Pharmacy services in a 240-bed Gordon Friesen-design hospital.

This article describes a typical Gordon Friesen-design institution in which the facility design and systems design complement one another to make an efficiently run hospital. The basic concepts of the hospital are to keep the nurse with the patient and to maintain a steady supply flow by centralizing responsibility for materials. Problems that occur in pharmacies of facilities designed by Friesen usually are related to logistics. The design of the institution make delivery of medication and communication with staff nurses more difficult. Pharmacy programs such as unit dose and intravenous admixtures, excellent examples of products in "patient-ready units," complement the other supply delivery systems that are typical of a Friesen-design institution. Good written and verbal communications between nursing staff and pharmacy can bring about a smoothly flowing pharmacy service that blends with the overall concept.

Determination of the operational molarity of solutions of bovine alpha-chymotrypsin, trypsin, thrombin and factor Xa by spectrofluorimetric titration.

Several esters of 4-methylumbelliferone and 2-naphthol were synthesized and examined as possible spectrofluorimetric titrants for bovine alpha-chymotrypsin, trypsin, thrombin, Factor Xa and for subtilisin Novo. 4-Methylumbelliferyl p-guanidinobenzoate hydrochloride (MUGB) is a satisfactory titrant for alpha- and beta-trypsin, thrombin and Factor Xa and 4-methylumbelliferyl p-(NNN-trimethylammonium)cinnamate (MUTMAC) is a good titrant for alpha-chymotrypsin. The amount of enzyme used for spectrofluorimetric titration is 0.02-3.00nmol and the amount of 4-methylumbelliferone liberated is independent of the concentration of titrant and stoicheiometrically equal to the amount of enzyme used. Results obtained with MUGB and MUTMAC have been checked by spectrophotometric titration with p'-nitrophenyl p-guanidinobenzoate hydrochloride and p-nitrophenyl N(2)-acetyl-N(1)-benzylcarbazate respectively. p-Nitrophenyl N(2)-acetyl-N(1)-(9-anthrylmethyl)carbazate has been synthesized; it did not react with alpha-chymotrypsin. A satisfactory spectrofluorimetric titrant for subtilisin Novo was not discovered.

Bulbo-cortical pathways and their possible relevance to migraine and epilepsy.

Facilitation of the visual evoked response from the reticular formation in the cat has been shown to depend on nicotinic cholinergic receptors distinct from the muscarinic cholinergic receptors responsible for the arousal reaction. An increase in the amplitude of visual evoked responses in migraineurs does not therefore imply any change in reticulocortical activity of relevance to epilepsy. Stimulation of locus ceruleus and nucleus raphe dorsalis exerted comparatively minor effects on the visual evoked response and did not alter the discharge rate of cortical neurons in the resting state. It is concluded that the changes in cerebral blood flow previously reported to result from stimulation of these monoaminergic brainstem nuclei, which resemble those observed in migraine, must be exerted directly on the cortical microcirculation and not simply follow the metabolic demand of cortical neurons. There is thus a place in the management of migraine for pharmaceutical agents acting on cerebral vessels even if the neural hypothesis for the mechanism of migraine proves to be correct.
