[Mesangial IgA-glomerulonephritis].

IgA-glomerulonephritis represents the most frequent glomerulonephritis (GN; 20%) among our patients. In contrast to data from the literature the prognosis is not benign. Renal insufficiency developed in 17 out of 50 investigated patients within 4 to 96 months, 3 of these patients had to undergo dialysis. Eleven of the 17 patients still had a normal renal function at the time of diagnosis. Malignant hypertension was present in 5 patients. An unfavourable course was predictable in cases of male gender, proteinuria, hypertension, age above 30 years, and histological changes indicating glomerulosclerosis, tubular atrophy, interstitial fibrosis and vascular lesions. Increased serum IgA levels, circulating IgA complexes, association with certain HLA-B or -Dr antigens as well as clinical symptoms and signs of haematuria, dysuria and kidney pains were not helpful either for diagnosis or for prognosis. The value of skin biopsy was comparatively small. Positive IgA demonstration was possible in 12 out of 41 cases with IgA-GN, however, also in 4 out of 21 patients with non-IgA-GN. None of 50 probands without renal disease showed IgA. Five out of 7 skin biopsies demonstrated IgA2, one IgA1 and one both IgA1 and IgA2. Increased serum IgA levels were found in a high percentage (21 out of 38 patients). The same applied to circulating IgA-complexes (8 out of 33 patients).

Emphasis on peanut lectin as a marker for granular cells.

Peanut lectin (PNL) binding to a total of 13 granular cell tumours was examined by means of the peroxidase antiperoxidase technique. The tumours included six tumourettes of the neurohypophysis, one malignant granular cell tumour of the brain, and six peripheral tumours of distinct locations. Every tumour studied showed intracytoplasmic fine granular PNL binding; after pretreatment with neuraminidase, the weakly positive reaction was enhanced to a great extent. In all tumours simultaneous examination for the detection of lysozyme and glial fibrillary acidic protein (GFAP) was also carried out. Lysozyme was negative in all cases, whereas GFAP expression could be demonstrated at the periphery of the malignant granular cell tumour of the brain. The data presented clearly demonstrate that PNL can be used as a histochemical marker for granular cells regardless of their location. The fact that the presence of lysozyme could not be proved does not support the view of a histiocytic origin for granular cells, whereas the expression of GFAP in some immature granular cells of the brain tumour examined is considered to be an argument in favor of its glial origin.

Clinical and serological features of mesangial IgA glomerulonephritis.

IgA-glomerulonephritis (IgA-GN) accounts for approximately 20 per cent of all glomerulonephritis in our unit. Seventeen out of 50 patients with IgA-GN developed renal failure, which appeared in 11 out of 17 over the course of a mean follow-up of 68 months. Haemodialysis was required in three patients. Twenty-two out of 50 patients had hypertension, five with malignant hypertension. Perivascular IgA deposits were found in skin biopsies of 29 per cent of patients with IgA-GN and also in 19 per cent of patients with other GN, but not in healthy controls. Mucosal (salivary and nasal) secretory IgA concentrations were normal. In cutaneous and glomerular IgA/IgM deposits, IgA1 was demonstrated using monoclonal antibodies. No excess of HLA-A, B or DR antigens and no relation of clinical course and HLA-Bw35 were found.

Mesangial alterations in steroid-responsive minimal change nephrotic syndrome.

Renal biopsies from 25 children with steroid-sensitive minimal change nephrotic syndrome were evaluated retrospectively to determine whether there is any relation between the morphological changes and the frequency of relapses. Biopsy material was examined by light-, immunofluorescence-, and electron microscopy, and by morphometric methods. The patients were divided in a group of 15 children with frequent relapses (FR) and another group of 10 children with an absence of, or only infrequent, relapses (NR/IR). Semiquantitative evaluation of biopsy specimens disclosed no significant differences between groups, but morphometric measurements performed on toluidine stained semi-thin sections showed a significant increase of mesangial nuclei in FR compared with NR/IR (P less than 0.01). Furthermore, the mean area of mesangial nuclei was decreased and the relative frequency of smaller nuclear profiles was higher in patients with FR compared to NR/IR (p less than 0.01). These findings suggest mesangial cell activation in FR which may be related to a longer course of the disease prior to renal biopsy (mean 4.0 years in FR vs. 1.4 years in NR/IR). In our opinion, morphometric assessment of discrete mesangial alterations is a promising method for exploring clinicopathological correlations in minimal change nephrotic syndrome.

Procollagen-type III-peptide serum concentrations in chronic persistent and chronic active hepatitis and in cirrhosis of the liver and their diagnostic value.

Until now the determination of fibrotic processes in liver disease was restricted to histological examination of liver tissue. Recently a RIA was developed to determine the procollagen-type III-peptide concentrations in biological fluids. We used this RIA to measure the serum procollagen-type III-peptide concentrations in patients with chronic liver disease. Additionally in 24 patients with chronic persistent and chronic active hepatitis the collagen content in liver biopsies was determined histomorphometrically. The serum procollagen-type III-peptide concentrations in healthy controls (n = 40) were 6.4 +/- 0.6 ng/ml, in chronic persistent hepatitis (n = 47) 8.7 +/- 0.5 ng/ml, in chronic active hepatitis (n = 53) 20.8 +/- 2.9 ng/ml, and in alcoholic cirrhosis of the liver (n = 22) 47.7 +/- 6.1 ng/ml. Thus the patients with chronic active hepatitis and cirrhosis of the liver showed significantly elevated serum procollagen-type III-peptide levels. In chronic hepatitis a highly significant correlation (p less than 0.001) could be demonstrated between collagen content in liver tissue and serum procollagen-type III-peptide concentrations. The determination of serum procollagen-type III-peptide concentrations may prove a new useful parameter in biochemical evaluation of liver disease.

Silicone filings in macrophages of viscera: an iatrogenic complication of haemodialysis.

In seven long-term dialysis patients, autopsy material showed varying amount of refractile foreign material in macrophages and giant cells of lung, liver, spleen, bone marrow, skin, thoracic and abdominal lymph nodes, but not in brain, heart, kidney or endocrine organs. Electron microscopy showed its presence within lysosomal membranes of macrophages. Such material was also demonstrable in liver biopsies of patients dialysed for only several months. Possible clinical consequences were hepatosplenomegaly, elevation of transaminases, hypersplenism with pancytopenia and possibly bile duct carcinoma. SEM with X-ray fluorescence showed particles within spleen and liver cells with a characteristic Si K alpha 2.09 kev peak. Identical material was found as fillings on silicone tubing exposed to roller pumps.
