treat them with sympathotropic agents.

[Pathogenesis and clinical significance of fat necrosis in acute pancreatitis].

An analysis of results of 200 experiments, 100 autopsies after death from acute destructive pancreatitis and clinical observation of 2000 patients with necrotic forms of acute pancreatitis has given data on pathogenesis of fat necrosis, dynamics of its development, its interrelationships with alterations of the pancreas parenchyma. A concept of unity of proteolytic and steatonecrotic components of acute destructive pancreatitis is substantiated. The authors propose a classification of periods of development of fat necrosis, determine the clinical significance of alterations of the fat tissue at early terms of the disease, give recommendations for the surgical and conservative treatment.

[Prevention and treatment of pulmonary complications in patients with combined injuries of the chest accompanied by shock in the early period of trauma].

Complications were noted in 83% of patients with an associated trauma of the chest. Bilateral pneumonia was the most frequent complication (57.7%), then were traumatic pulmonitis (15.9%) and total pulmonary edema (18%). The adequate anesthesia, liquidation of a disturbance of the chest carcass, timely elimination of hemo- and pneumothorax are thought to be the most effective measures of struggle against pulmonary complications in patients in the early period of the trauma disease. The complex treatment has given 1.5 times less amount of pulmonary complications.
