Dimensional accuracy of castings produced with ringless and metal ring investment systems.

STATEMENT OF PROBLEM: The ringless casting procedure is in use in clinical dentistry, although there is no adequate scientific data to support its use. PURPOSE: This study compared the vertical margin accuracy of lost wax castings produced with the conventional casting technique using a metal ring and a technique that uses a ringless system. MATERIAL AND METHODS: From copings fabricated on a metal die, 30 castings were produced from a high palladium alloy for metal ceramic restorations (Argedent). Ten castings were cast with Bellavest T (Bego) phosphate-bonded investment with the ringless technique, 10 were cast with the same investment with a metal ring, and the final 10 were cast using Hi-temp (Whip Mix) phosphate-bonded investment with a metal ring. The internal surface of the castings were not modified before seating with finger pressure. For vertical margin discrepancy measurements, an optical microscope at a magnification of 100x was used. Data were analyzed with 1-way multivariate ANOVA (repeated measures) and the Student-Newman-Keuls test. RESULTS: When following the manufacturers' recommendations, the castings of the ringless technique provided less vertical margin discrepancy (mean value 181 +/- 71 microm) than the castings produced with the conventional metal ring technique (290 +/- 87 microm and 291 +/- 88 microm). The difference was significant (P <. 001). CONCLUSION: The ringless technique may produce accurate castings for use in fixed prosthodontics, therefore further investigation is needed to develop a protocol for its use.

Custom adaptation of an occlusal plane analyzer to a semiadjustable articulator.

The use of a semiadjustable articulator has become more popular for prosthodontic rehabilitation. An occlusal plane analyzer has long been used to assist the operator in the development of an initial mandibular occlusal plane in diagnostic contour casts and later as an integral part of both the contours of the definitive restorations as well as guidelines for the actual tooth preparations. Many manufacturers of semiadjustable articulators offer no such occlusal plane analyzers for use with their instruments. This article describes modifications for the adaptation of an occlusal plane analyzer to the WhipMix 2240 semiadjustable articulator, a possibility heretofore unavailable from any manufacturer.

A technique to create appropriate abutment tooth contours for removable partial dentures.

A technique is presented for using light-cured composite resin to create desirable contours on abutment teeth for the retention and support of removable partial dentures. The desired tooth shape is created on a diagnostic cast, captured in a clear temporary splint material, and formed in resin on the abutment teeth with the splint acting as a matrix.

Computed tomographic scan template for maximum accuracy of reformatted CT scan images.

The dental computed tomographic (CT) scan is useful in presurgical planning for implant prosthodontics. To provide accurate information, the patient must be positioned properly during the scan procedure. This article describes a method for fabricating a template that provides guidance to the CT technologist for desired head orientation of the patient.

Bone grafting and osseointegrated implants in the treatment of cleidocranial dysplasia.

Cleidocranial dysplasia is a rare condition that presents a variety of problems for the restorative dentist. This article defines the basic characteristics, including the oral manifestations, of cleidocranial dysplasia and describes some of the available treatment modalities. It also presents a case in which iliac crest bone grafts and endosseous implants were used in the maxillary arch in an attempt to provide a retentive and esthetic overdenture for a young patient. Discussion of this case includes several questions concerning the rationale for using osseointegrated implants for a patient with this condition.

Protection of recent extraction sites when fabricating immediate acrylic interim fixed restorations.

Protection of all soft tissues is an early and integral component of the total treatment plan involving any dental restoration. A procedure is presented to provide protection of recent extraction sites by using adhesive Burlew foil when making an immediate fixed restoration. The foil provides protection, at the extraction site, from both mechanical and chemical irritation.
