Total parenteral nutrition cholestasis: a cause of mechanical biliary obstruction.

In a newborn baby with Hirshsprung's disease obstructive jaundice developed following prolonged parenteral nutrition. At laparotomy, thick inspissated bile was flushed from the biliary tree and prompt resolution of the jaundice followed. To our knowledge, this is the first reported case in which inspissated bile appeared to be a complication of total parenteral nutrition. Mechanical obstruction must be recognized as an extreme in the spectrum of total parenteral nutrition cholestasis.

Initial antibiotic treatment of purulent meningitis in infants 1 to 2 months of age.

The bacterial cause of purulent meningitis in patients younger than 2 months of age was reviewed. Hemophilus influenzae was the most common organism isolated from infants 1 to 2 months of age. Gram-negative enteric organisms were not shown to cause meningitis in this age group in term infants without neonatal complications. Initial antibiotic therapy for infants in this age group should include ampicillin sodium and chloramphenicol sodium succinate.

Eosinophilic meningitis and hydrocephalus in an infant.

Central nervous system invasion by helminths is the most frequent cause of eosinophilic pleocytosis in the CSF. Although CSF eosinophilia is an unusual finding in the continental United States, it is not an uncommon observation in cases of meningitis in Hawaii and the South Pacific. The rat lungworm, Angiostrongylus cantonensis has been implicated as the causal agent responsible for cases of eosinophilic meningitis in these areas. The diagnosis of eosinophilic meningitis secondary to A cantonensis is generally an indirect one, based on the characteristic clinical findings, documented eosinophilic pleocytosis of the CSF, and history of consumption of food likely to contain infected larvae. Hydrocephalus developed in a 9-month-old infant from Samoa with absolute eosinophilia and an eosinophilic pleocytosis of the CSF.
