Evolution of the delta 4-adrenal androgens responsiveness in childhood during insulin test.

Serum 11-beta-hydroxy-androstenedione (11OHAD), androstenedione (AD) and cortisol (F) were estimated during an i.v. insulin test in 2 large groups of normal prepubertal children and in normal adults. The results revealed that the basal level and responsiveness of AD were much lower in both groups of children than in adults whereas the corresponding 11OHAD data were similar or superior to those of adults. This suggests that during childhood the limited amount of adrenal AD is preferentially switched to the 11-hydroxylation step. Comparison of hormonal data observed in adults after i.v. insulin (mild ACTH stimulation) and a maximal stimulating dose of synacthen showed that the sensitivity of delta 4-3 keto-androgens pathway is inferior to that of F.

[Experimental reproduction of lactic acidosis in the pony].

One pony has been subjected to the intravenous injections of L-lactic acid. Two other ponies have been trained to intracaecal administration of L-lactic acid or sucrose. The obtained results show that: Intravenous injection of lactic acid increases the concentration of histamin and lactic acid, decreases the level of magnesium and reduces the pressure of carbon dioxide in the blood (the control animals and the treated animals) without the clinical symptoms of lactic acidosis. Intracaecal administration of lactic acid induces a high liberation of histamin in the caecum (the control animals and the treated animals), however, the level of histamin in the blood is not modified. Intracaecal administration of sucrose decreases pH and increases the concentration of lactic acid in the caecum, although, in this case, the accumulation of histamin in the caecum appears only in the non-fasting ponies, the starved animals, and the fed animals, which suggests the necessity of food's proteins for histamin synthesis.

Hypopituitarism and idiopathic delayed puberty: a longitudinal study in an attempt to diagnose gonadotropin deficiency before puberty.

Fifty-five hypopituitary patients (43 boys and 12 girls) treated with human GH were studied longitudinally before and during puberty, occurring either spontaneously or induced with testosterone enanthate (100 mg/month, im) in boys and ethinylestradiol (10 micrograms/day, orally) in girls. In addition, 53 boys with idiopathic delayed puberty (IDP) were studied. Gonadotropin integrated responses (IRs) during 90 min after the iv injection of 25 micrograms/m2 LRH, bone ages (BA), and plasma levels of dehydroepiandrosterone sulfate and testosterone were determined at least yearly. Prepubertal hypopituitary patients with gonadotropin deficiency were characterized by: 1) a lowered FSH IR to LRH in most boys and in all girls; 2) a low LH IR for BA; 3) adrenarche either absent or delayed BA; 4) height age close to BA; and 5) the presence of several pituitary deficiencies. In contrast, most prepubertal hypopituitary patients without gonadotropin deficiency showed: 1) a normal FSH IR to LRH; 2) a normal or intermediate (greater than or equal to 75 mIU/ml . 90 min) LH IR for BA; 3) a normal adrenarche for BA; 4) a height age below BA; and 5) isolated GH or GH plus TSH deficiencies. A significant linear correlation was found between LH IR and the logarithm of plasma testosterone. The slopes and levels were similar in controls and hypopituitary boy without gonadotropin deficiency. In IDP, the level was significantly higher. All data obtained in these patients show that the increase in plasma testosterone and the clinical onset of puberty are delayed for the observed pubertal pattern of LH responsiveness. It is concluded that the study of several clinical and biological features, especially the gonadotropin IR to LRH, are of predictive value for the diagnosis of normal or deficient gonadotropic function in prepubertal patients with IDP and hypopituitarism.

An improved method for the detection of heterozygosity of congenital virilizing adrenal hyperplasia.

A modified short ACTH test for the detection of heterozygote carriers of 21-hydroxylase deficiency (21-OHD) was applied to twenty-one controls and fourteen parents of children with 21-OHD. The following modifications were introduced: (1) Endogenous ACTH was suppressed by dexamethasone administration prior to the test, (2) Plasma 17-hydroxyprogesterone (17-OHP), cortisol (F), progesterone (P), corticosterone (B) and delta 4- androstenedione (A) were measured, (3) Variables studied were the ratio of plasma increments (delta) between precursors and end products, (4) Data were analysed by a step-wise discriminant analysis. Significant alterations in the metabolic pathway of F, B and A were demonstrated. The discriminant analysis showed that the addition of B pathway data did not improve the discrimination potency of the test performed on F pathway data. The combination of variates which provided the best discrimination was the logarithmic sum of delta 170HP/delta F at 15 and 30 min. It led to a 94% correct classification for normals and carriers.

[Comparative study of the digestion of a complete food in the pony and the rabbit].

A complete pelleted diet (table 1) containing 11 p. 100 of crude protein and 17 p. 100 of cellulose (ADF-lignine according to the method of Van Soest) was distributed to ponies and rabbits. Total digestibilities, partial cumulative digestibilities in different compartments of the digestive tract (stomach, small intestine, caecum, proximal and distal colon) and changes in biochemical composition, measured by pH, VFA, lactic acid and ammonia concentrations, were compared. Total digestibilities (table 2). There was no significant difference in dry matter between the two species. Crude protein digestibility was higher in ponies, but a greater difference was observed for crude fiber digestibility: digestibility in the rabbit was 60 p. 100 of that in the pony. Starch was entirely digested in ponies and rabbits. Partial digestibilities (table 3). The dry matter content was the same in the stomach, the different parts of the small intestine and the caecum of both species. Starch was rapidly digested, but crude protein digestibility was highly negative in the stomach, small intestine and caecum of both species. Changes in biochemical composition (table 4). VFA concentrations were higher in the caecum and colon of ponies but molar percentages of acetic, propionic and butyric acids were very comparable (fig. 1); L-lactic acid concentration was higher in the first part of the digestive tract of ponies and ammonia concentration was lower.

Determination of reference standards for adrenal steroids excretion in normal prepuberal children.

An improved method of gas-liquid chromatography (GLC) for routine estimation of adrenal steroids in urine is presented. It was applied to the exploration of steroid output in 120 normal prepuberal children to both sexes aged 3 months-12 years. Urinary data were compared to plasmatic values of adrenal hormones estimated by radio-immuno-assays (RIA). Analysis of the relationships between steroids outputs and bone age revealed that the excretion of cortisol catabolites was expressed by a saturation curve whereas that of delta 4-3 keto androgens catabolites were expressed by geometrical pregression curves; urine dehydroepiandrosterone (DHEA) and plasma DHEA-sulfate showed outbreaks of high values beyond 6 years of age which ruled out all curve fitting attempts. The computation of these relationships allowed to determine the confidence belts of the various regressions which appear as a useful tool to estimate the delayed or accelerated character of adrenal activity in prepuberal childhood. Comparison of the various urinary (GLC) and plasmatic (RIA) parameters enabled us to comment on adrenal puberty.

Kinetics of plasma corticosteroids during metyrapone infusion in man.

The effect of metyrapone on plasma kinetics of cortisol (F), 11-deoxycortisol (S), corticosterone (B), 11-deoxycorticosterone (DOC), aldosterone (Aldo) and 17 hydroxyprogesterone (17 OHP) was studied in ten normal prepubertal children intravenously infused with the drug (1 gm/1.73 m2 body surface area in 4 h). Similar falls in F, B and Aldo were observed during the first hour expressing similar sensitivity of the 11 hydroxylases to the drug. The effect of the compensatory increase of endogenous ACTH could be detected after 1 hour by the rise in 17 OHP. However the concentrations of the 11-hydroxy compounds, F, B and Aldo, remained low throughout the infusion period. From the second hour onwards, facilitation of the synthesis of DOC was noticed. After the end of infusion, the levels of the precursors decreased and F and B were restored to initial values. Aldo remained low probably due to the inhibitory effect of the elevated DOC on the renin-angiotensin system.

Neuropsychological study in treated thyroid dysgenesis.

Neuropsychological assessment was carried out in 57 patients aged 3.0 to 17.5 years (mean 8.5) with thyroid dysgenesis under adequate long-term therapy. Starting age of hypothyroidism as estimated by bone age at diagnosis was prenatal in 32 cases, close to birth in 13 cases and postnatal of 1-12 months in 12 cases. Hypothyroidism of prenatal onset results in severe neuropsychological disorders and mental retardation if not treated early. Only some signs of "minimal brain dysfunction" which compensate with advancing age and normal IQ are found in these children if therapy is started before one month of age. Hypothyroidism starting at birth does not result in mental retardation but neuropsychological disorders are found. They are more frequent if treatment is started after 6 months of age. Children who become hypothyroid between 1 and 12 months of age are usually not mentally retarded and show minor neuropsychological disorders.
