[The accuracy of soft tissue profile prediction with the "Dentofacial Planner" in skeletal prognathism].

30 pre- and postoperative lateral cephalograms of patients with mandibular prognathism are used to evaluate the soft tissue profile prognosis given by the "dentofacial planner". As a result, the horizontal displacement of SLS (sulcus labrale superius) and Pg' (soft tissue pogion) in patients with a normal vertical dimension and additional LMF (labiomental fold) in persons with a marked overbite are calculated well. The horizontal changes of the other points analyzed as well as all vertical changes are not predicted satisfactorily in the momentary version 4.22 A (febr. 1989), which will be improved by the company.

[Complement activation during storage of preserved blood].

We have provided evidence that during storage of CPD-Al stabilized whole blood factors of the complement cascade get activated. We prospectively measured C3a-desArg, C4a-desArg, and C42C3-activatorcomplex in 30 units of whole blood in the absence and presence of the proteinase inhibitors aprotinin and eglin over a storage period of 28 days. C3a-desArg increased from basic levels of 276 +/- 35 ng/ml to maximal levels of 1922 +/- 218 ng/ml after 2 weeks of storage. Similarly there was an increase in C4a-desArg levels from 172 +/- 16 ng/ml (day 0) to 1483 +/- 221 (day 14), but C4a-desArg levels further increased with time. By contrast C42 C3-activatorcomplex levels increased only moderately from 170 +/- 18 micrograms/ml (day 0) to 207 +/- 13 micrograms/ml (day 28). The addition of aprotinin and eglin could not inhibit the increase in activated complement factors. Our observations might be of clinical importance, especially during massive transfusions, since C3a-desArg in concentrations less than 1 ng/ml enhances the activation of platelets.

Immunization of children with acute lymphoblastic leukemia with live attenuated varicella vaccine without complete suspension of chemotherapy.

A total of 44 children with acute lymphoblastic leukemia were immunized against chickenpox with the Oka/Merck strain live attenuated varicella vaccine. Of these children, 24 continued oral chemotherapy with 6-mercaptopurine during the immunization period and 20 had suspension of all chemotherapy for 1 week before and 1 week after the vaccine. Seroconversion, as determined by the detection of fluorescent antibody to membrane antigens, occurred in 91% and did not differ between patients continuing 6-mercaptopurine from those in whom chemotherapy was suspended. Fever and/or rash occurred in less than one third of vaccinated children. Unexpected reactions occurred in two vaccinated children, one from each group, both of whom had low absolute lymphocyte counts (less than 750/microL) on the day of immunization. Vaccine-induced immunity appeared effective in preventing or modifying chickenpox after exposure to natural disease.

Long-term reproducibility of quantitative computed tomography vertebral mineral measurements.

Similar methods for measuring bone density using quantitative computed tomography have been proposed by Cann and Genant and by Firooznia et al. This study was done to evaluate the long-term reproducibility of quantitative computed tomography in the clinical setting. An anthropomorphic phantom was evaluated over an 18-month interval. A variation of +/- 2.2% was obtained, substantiating that quantitative computed tomography can be done in a reproducible manner over an extended time interval.

High-dose methotrexate and adriamycin in osteogenic sarcoma: the children's hospital of Philadelphia study.

From 1975 through 1979, 25 patients with osteosarcoma received therapy with vincristine, high-dose methotrexate, citrovorum factor, and Adriamycin. Five patients had metastases prior to receiving chemotherapy, and 11 of the remaining 20 nonmetastatic patients received preoperative or preirradiation chemotherapy. Chemotherapy caused objective tumor regression in 1 of 5 patients with metastases and 1 of 11 with measurable primaries. All five patients with metastatic disease died 7-16 months from diagnosis. Of the 20 nonmetastatic patients, 4 did not have primary amputations: all died. Of 16 patients with primary amputations, 6 are alive relapse-free 24-86 months from diagnosis, and 10 are dead. The actuarial survival of 36% is not statistically different from that of historical control groups or from that of concurrent control groups treated with surgery alone. However, because most adjuvant chemotherapy studies have involved few patients, 36% survival is not statistically different from the 50-70+% survival reported in previous studies. Our data fail to demonstrate that the adjuvant chemotherapy has improved the survival rate of children with osteosarcoma. We support a randomized, controlled comparison of adjuvant chemotherapy and aggressive surgical resection.

A convenient large-scale preparation of high molecular weight kininogen from human plasma.

Human high molecular weight (HMW) kininogen was purified by chromatography on DEAE-Sephadex A-50 and CM-Sephadex C-50, followed by gel filtration on Sephadex G-50. From 5 l fresh human plasma approximately 120 mg HMW kininogen was obtained. The yield was 40%. The preparation had a specific activity of 14 microgram bradykinin equivalent/A280 unit. Upon polyacrylamide disc gel electrophoresis HMW kininogen was separated into two close bands, whereas only one band with an apparent Mr of 120 000 was obtained in sodium dodecyl sulfate electrophoresis. Both protein fractions separated in disc gel electrophoresis released kinins upon incubation with kallikreins. The purified HMW kininogen had an isoelectric point of 4.65 when measured by isoelectric focusing. The amino acid composition of the purified HMW kininogen is given. The amino terminus of the molecule is blocked. In oligomerization studies adducts with molecular weights up to 810 000 were obtained. HMW kininogen gave a single precipitin arc in immunoelectrophoresis with antiserum directed against HMW kininogen.

The effect of human granulocyte proteinases on kininogens.

From the results obtained in preliminary in vitro experiments we may conclude that the unspecific breakdown of kininogens by neutral and acidic granulocytic proteinases may occur also in the organism during pathological conditions. Inhibitor application may serve as a valuable tool to prevent unspecific proteolytic degradation and thus elimination of kininogens and other clotting or complement factors from their biological function. Furthermore, specific cleavage products of the kininogens produced by granulocytic enzymes may be useful in the diagnosis of a beginning septicemia as well as in elucidation of the primary structure of the human kininogens.
