[Enzyme-linked immunoadsorbent assays for myelin basic protein and antibodies to myelin basic protein in serum and CSF of patients with diseases of the nervous system].

Simplified enzyme-linked immunoadsorbent assays (ELISA) for myelin basic protein (MBP) and antibodies to myelin basic protein (Anti-MBP) have been used to test 337 patients with diseases of the nervous system including 36 compressive diseases (CMP), 33 multiple sclerosis (MS), 34 cerebrovascular diseases (CVD), 31 inflammatory diseases of central nervous system (ID), 161 epilepsy (EP) and 42 other nervous diseases (OND). Comparison of results among various groups indicates that serum mean MBP values of CMP, MS, CVD, ID and EP groups are significantly higher than those of OND group and normal control (P < 0.01). The serum mean MBP value of 33 acute trauma patients with spine fracture and paraplegia, the majority of CMP group, is the highest compared with MS group (P < 0.05), CVD, ID and EP groups (P < 0.01). CSF mean MBP value of 15 CVD patients is markedly greater than that of OND group (P < 0.05). No statistically significant differences are found in serum MBP values between OND group and normal control, and in serum and CSF Anti-MBP values among six groups by using our method.

[The value of serum myelin basic protein in assessment of severity of acute closed head trauma].

Serum levels of myelin basic protein (MBP) were measured in 112 patients after acute head injury by enzyme linked immuno-absorbent assay (ELISA). Patients with cerebral concussion showed no significant change in serum MBP. Patients with cerebral contusion, extradural hematoma or intracerebral hematoma had a mean serum MBP concentration much higher than that of patients with cerebral concussion. And patients with extradural hematoma had a mean serum MBP concentration much lower than that of patients with cerebral contusion and intracerebral hematoma; the differences were significant (P < 0.05). But there was no significant difference between patients with cerebral contusion and intracerebral hematoma (P > 0.05). The amount of serum MBP was significantly correlated with the volume of extradural hematoma, intracerebral hematoma and with the extent of the cerebral contusion (P < 0.05). This study suggested that the serum MBP may reflect the type and severity of closed head injury patients.

[Myelin basic protein level in serum of patients with chronic cor pulmonale in acute phase].

With a sensitive enzyme-linked immunoadsorbent assay that can detect as little as 0.5ng/ml of myelin basic protein (MBP), we studied serum specimens from 34 patients with chronic cor pulmonale (CP) and 33 patients with chronic bronchitis (CB) during acute attack in hospital and 30 control subjects. The results showed that the serum mean MBP level of CP patients was markedly higher than those of CB patients and controls (P < 0.01), but no statistically significant differences were found between CB patients and controls (P > 0.05). There was a positive correlation between elevated levels of serum MBP and carbon dioxide partial pressure (PaCO2). This suggested that elevated serum MBP levels may be associated with brain destruction by hypercapnia and hypoxia of CP patients.

[Inhibitory effects of calcium channel antagonist on the proliferation of vascular smooth muscle cells induced by adrenoceptor activators].

In cultured vascular smooth muscle cells (VSMC) of SD rat, it was demonstrated that 10(-6) - 10(-5) mol/L NE and 10(-7) - 10(-5) mol/L isoproterenol stimulated proliferation and 3H-TdR incorporation of the cells in a dose dependent manner. These effects might be significantly inhibited by co-incubation with either alpha-receptor blocker phentolamine (10(-6) mol/L) or beta-receptor blocker propranolol (10(-5) mol/L). It was also found that cell counting and 3H-TdR incorporation were markedly decreased after incubation nifedipine (10(-6) mol/L) or verapamil (10(-6) mol/L) with NE (P < 0.01 as compared to that of NE group). It was further observed that nifedipine and verapamil inhibited DNA synthesis and proliferation of VSMC induced by isoproterenol.

Antipeptide antibodies against overlapping sequences differentially inhibit human CYP2D6.

Two peptides that correspond to sequences within the major 33-amino acid sequence recognized by human liver-kidney microsomal-1 autoantibodies were used to elicit antibodies in rabbits (four per peptide) against CYP2D6. Peptide 1(DPAQPPRDLTEAFLA) corresponded to amino acids 263-277, and peptide 2 (LLTEHRMTWDPAQPPRDLTE) corresponded to amino acids 254-273 of CYP2D6. The peptide-keyhole limpet hemocyanin conjugates elicited good immune responses against their respective peptides as judged by enzyme-linked immunosorbent assay (titers of 1/10,000 to 1/30,000). The antisera recognized CYP2D6 on Western blots and, to varying extents, inhibited recombinant CYP2D6 and liver microsomal CYP2D6 activity. Immunization with peptide 2 produced antisera with the greatest inhibitory potency. Antiserum from a rabbit (#236) immunized with peptide 2 inhibited up to 95% of dextromethorphan O-demethylase activity in human liver microsomes at the highest concentration tested (40% v/v) but did not significantly inhibit CYP1A2, CYP2C9, CYP2E1, or CYP3A4 marker activities. On Western blot, only a single immunoreactive protein comigrating with recombinant CYP2D6 was recognized. In liver microsomes from a CYP2D6-deficient individual, no proteins were recognized, and the antisera did not cross-react with recombinant CYP1A2, CYP2C9, CYP2E1, or CYP3A4. There was a significant correlation between the quantity of immunoreactive CYP2D6 as determined by immunoblotting with anti-peptide 2 antiserum and dextromethorphan O-demethylation in a panel of 10 human liver microsomes (r = 0.95). These data identify a peptide sequence (peptide 2) that can be used to raise antisera that specifically recognize and inhibit CYP2D6.

[Longitudinal studies of developmental status in term-born infants with hyperbilirubinemia].

Thirty-six infants born at term who developed hyperbilirubinemia were assessed for developmental status at 2, 4, 6, 9 and 12 months of age according to a mental and psychomotor scale for 0-4 year old. The development quotient (DQ) of infants with hyperbilirubinemia were lower than controls, and the reduction was significant at 2 months of age (P = 0.03). When single component of DQ was compared, it was shown that the DQ score for social behavior was significantly lower in patients at 2 months of age than in control (P = 0.007), and except for motor, other four single DQ scores were lower in patients than in control and the reduction was significant in the DQ scores for adaptability and social behavior (P = 0.05, 0.032, respectively). Furthermore, no significant correlation was found between DQ and serum peak bilirubin value or duration of hyperbilirubinemia. The results indicate that hyperbilirubinemia may have a long term impact on mental development of infants. It is necessary to monitor all infants with hyperbilirubinemia, including full term infants with no severe complication.

Expression of the antifreeze protein gene in transgenic goldfish (Carassius auratus) and its implication in cold adaptation.

Ocean pout (Macrozoarces americanus) antifreeze protein (AFP) genes (approximately 10(6) copies) were microinjected into the oocytes of goldfish (Carassius auratus). Out of a total of 303 oocytes injected, 235 (77.6%) were normally fertilized after in vitro maturation and insemination, and 136 (57.9%) of the fertilized eggs were hatched. Dot blot analysis of genomic DNA from 2-month-old fish indicated that 30 of the fish (22%) were positive for the AFP transgene. The numbers of integrated AFP genes ranged from a single copy to multiple copies per cell. Two P1 male founders, Y45 and GV16, were crossed separately with a control female, and the inheritance of the AFP transgene was analyzed. Results from Southern blotting and polymerase chain reaction were found to be 7% (5 out of 82) and 58.8% (20 out of 34), respectively, indicating that these two founders are mosaic. The percentage of positive F2 generated from Y45F1 crossed with a control female was 56%, consistent with a Mendelian inheritance. Mature AFP detected by immunoblotting was expressed in both F1 and F2 offspring. These studies indicate that the antifreeze protein gene is successfully transferred and expressed in goldfish. Transgenic goldfish are significantly more cold tolerant than controls when challenged with low temperatures. These studies suggest that the AFP gene may have application in providing cold tolerance in addition to freeze resistance for a variety of fish species.

Factors related to long-term effects of thymectomy on myasthenia gravis.

The authors report the results of long-term follow-up after thymectomy on 92 cases with myasthenia gravis (MG), including 22 cases of thymomatous MG. These patients were followed up for 6-73 months with an average of 23 months. The surgically effective rate was 88.0% and the late mortality, 6.5%. The sex of the patients and the durations of disease were not obviously related to the prognosis, however the effect of thymectomy was much better in patients aged 10-39 years than in other age groups. The modified Osserman's clinical classification, acetylcholine receptor (AchR) antibodies and citric acid extract (CAE) antibodies titers in plasma and pathological patterns of the surgical specimens of thymus including immunohistochemical observation play an important role among various factors related to the long-term effects.

Reducing the concentration of contrast medium in dynamic computed tomography of the neck: consequences for image quality, side effects and cost.

OBJECTIVE: To determine the efficacy of two concentrations of nonionic contrast medium (320 and 240 mg iodine/mL; Optiray [ioversol], Mallinckrodt Medical) for enhanced dynamic computed tomography (CT) of the neck. PATIENTS AND METHODS: Between March 1992 and August 1993, 50 patients undergoing dynamic CT of the neck were assigned randomly to receive either the higher-strength or the lower-strength contrast agent. The amount of opacification was measured objectively, and the quality of the scans was assessed subjectively by two radiologists without knowledge of which contrast agent had been administered. Both immediate and delayed side effects were recorded. RESULTS: The higher-strength contrast medium gave greater opacification (p = 0.0006, t-test), but both concentrations yielded images of excellent quality (p = 0.125, chi 2 test). The number of patients affected by immediate minor complications was lower with Optiray 240 (11 [44%]) than with Optiray 320 (23 [92%]) (p < 0.001, chi 2 test). The number of patients affected by delayed side effects was similar in the two groups (7 [28%] with Optiray 240 and 9 [36%] with Optiray 320; p > 0.70, chi 2 test). No predictive relation between immediate and delayed complications was observed. CONCLUSIONS: Because adequate contrast can be obtained with the lower-strength contrast medium, it can be used in dynamic CT of the neck as a means of reducing the cost and the side effects associated with such imaging.

[Histopathologic observation and 3DCT reconstruction of the nasal cavity and paranasal sinuses in children with cleft palates].

The morphologic characteristics of nasal cavity and paranasal sinuses in children with cleft palate were studied by means of histopathology, CT scan, computer-aided image analysis and three-dimensional CT (3DCT) reconstruction. Our results suggested (1) incompetent velopharynx is the main reason for high incidence of sinusitis in children with cleft palate; (2) computer-aided image analysis, together with 3DCT reconstruction, may calculate and demonstrate the volume and shape of the paranasal sinuses. We believe this method may be useful in the future to study the development of the paranasal sinuses in children. 3DCT reconstruction provides a better understanding of the size and invasion of the lesion located in nasal cavity and paranasal sinuses. This method may be used as an aid to teaching and surgical planning.

Abnormal regulation of cytosolic free calcium in vascular endothelial cells from spontaneously hypertensive rats.

OBJECTIVE: To investigate whether the resting cytosolic free calcium concentration and the agonist-induced increase in this concentration in vascular endothelial cells from spontaneously hypertensive rats (SHR) differ from those in normotensive Wistar-Kyoto (WKY) rats. DESIGN: The Fura-2 fluorescence technique was used to monitor changes in cytosolic free calcium concentration in isolated aortic endothelial cells from SHR (aged 12 weeks) and age-matched WKY rats. METHODS: Aortic endothelial cells from primary culture to the third passage were used. The cells were grown to a confluent monolayer on coverslips before the fluorescent measurement of cytosolic free calcium concentration. The resting cytosolic free calcium concentration and changes in it induced by bradykinin, endothelin 1, angiotensin II and thapsigargin were examined and compared between endothelial cells from SHR and WKY rats. RESULTS: The resting cytosolic free calcium concentration in aortic endothelial cells was significantly lower in SHR than in WKY rats. Bradykinin induced a lesser increase in cytosolic free calcium concentration in endothelial cells from SHR than in those from WKY rats. However, aortic endothelial cells both from SHR and from WKY rats had a similar calcium response to endothelin 1 and angiotensin II. Furthermore, the thapsigargin-induced increase in cytosolic free calcium concentration was significantly less in SHR than in WKY rat endothelial cells. These results suggest that the altered calcium response to bradykinin in endothelial cells from SHR could be caused by an alteration of bradykinin receptors or changed bradykinin degradation. Alternatively, the intracellular calcium stores that are sensitive to thapsigargin and bradykinin may differ from those that are activated by endothelin 1 and angiotensin II in SHR aortic endothelial cells. CONCLUSION: A lower level of resting cytosolic free calcium concentration and the impaired bradykinin-induced increases in it in SHR endothelial cells could reflect dysfunction of vascular endothelium underlying a reduced endothelium-dependent vasorelaxation in this genetic hypertension model.

[CF/5-FU-DDP therapy for esophageal carcinoma].

From March 1992 to May 1994, 105 patients with esophageal carcinoma patients were treated with CF/5-Fu-DDP (group A, n = 40) or 5-Fu-DDP (group B, n = 65) protocol. The response rate (CR+PR) was 80% in group A and 60% in group B. (P < 0.01) Induction chemotherapy was followed by surgery or radiation therapy. The incidence of nausea, vomiting, cardiotoxicity and leukopenina was higher in group A than in group B. Buccal mucositis, diarrhea, abdomenal pain and skin pigmentation were noted only in group A. However, the toxic effects were tolerable. The results suggest that the CF/5-Fu-DDP regimen is more effective than 5-Fu-DDP regimen and may be one of the best chemotherapy regimens for the treatment of esophageal carcinoma.

[Studies on the synthesis and structure-activity relation of deltorphin I analogues].

Endogenous opioid deltorphin I (DEL I) and its three analogues (progressive, stepwise repositioning of Asp from 5 to 7) were synthesized by solid phase method. DEL I at 10(-14)-10(-10) mol.L-1 in vitro and at 0.5-5 micrograms.kg-1 in vivo was found to increase the percentages of erythrocyte rosette forming cells (E-RFC) and red blood cell C3b receptor garland (RBC-CR1). This augmentative effect of DEL I was antagonized by naloxone. The results indicate that DEL I enhanced the immune function of rats. The order (from strong to weak) of the analgesic and immune activity was shown to be Asp4, Asp7, Asp5, Asp6 and Asp4, Asp7, Asp6, Asp5 respectively.

[A primary observation on the effect of cell fusion on metastatic potential of tumor cells].

Mouse spleen cells transfected with pSV2 neo by CaPO4 precipitation were fused with highly metastatic cell clone (PLA801-D95) from human large cell lung cancer cell line. Hybrid cell clone PMS-2 was obtained after G418(400/ml)selection. After injection of 7 x 10(6) PMS-2 cells into nude mice, there was a tumor nodule developed, but the metastatic foci could not be found while 3 x 10(6) PLA801-D95 cells would metastasize to lung and lymph nodes after they were injected into nude mice. It might indicate that sometime mouse spleen cells could not suppress tumor formation but the metastatic potential could be suppressed by the fusion of mouse spleen cells with the lung cancer cells. The results of growth curve, serum independence and incorporation rates of H-thymidine all showed that the growth rates of parental cells were higher than those of PMS-2. Our data suggest that suppression of tumorigenicity and metastatic potential could be controlled by different kinds of genes, and the cloning of metastasis suppressor gene by subtractive hybridization is ongoing in our laboratory.

[Transsphenoidal microsurgical removal of GH-secreting pituitary adenoma in: a report of 200 cases].

From July 1979 to April 1992, 212 cases of GH-secreting pituitary adenoma were treated transsphenoidally by microsurgical technique, of whom two hundred cases monitored with their GH levels at preoperative and postoperative periods. The majority of cases totally 132 cases were of macroadenoma, and 68 cases were of microadenoma. 138 cases were operated on after April 1986. The cure and remission rate averaged to 72.5%, rising from 44.4% (before 1986) to 80.5% for microadenoma and from 31.4% to 69.1% for macroadenoma due to improvement of technique and accumulation of experience. One patient died of angiocardiopathy suddenly during operation. Mortality rate was 0.5%. The follow-up observation of postoperative GH levels in 52 cases revealed that the GH levels within 2 weeks after operation were the same as in 3-6 months. It is thought that the early GH levels may represent the surgical results. The surgical technique is described and factors influencing the surgical results, prevention and treatment of operative complications are discussed in this paper.

[Studies on regulatory effects of acupuncture on mucosal secretory IgA in patients with allergic asthma].

We studied mucosal SIgA immunity of patients with allergic asthma treated by acupuncture. Our results showed that concentrations of SIgA and total IgA in saliva (P < 0.01, < 0.02) and in nasal secretions (P < 0.02, < 0.02), and levels of IgE in sera (P < 0.001) from patients with allergic asthma were significantly decreased after treatment with acupuncture. The results suggested that attacks of delayed and immediate allergic asthma could be effectively inhibited by acupuncture. The facts that there were no significant changes in IgA in supernatants of peripheral blood lymphocytes stimulated with PWM of patients with allergic asthma after treated by acupuncture were associated with regulation of negative feedback of T-lymphocyte expressing IgA-R and it released bound factor of IgA in production of polymeric IgA.

[Ultracytochemical observation of secretory cells and surfactant-like lamellar bodies in mucosa of eustachian tube].

The distribution of secretory cells and surfactant-like lamellar bodies in mucosa of the guinea pig eustachian tube were studied ultracytochemically. Classified by means of their morphologic characteristics, three types of secretory cells were identified. The dark granulated cells were predominant in the tympanic orifice, the mixed cells were predominant in the isthmus portion, while the light cells were predominant in the pharyngeal orifice. The distribution of the secretory cells in different part of the eustachian tube might play a role in the pathogenesis of middle ear effusions. Surfactant-like lamellar bodies were found in ciliated cells, nonciliated cells and secretory cells of the eustachian tube mucosa. Surfactant-like lamellar bodies (surfactant-like substance) are important in maintaining the physiological function of the eustachian tube, their behavior in normal and pathological states should be further studied.
