Histiocytoid hemangioma: a disease spectrum. Report of a case with simultaneous cutaneous and bone involvement limited to one extremity.

The collective term histiocytoid hemangioma encompasses a spectrum of diseases that share a histologic features characterized by distinctive histiocytoid endothelial cells. Several incompletely defined cutaneous and extracutaneous vascular tumors, including atypical pyogenic granuloma, pseudopyogenic granuloma, papular angioplasia, angiolymphoid hyperplasia with eosinophilia, Kimura's disease, and inflammatory arteriovenous hemangioma, have been included in this group. We describe a 31-year-old Mexican-American man with multiple histiocytoid hemangiomas involving skin, subcutaneous tissue, and bone limited to the left arm and hand. Examination of skin and bone lesion biopsy specimens showed a proliferation of histiocytoid endothelial cells with intracytoplasmic vacuoles and associated vascular lumen formation. Ultrastructural and immunoperoxidase studies supported endothelial cell differentiation. Radiation therapy resulted in significant clinical improvement. Current concepts regarding the clinical and pathologic features of this disease spectrum also are discussed.

Merkel cell carcinoma--a problem with microscopically controlled surgery.

Trabecular cell carcinoma, originally described by Toker, is a malignant skin tumor, probably of Merkel cell origin. These tumors are locally aggressive and may metastasize. Prompt surgical removal is mandatory. The clinical features are often not diagnostic, and diagnosis is primarily by histologic examination. Special studies, especially electron microscopy, may be required to differentiate this tumor from others. During surgery, particularly with Mohs microscopically controlled excision, provision must be made for proper handling of the specimen so that these special studies can be carried out.

Adenocarcinoma

Erythrokeratodermia variabilis treated with isotretinoin. A clinical, histologic, and ultrastructural study.

A 30-year-old woman with erythrokeratodermia variabilis was treated with oral isotretinoin for four months. Clinical and light and electron microscopic observations were made before and after treatment. A characteristic electron microscopic feature was subnormal numbers of keratinosomes within the stratum granulosum of hyperkeratotic plaques. Isotretinoin therapy resulted in almost complete clinical clearing of these plaques and restoration of normal numbers of epidermal keratinosomes. In addition, distinctive dyskeratotic cells containing clumped tonofilaments were observed within the stratum granulosum by electron microscopy. These cells persisted after retinoid treatment.

Persistent cutaneous lymphoid hyperplasia of the face with conjunctival involvement.

Lymphocytic infiltration of the orbital conjunctiva has not, to our knowledge, been previously reported as a phenomenon of secondary involvement following long-standing cutaneous lymphocytic infiltration of the face. We encountered a woman with facial cutaneous lymphocytic infiltration pursuing a benign but persistent course, with eventual involvement of the conjunctiva after 15 years, which responded to low-dose radiation therapy.

HLA and beta 2-microglobulin expression in basal and squamous cell carcinomas of the skin.

Histologic sections of seven squamous cell carcinomas (SCC), 13 basal cell carcinomas (BCC), and a Bowenoid actinic keratoses were examined for expression of HLA class 1 antigens (HLA-ABC) using a monoclonal antibody and an immunoperoxidase technique. Expression of beta 2-microglobulin was examined with a polyclonal antibody method. Neither cell marker was detected within the Bowenoid actinic keratoses. Squamous cell carcinomas and basal cell carcinomas exhibited decreased expression of both HLA-ABC and beta 2-microglobulin and often did not express these antigens at all. HLA-ABC was present in only two of 13 basal cell carcinomas and four of seven squamous cell carcinomas. beta 2-microglobulin was present in one of 13 basal cell carcinomas and two of seven squamous cell carcinomas. When present, these antigens often were present in a few areas of the tumor, but absent in others. In both SCC and BCC, both antigens were usually lost simultaneously. In all tumors with beta 2-microglobulin, HLA-ABC also was retained. There was no apparent relationship of anatomic site or type of tumor with retention of surface antigens. Since some tumors or portions of tumors retained HLA-ABC and beta 2-microglobulin on their surfaces, the absence of these antigens is not an absolute marker for malignancy.

Basal Cell Carcinoma

A comparison of Cleocin T 1 percent solution and Cleocin T 1 percent lotion in the treatment of acne vulgaris.

A randomized, investigator-blind study was conducted to compare the efficacy and skin tolerance of Cleocin T Topical 1 percent Solution and Cleocin T Topical 1 percent Lotion. Both treatments reduced acne lesion counts. More than 70 percent of the evaluable patients receiving each treatment reported that their acne improved by the end of the twelve-week study. Skin dryness was reported significantly more often by patients applying the solution than by those applying the lotion. This newly developed lotion formulation of topical clindamycin phosphate is equal in efficacy to, and appears to be less irritating than, Cleocin T Topical Solution.

Leukocyte function in granuloma annulare.

Leukocytes from patients with granuloma annulare were studied in vitro in order to detect functional abnormalities. Random migration and chemotaxis of both polymorphonuclear leukocytes and monocytes were similar in patients and controls. Preincubation of normal cells with patients' plasma did not enhance or inhibit locomotion. Phagocytosis and oxidative burst as reflected by chemiluminescence were also normal. Peripheral blood lymphocytes incorporated uridine normally with and without stimulation by the non-specific mitogen phytohaemagglutinin. These studies suggest that peripheral blood leukocyte function in patients with granuloma annulare is normal, but do not exclude a functional abnormality in tissue.

Human cutaneous topography a new photographic technique: observations on normal skin.

We describe herein a convenient and inexpensive technique for photographic examination of the surface of the skin. With this method, the fine lines of the skin appear as furrows or folds. The furrows demarcate units of epidermal structure which we have designated polyangular units. These, in turn, are arranged in a mosaic pattern. This technique was used to define the topography of normal young adults. Surface features fall into four major patterns: 1) dermatoglyphic, 2) hair bearing, 3) frictional, and 4) flexural.
