The metabolism of fluorides in impaired renal function is described. The use of fluoridized drinking-water in the production of dialysis fluid is discussed controversial. In patients treated by chronic hemodialysis the plasma fluoride level rises at a content of 1 mg fluoride per liter dialysis fluid. A relevant effect on the state of health is not certain established today. Therefore, it will be advised to the use of fluoride-free dialysis water. Its production should be made by use of a reverse-osmosis unit.
The successful outcome of a pregnancy complicated by reversible renal failure secondary to total ureteral obstruction caused by a pregnant uterus and treated temporarily with nephrostomy is reported. The cyclosporine A (CsA) and prednisone treated female recipient of a cadaveric renal allograft gave birth to a male child, which at 2080 grams was small for gestational age (35 weeks of pregnancy). The child presented neither signs of congenital anomalies or chromosome aberrations nor nephrotoxicity, hepatotoxicity or anemia. Simultaneous measurement of trough CsA blood levels (CsA RIA, Sandoz) displayed reduced values in the child's blood (mother 864 ng/ml-4 hours after oral CsA intake; son 312 ng/ml). Beside postrenal failure the patient's pregnancy was complicated by 7 rejection episodes treated with high doses of methylprednisone (total dose 5 g) with reversible damage of the transplant function, two episodes of a urinary tract infection and increasing anemia necessitating blood transfusions. The HIV negative patient had developed a Kaposi's sarcoma 6 weeks after grafting. The progression of infiltrating skin lesions during pregnancy was not seen.

Percutaneous gastrostomies or gastroenterostomies serve for temporary or permanent enteric feeding in patients with obstruction or functional derangement of the esophagus or hypopharynx. In addition, this radiological procedure may be indicated for small bowel decompression. The authors present their experience in 71 patients. Insufflation of air through a nasogastric tube or catheter is the preferred method for gastric distension. The inferior margin of the left lobe of the liver and the transverse colon are localized sonographically and fluoroscopically prior to puncture. Either Seldinger or Trocar-techniques have proven effective in establishing access to the stomach. The feeding tube is advanced into the proximal jejunum to reduce the likelihood of gastroesophageal reflux and possible aspiration. Complications were encountered in four patients and included catheter dislocation in three and respiratory distress in one patient.

A mobile unit for child and adolescent psychiatry is described that serves two counties with a combined population of 400 000. In both counties the unit is the third most frequently used outpatient service, providing 21% of all outpatient care in child and adolescent psychiatry in Marburg-Biedenkopf and 40% in Waldeck-Frankenberg. After the outpatient clinics this unit serves the largest number of patients from the lower social classes, in the case of psychotherapy the majority (54.1%). This unit also makes the largest number of home visits. On the whole, the mobile unit can be considered an effective means of providing mental health services in rural areas.
Surgical operations for metastases are palliative. If no effective alternative treatment is available, excision of metastases may be the treatment of choice also for extrapulmonary metastases in the thoracic region and for multiple pulmonary metastases. Experience has shown that, in some cases, the course of a patient markedly improved by resection. However, the success is dependent on an effective chemotherapy, not on surgical enthusiasm. A case selection based on favourable prognostic criteria is not justifiable today. More experience is needed before a therapeutic algorithm may be developed.
