Effect of unilateral condylectomy on the sheep temporomandibular joint.

Unilateral condylectomy was performed on five young adult sheep. The animals were killed at three months and both joints and the excised condyles were examined macroscopically and histologically. All five showed pronounced regeneration of the condylar head on the operated side. The articular surface was fibrous and fused to the disc. Four of the five opposite joints showed medial remodelling. Young sheep have a higher regenerative capability than human adults of equivalent age, and similar reactions to those of children.

Nocturnal hypoxaemia in late pregnancy.

We have measured arterial oxygen saturation (SpO2) continuously overnight in 13 non-pregnant (NP), 13 pregnant normotensive (NPIH) and 15 pregnant patients with a diagnosis of pregnancy-induced hypertension (PIH). The two pregnant groups did not differ in duration of pregnancy (> 35 weeks) and none was in labour. There was no significant difference in age between these three groups. Mean SpO2 in group NP was 98.5% (range 97-99%). This was significantly higher than that in group NPIH (95.2 (91-98) %) and group PIH (94.9 (89-99) %). In seven pregnant patients, more than 20% of the recording was spent with an SpO2 < 90%. We conclude that a significant number of pregnant women (> 35 weeks' gestation) suffer from prolonged nocturnal hypoxaemia.

Management of chronic orofacial pain.

The diagnosis and management of chronic orofacial pain can be difficult due to the multifactorial nature of the problem involving organic, psychological and social and cultural variables. The establishment of multidisciplinary pain clinics can assist the medical practitioner in both diagnosis and treatment. A review of 185 patients complaining of chronic orofacial pain is presented to show the main conditions diagnosed at a pain clinic.

Impacts associated with special care units in long-term care facilities.

This study is a comprehensive, longitudinal assessment of the characteristics of special care patients. Demented patients in special care units (SCUs) within four nursing homes were compared with their demented counterparts in the same facilities who were not placed in SCUs. Results of this preliminary study suggest that the two groups differ in level of cognitive impairment, in behavior, and in functional and physical status. No deleterious or beneficial effects were associated with SCU residence during a 6-month period.

Immunocytochemical and biochemical characterization of the connective tissue component of fibrous papular lesions of oral mucosa.

In an attempt to define the fibroblastic components of fibrous papular lesions of oral mucosa, 21 cases representing these lesions were selected for study. Paraffin or frozen sections of the lesions were stained, using the immunoperoxidase technique, with antibodies to: alpha-l-antitrypsin, alpha-l-antichymotrypsin, lysozyme, fibronectin, actin, myosin, and for Clq binding. Three of these cases were biochemically analyzed for collagen Types I, III, and V. This study demonstrated the presence of cytochemical markers in fibrous papules that were similar to those observed for the compartment of circumvascular fibroblasts in control normal mucosa. Analysis of the collagens present indicated that in addition to histologic similarities, the gene products of these extracellular matrices were similar to those reported for angiofibromas and Shagreen patches in tuberous sclerosis. The cells in these lesions appear to be distinct from myofibroblasts and the high affinity complement binding fibroblasts characterized by Bordin et al.(1).

Attitudes of health workers toward old people.

The Tuckman-Lorge Questionnaire was used to study the attitudes of three groups of health workers toward old people and to test their acceptance of geriatric stereotypes. The health workers tested were medical students, housestaff members, and members of a mobile psychogeriatric screening team. Many significant differences were found between and within the groups tested, as well as between male and female subjects. The female housestaff had extremely high and significantly different scores from all other groups. The geriatric staff adhered least to the stereotypes. The results are discussed in the framework that the attitudes of care givers are directly related to the quality of the care provided. It is hypothesized that female housestaff members have special difficulties with role conflicts that cause them to adhere to stereotypes of the aged. The milieu of geriatric treatment, rather than knowledge of statistics about old people, is the most effective background for positive changes in attitudes toward the elderly.

The therapeutic milieu and the older depressed patient.

A team screening referrals for admission to a state hospital evaluated 434 consecutive cases. Over 51% had a diagnosis of depression. By treating patients in the community, including brief local hospitalization where indicated, total geriatric admission were reduced by 50% per annum. In the final phase of the program, only 10% of admissions to the state hospital were for a primary diagnosis of depression. Almost half of the evaluations required only examination and recommendations, treatment being undertaken by the referring source or another community agency. A further 22% required but one visit to initiate therapy. 17.6% were admitted to treatment, which included drug and group therapy and also milieu therapy by means of transfer, environmental manipulation, and attitude therapy. Only 11.3% needed to be seen weekly or oftener. The program results in considerable savings by reducing unnecessary hospitalization.
