Colorimetric measurement of glycosylated protein in whole blood, red blood cells, plasma and dried blood.

A colorimetric method for the measurement of whole-blood glycosylation (WBG), glycosylated haemoglobin in red blood cells (GHb), glycosylated plasma protein (GPP) and dried-blood glycosylation (DBG) is described which is rapid, inexpensive and precise. GHb correlated well with HbA1 measured by cation-exchange chromatography and was also correlated with WBG, DBG and GPP. DBG, which showed good correlation with GHb, could be measured on several drops of blood dried on filter paper treated with glucose oxidase. Filter papers are posted to the laboratory prior to clinic visits, and by having the DBG result available more rational diabetic management is possible. For DBG, the intra- and inter-assay coefficient of variation at three levels is less than 6%. Reference values in non-diabetic children have been calculated and are compared with values obtained in 'good' clinical diabetic control and in 'fair' control. The filter-paper assay DBG method has a high degree of patient acceptance.

The obesity hypoventilation syndrome and the Prader-Willi syndrome.

Fourteen children with the Prader-Willi syndrome have been managed at the Royal Alexandra Hospital for Children between the years 1964-1980--twelve male, two female. Six male children developed features of the obesity hypoventilation syndrome. The age of onset of this complication ranged from 4.0 to 12.6 years. With one exception those children with the obesity hypoventilation syndrome were more obese than those without it. At the time of onset of the syndrome, five of six patients had weights greater than or equal to 6.5 standard deviations above ideal body weight. Those children without the obesity hypoventilation syndrome had a range of standard deviations 1.0 to 4.2 above the ideal body weight. In four of six cases weight reduction and a cardiac failure regimen resulted in reversal of the obesity hypoventilation syndrome. With two of the six children there had been cardiomegaly and increased pulmonary venous vascularity on x-ray at a chronological age of three months. Two of the six children died.

The diagnostic problems posed by unusually great height at various ages are discussed, together with the principles governing treatment to curtail height by exhibition of oestrogens or androgens. Short stature is then discussed in terms of an initial approach to assessment and diagnosis, followed by a systematic one. The main agents used in treatment are briefly mentioned, together with some details concerning the use of human growth hormone (hGH).

General management of the diabetic child.

An acute onset with polyuria and polydipsia is the pattern of onset for something approaching 90 per cent of child diabetics. Their management, then, must include insulin and diet together with education of the parents in how to use these measures.
