Distension of the operation site after posterior fossa surgery.

In 130 posterior fossa operations 6 cases of progressive distension of the operation site have been recorded and documented. The possible causes of this complication and the surgical management are discussed. Isotope studies after injection of Ytterbium 169DTPA into the underlying cyst have shown abnormal patterns in CSF circulation and absorption. All the cases have resolved after ventriculo-peritoneal shunts.

Hydrocephalus after posterior fossa operations.

Sixteen patients with hydrocephalus which developed after operations on the posterior fossa were studied by radioisotope ventriculography. The results indicate that postoperative hydrocephalus is of communicating type, and that block of the basal cisterns is always present. Mechanisms involved in its pathogenesis are discussed, and it is felt that pre-existent adhesions in basal cisterns, surgical bleeding and cerebrospinal fluid (CSF) extra-arachnoidal collections are possible causes of the hydrocephalus.

Quantitative radioisotope ventriculography: a comparative study.

This report deals with quantitative techniques of isotopic ventriculography. In a series of 25 hydrocephalic patients, several parameters have been studied. An effort has been made to determine which of them is more significant when compared with the clinical course of the patients. Confidence intervals for population mean of this parameter were calculated. The results can be of clinical use in deciding if the clinical course of a hydrocephalic patient is arrested or progressing, and in assessing the advisability of surgical intervention.

[Morphology of Sarcoptes Scabiei (variety hominis) under scanning electron microscopy].

Some morphologic aspects of S. S. by SEM in twelve parasites (female) extracted from the lesions with a pin are studied. We think that the extraction method and the posterior colocation of the parasite has contributed to its better understanding. We describe the dorsal cleft, a characteristic formation of the sarcoptidos, tapered on both sides with thorns in the form of shark's teeth. We analyze the details of the Ambulacro, constituted by the cut or the strait Pedicelo, with a distal extremity in the form of Condyl in which the suction cups are articulated. We can see, in the anogenital portion, two independent anal and genital orificius and, in the ventral area, the tocostoma, or cleft, where the eggs are deposited. Finally we analyze the bucal organs, the Pedipulpos, the morphology and disposition of the hipostoma and the queliceros.

Dermic peripheral microangiopathy in plasmo-proliferative disorders.

Skin biopsy specimens from 12 patients with plasmo-proliferative disease (myeloma and macrogobulinemia) were histopathologically and histochemically examined. In all the cases studied (by a biopsy obtained from the hand) there were alterations in the dermal microvasculature characterized by thickening of the PAS-stained basal membrane, endothelial swelling and proliferation with partial obliteration of the vascular lumen. The lesions were morphologically similar to those found in diabetic and paralymphomatous microangiopathy but in the present investigations greater endothelial proliferation has been observed. We did not find any lesions in the healthy control group. Different immunological, metabolic and hyperviscosity mechanisms are discussed. It is interesting to report that the results for blood coagulation and carbohydrate metabolism were normal; hyperviscosity was found in the serum of the patients examined, and in 3 cases cryoglobulinemia was also found. It will be interesting to carry out further investigations on the different pathogenic and biological aspects of the skin microvascular lesions in plasmo-proliferative diseases in order to increase our knowledge of the microcirculation in immunologic malignant disorders.

Secretion and glycosylation of Clostridium thermocellum endoglucanase A encoded by the celA gene in Saccharomyces cerevisiae.

The Clostridium thermocellum celA gene encoding endoglucanase A is expressed in Saccharomyces cerevisiae but the enzyme produced from the native celA gene is not secreted. After removal of the bacterial signal peptide-coding sequence, the gene was fused to the promoter and prepro segment of the S. cerevisiae MF alpha 1 gene. This construction directs secretion of active endoglucanase A into the culture medium when introduced in yeast on either replicating or integrating vectors. Secretion of endoglucanase A required growth of transformants on rich medium. The secreted enzyme is a 97,000 Da glycoprotein containing about half of its molecular weight as carbohydrate. This new gene fusion could facilitate further research on protein secretion in yeast by using a cellulase as a marker enzyme.

Malnutrition and hyperglycemia associated with TPN do not alter polymorphonuclear leukocyte adherence to nylon.

Polymorphonuclear leukocyte (PMN) adherence to nylon was assayed in blood obtained from 10 malnourished patients before and during therapy with total parenteral nutrition (TPN). All patients demonstrated minimal to moderate loss of visceral and somatic protein. Adherence of PMN was unaltered by malnutrition or during the 4 wk of TPN in spite of hyperglycemia, which required insulin therapy in 4 patients. There was no correlation of blood sugar levels and PMN adherence.

Development of monofixation syndrome in congenital esotropia.

Fifty patients with 8 prism diopters or less of esotropia after the last surgery were studied. Forty-nine percent of those straightened before 12 months of age and 41% of those straightened between 13-24 months developed the M.S. Only one of 22 patients straightened after the age of two developed the M.S. Although D.V.D and overaction of the inferior oblique muscles were associated findings, neither precluded the development of the M.S. Those patients without M.S. had a higher incidence of subsequent exotropia and manifest D.V.D.
