A combination of cis-platinum and radiotherapy was applied in five patients with advanced bladder carcinomas. The patients were examined by nuclear magnetic resonance imaging prior to and following therapy. Corresponding to cell-kinetic considerations regarding tissues with slow cell turnover, a maximum therapy effect is shown about nine months after the end of local therapy. Smaller control intervals are recommended in case of bladder carcinomas with lymph node metastases. Due to the possibility of multiple slice imaging, the nuclear magnetic resonance technique is particularly suited for the monitoring of bladder carcinomas. Sagittal and coronary slices show a better image of tumoral extension at the roof and floor of the bladder than axial computed tomography. Lymph node metastases are represented relatively late by both methods.
Plasma levels of the circulating metabolite of prostaglandin (PG) E2, 15-keto-13,14-dihydro-PGE2 (KH2PGE2), were determined radioimmunologically after conversion to the stable degradation product 11-deoxy-15-keto-13,14-dihydro-11,16-cyclo-PGE2 (DKH2-cyclo-PGE2). In healthy volunteers a plasma level of 25 +/- 2 pg/ml (mean +/- S.E.M., n = 24) was found. The plasma level of KH2PGE2 was significantly decreased after administration of acetylsalicylic acid (4 x 1 g/24 hours). A significant elevation of the plasma levels of the circulating metabolite of PGE2 was observed in patients with bronchogenic carcinoma as compared to healthy controls, while no elevation was found in patients with chronic myeloid leukemia, lymphatic leukemia and non-Hodgkin lymphoma. The increased plasma levels of KH2PGE2 in the patients with bronchogenic carcinoma were independent of the clinical condition, histological type of tumor, tumor spread and therapeutic regimen. The results indicate that the elevated plasma level of the circulating PGE2 metabolite in patients with bronchogenic carcinoma is not an expression of malignant disease in general. On the other hand, the results do not suggest that the increase in the plasma level of KH2 PGE2 is a biochemical tumor marker closely related to a particular clinical feature of patients with bronchogenic carcinoma.

Thirty adult patients suffering from acute nonlymphocytic leukemia (ANLL) were treated according to a modified COAP regimen. Vincristine, cyclophosphamide, and prednisone were given by push injection, while cytosine arabinoside was infused over periods of 8 h. Nineteen patients (63%) achieved complete remission. Remission maintenance therapy consisted of 6-mercaptopurine daily and methotrexate twice weekly. Later in the study, COAP consolidation and reinduction was added, which improved the median duration of complete remission from 7 to 24 months. Comparison of the results with the literature shows that the modified COAP regimen is one of the most effective treatment schedules for adult ANLL.
A randomized study in 20 patients with cancer was carried out to test the clinical efficacy of sodium-2-mercaptoethane sulfonate (ASTA D-7093; mesnum) as an agent to prevent urotoxic side effects (in particular, hemorrhagic cystitis) during cytostatic therapy with the oxazaphosphorines cyclophosphamide and ifosfamide. Eleven patients received mesnum iv and nine patients received a standard prophylaxis. The frequency of microhematuria was significantly lower in the patients receiving mesnum. A slight microhematuria was observed in one patient. With the standard prophylaxis, all nine patients receiving single-agent therapy with ifosfamide or cyclophosphamide had hematuria and three of these had macrohematuria. According to the available results, a daily mesnum dose of 60% (wt/wt) of the ifosfamide or cyclophosphamide dose is recommended. This dose should be divided into three equal fractions. The first administration should be given concurrently with the cytostatic agent and the subsequent two administrations at 4 and 8 hours after administration of the cytostatic agent. Significantly higher doses of mesnum (eg, 133% of the cyclophosphamide or ifosfamide doses) lead to gastrointestinal disorders, which are easily reversible.
Pronounced, short-length narrowing of the trachea combined with dyspnoea at rest is an absolute indication for partial resection. For stenoses of moderate degree the indications for surgical intervention are as yet not clearly defined. They depend not only on the clinical, roentgenological and tracheobronchoscopic findings, but also on the results of lung function tests. Changes in respiratory function become noticeable at reduction of under 30% of the tracheal diameter. Experiments with artificially induced stenosis have shown an exponential rise in flow resistance when the diameter of the trachea was narrowed to below 8 millimetre. observations made in patients have proved that the experimental results also apply clinically. The indications for surgery of a tracheal stenosis should be made dependent on the results of the determinations of flow resistance, whole body plethysmography and expiratory and inspiratory volume/1 sec. Flow resistance exceeding 5 cm H2O/1 sec, equivalent to narrowing to under 8 mm in diameter, is considered to be an indication for partial resection.
