Accentuated cyclic activation of peritoneal macrophages in patients with endometriosis.

Peritoneal fluid was collected from 107 women undergoing laparoscopic sterilization or diagnostic laparoscopy for evaluation of infertility. Cells consisting mainly of macrophages were separated and subjected to sophisticated flow fluorocytometric analysis. In this way more detailed information was obtained about activational characteristics of the pelvic macrophage population during the menstrual cycle. In normal women the macrophages, as compared to peripheral monocytes, showed evidence of elevated baseline activation, and a gradual increase in several markers occurred during the menstrual cycle. Cells increased in size, lost their ability to stain for myeloperoxidase, and increased in activity of both endoenzymes and ectoenzymes. These results suggest that female peritoneal macrophages are continuously responding to stimuli. The macrophage irritation was much more pronounced in women with mild endometriosis. This accentuated cyclic activation may represent an inflammatory response to bleeding from ectopic implants or retrograde menstruation or may be a consequence of some defect in the cell-mediated immune response in endometriosis.

Artificial donor insemination: analysis of 149 cases at North Carolina Memorial Hospital.

We analyzed 149 cases of artificial insemination with donor semen at North Carolina Memorial Hospital, Chapel Hill, to study its effectiveness. An overall pregnancy rate of 62.1% was achieved. Based on the mean monthly fecundity rate, the average monthly pregnancy rate was 16.5%, and the cumulative pregnancy rate after three, six, and ten cycles was, respectively, 40.3%, 68.5%, and 95.5%. A uniform evaluation of the women and correction of any treatable female factors before insemination contributed to a high success rate. Diagnostic laparoscopy after three to four failed insemination cycles yielded a high percentage of abnormal findings. These data suggest that almost all properly screened patients will conceive if allowed to undergo a sufficient number of inseminated cycles and that this procedure is a highly effective alternative means of circumventing male infertility.

Ovarian function in monkeys after bilateral salpingectomy.

Since ovarian dysfunction after sterilization may be of clinical importance in women, we studied whether microsurgical removal of both fallopian tubes would have adverse effects on ovarian function in the monkey. Macaca fascicularis monkeys were divided into two surgical groups: those that had undergone bilateral salpingectomy or a sham operation. Femoral blood samples were drawn before and after the operation for radioimmunoassays for progesterone. No significant difference was found between pre- and postoperative levels of the salpingectomized vs. the sham-operated group. At a second laparotomy during the luteal phase, all salpingectomized monkeys had at least one corpus luteum with a distinct stigma on one ovary, as did the sham-operated monkeys. The absence of any demonstrable endocrine change after salpingectomy makes it unlikely that any poststerilization symptoms, if they exist, are caused by the absence of tubal tissue.

Direct trocar insertion at laparoscopy: an evaluation.

The need for pneumoperitoneum before trocar entry in laparoscopy is evaluated. In over 2000 unselected cases performed in a residency training program from January 1979 to December 1982, three complications of bowel perforation and peritonitis occurred, one despite needle induction of pneumoperitoneum. In entering the abdomen directly with a trocar, critical surgical points are emphasized: adequate relaxation, sharp trocars, adequate skin incision, elevation of the abdominal wall, and insertion of the trocar into the true pelvis. For most patients with no previous abdominal surgery, the authors suggest that this technique offers more clinical security because it does not place reliance on secondary tests but emphasizes concentration entirely upon surgical skill and anatomic knowledge during entry.

The removal of fallopian tubes has no adverse effect on subsequent ovarian function in rabbits.

The effect of bilateral salpingectomy on ovarian function was studied in the rabbit. Nine rabbits underwent a microsurgical removal of both fallopian tubes, and nine other rabbits underwent sham operations. Four weeks after surgery the rabbits were mated, and the number of ovulation sites were recorded 18 hours after mating. Serum progesterone (P) levels were also determined prior to the operation and at 6 days and 8 days after mating. Eight days after mating the ovaries were recovered, and the corpora lutea were removed and weighted. The mean number and weights of corpora lutea as well as serum P levels did not differ significantly between the groups. These findings suggest that the absence of fallopian tubes has no effect on subsequent ovulatory function in this animal model.

Crystal structure analysis of a complete turn of B-DNA.

DNA is probably the most discussed and least observed of all biological macromolecules. Although its role in biology is a central one, with many examples such as operators and restriction sites where specific base sequences have control functions or interact with specific enzymes, the structures that DNA can adopt have been based until now only on sequence-averaged fibre diffraction patterns. Recent improvements in triester synthesis methods have made possible the preparation of sufficient homogeneous DNA of predetermined sequence for crystallization and X-ray structure analysis. We report here the first single-crystal structure analysis of more than a complete turn of right-handed B-DNA, with the self-complementary dodecamer sequence d(CpGpCpGPApApTpTpCpGpCpG) or CGCGAATTCGCG.
