Sleep and benign partial epilepsies of childhood: EEG and evoked potentials study.

The electroclinical picture and nosological limits of benign partial epilepsy of childhood with rolandic spikes (BERS) have been better defined by nocturnal sleep records. In all stages of sleep, there is a significant increase in frequency and amplitude of rolandic spikes (RS) without change of their morphology. Another interesting observation is the appearance of independent spike foci in sleep, or brief subclinical spike wave discharges which are limited to the state of drowsiness. More recently, other types of partial epilepsy of childhood with benign evolution have been identified: (a) partial epilepsy with induced spike representing somatosensory evoked potentials; (b) benign psychomotor epilepsy; (c) partial epilepsy with occipital spike waves. In all these forms, the sleep records are essentially similar to those in BERS and have been very helpful in the nosological identification of these forms of epilepsy. For this reason, the sleep records of these special forms are truly informative for the clinician from the diagnostic and prognostic viewpoint. On the other hand, some investigators, have pointed out that, in the initial stage of these benign forms of partial epilepsy, there may be more or less significant intellectual impairment and behavioral disorder, sometimes accompanied by frequent brief absences. From the EEG viewpoint, this condition is characterized by brief discharges of slow spike wave complexes amounting to a pattern of "electrical status epilepticus". This special electroclinical condition mimics the Lennox-Gastaut syndrome but is generally self-limited. Thus, a correct differential diagnosis is very important from the diagnostic viewpoint. There is good evidence that sleep records permit an earlier identification of these conditions and strongly contribute to a correct differential diagnosis. In the benign partial epilepsy the SEPs, during awake and sleep, morphology and latency are normal, while the N60 amplitude is increased. A group of children with benign partial epilepsy shows EEG spikes evoked by tapping, and giant N60 component. This giant component persists during sleep and is not specific for any type of benign partial epilepsy. In conclusion, the results of sleep recordings are conducive to a correct diagnosis and better definition of the nosological delineation of partial epilepsies in childhood; they also provide a better comprehension of their evolution, and thus of their prognosis. The Evoked Potentials seem be a useful tool in the study of benign partial epilepsy.

Cutaneous malignant melanoma in females: the role of hormonal and reproductive factors.

The role of hormonal and reproductive factors as risks for cutaneous malignant melanoma (CMM) was explored by means of a population-based case-control study conducted in Turin, north-western Italy, on 186 female CMM cases and 205 controls. Past use of oral contraceptives (OC) did not seem to exert any effect on CMM risk odds ratio (OR) = 1.12, 95% confidence interval (CI): 0.56-2.24). No association was found even for the longest duration of use, for superficial spreading melanoma cases and for CMM of lower limbs only. While age at first birth did not affect CMM risk, women who had had three or more children seemed to be significantly protected as compared to nulliparous ones. The association was, however, substantially diminished by adjustment for education and other CMM risk correlates (OR for at least three children versus none = 0.62; 95% Cl: 0.29-1.31).

Interaction of fatty acid binding protein with microsomes: removal of palmitic acid and retinyl esters.

[14C] palmitic acid or [3H] retinyl esters incorporated in microsomal membranes were removed by a cytosolic fraction enriched in fatty acid binding protein. When mouse liver cytosol was fractionated by 70% ammonium sulphate, a precipitate and a soluble fraction were obtained. The soluble fraction containing the fatty acid binding protein was able to remove from microsomal membranes, [14C] palmitic acid or [3H] retinyl esters, whereas the precipitate fraction had no removal capacity. Retinoid analysis indicated that 70% ammonium sulphate soluble fraction was enriched in endogenous retinyl esters with regard to cytosol or 70% ammonium sulphate precipitate fraction.

Selective removal of retinoids and palmitic acid from microsomes by fatty acid binding protein and cellular retinol binding protein.

Studies were conducted to explore the effect of mouse liver cytosolic proteins enriched in fatty acid binding protein or cellular retinol binding protein on the removal of palmitic acid and retinoids from microsomal membranes. When the soluble proteins were incubated with mouse liver microsomes containing (14C)palmitic acid or (3H)retinoids, it was observed that fatty acid binding protein removed selectively the fatty acid and retinyl ester, whereas the retinol was mainly removed by cellular retinol binding protein.

[Mortality according to the birthplace in a Turin longitudinal study].

Mortality 1981-85 in the Turin Longitudinal Study population, 25-74 years old, was analyzed according to selected geographic areas of birth and socioeconomic status. People born in the Southern Regions and in the Isles, when compared with people ever resident in Turin, have low mortality from malignancies and accidents and in general from all causes of death but respiratory diseases. People born in the North-Eastern Regions have high mortality, mainly due to malignancies, and respiratory and digestive diseases. Such differences are stronger among men in low socio-economic status and tend to weaken with time from migration.

Cancer mortality in migrant populations within Italy.

Italian death certification rates from all causes of death, all diseases of the circulatory system, all neoplasms, and cancers of the upper digestive and respiratory tract, stomach, intestines, lung and breast in middle age (45-64 years) were analyzed according to selected geographic areas of birth and residence at death. For total cancer mortality and most neoplasms considered, the rates in middle age were closer to those of place of birth than to those of area of residence, although this pattern was more evident for some sites (e.g., mouth or pharynx, esophagus, larynx, stomach or bladder) than for others (e.g., intestines or breast). In most cases, migration had an adverse effect on cancer rates, and the lowest mortality was reported among stable populations (i.e., those with the same area of birth and death). These findings are discussed in relation to the major migration fluxes within Italy during the current century. Moreover, these analyses give information on the quality of Italian cancer death certification, since the observation that area of birth is often a more important determinant of cancer rates than area of residence provides indirect evidence that cancer death certification in various Italian geographic areas is satisfactorily reliable and consistent.

Lipid binding properties of fatty acid binding protein isolated from mouse liver.

The present studies were designed (1) to examine the lipid composition of two protein fractions from mouse liver cytosol, and (2) to correlate this composition with the previously observed transfer and lipid binding properties of a protein named fatty acid binding protein. In the first part of this study, the ability of the cytosolic protein fractions to bind fatty acids was studied by gel filtration chromatography of cytosol in the presence of [1-14C] oleic acid. Two fractions with affinity for oleic acid were identified. The amount of phospholipids, cholesterol and cholesterol esters found in the low molecular weight protein fraction was significantly higher than that present in the protein fraction with high molecular weight. When mouse liver microsomes prelabeled with [1-14C] stearate were incubated with the protein fraction enriched in fatty acid binding protein, the protein removed radioactivity from the neutral rather than from the polar lipid fraction. Such an effect was drastically reduced when the protein was presaturated with oleic acid. The results suggest that fatty acid binding protein may bind cholesterol and its esters in addition to free fatty acids.

[A case-control study of melanoma of the skin in the province of Torino, Italy].

A population based case-control study was conducted in the province of Torino (Northern Italy) where internal migration produced a population with mixed ethnicity and lifestyles. Two hundred and eight cases of histologically confirmed cutaneous malignant melanoma (CMM) and four hundred and sixteen controls were interviewed. Our results gave evidence to the role of skin reaction to the sun (OR 5.0 in men and 2.2 in women) as an indicator of a group at risk. The increased risk of intense and occasional sun exposure as can be inferred from sun exposure during holidays and outdoor sports, has been confirmed (OR 4.1 in men and 2.7 in women). Ethnic differences did not play an important role in our population, women who used oral contraceptives also did not show an increased risk.

Childhood Cancer Registry of the Province of Torino: survival patterns since 1967 and update of incidence rates.

During 1967-1981, the population-based Childhood (ages 0-14 years) Cancer Registry of the Province of Torino has recorded 1057 resident children with incident cancer. The life status of each child has been ascertained as of December 31, 1983. Sex- and age-specific incidence rates have been stable throughout the period. Annual mortality rates (per million) from leukemias (all types) decreased from 34.0 in 1967-69 to 19.4 in 1979-81. Correspondingly, the survival percentage cumulative of leukemic children at 3 years after diagnosis increased from 16 for children diagnosed in 1967-69 to 57 for those diagnosed in 1979-81. For other cancer types, no increases or debatable decreases in mortality rates and increases in survival rates were recognized.

Planned release dihydroergotamine in common migraine and "tension-vascular headache": multicentre clinical trial.

The antimigraine efficacy of dihydroergotamine methanesulphonate in the pharmaceutical form of planned release capsules has been evaluated. Forty-nine subjects were treated, 39 suffering from common migraine 10 from tension vascular headache, for a period of 90 days, with an oral dose of 15 mg twice daily. The product was found to be efficacious in 71% of the common migraine cases, while unsatisfactory or nil results were obtained in the remaining 29%. In the tension-vascular headache cases, efficacy was 40% compared to 60% unsatisfactory or nil results. Tolerance was good in 43 cases, while transient digestive disorders (nausea and vomiting) occurred in 6; in 2 cases the treatment had to be withdrawn for allergic erythema. The results demonstrate that the drug is indicated in cases of common migraine, while it does not appear to significantly influence the course of attacks of tension-vascular headache.

Comparative completeness and correspondence of cancer mortality data as collected by ISTAT and Cancer Registries.

The authors have compared individual cancer mortality records from the file of the Central Statistics Office (ISTAT) with those of the Cancer Registries of Varese and Torino for the period July-December 1976. Most diagnoses ascertained by the Cancer Registries were histologically confirmed. The results showed a high degree of reciprocal completeness (over 90%) for both systems (Cancer Registries and mortality statistics) and a satisfactory level of concordance (75% at Varese, 70% at Torino). The discrepancies, concentrated in certain nosologic codes, were analyzed. The conclusions suggest that the correspondence between mortality statistics and clinical diagnoses is high, at least in the geographical areas examined.

Descriptive epidemiology of breast cancer in the city and province of Torino.

Between 1950-54 and 1970-74 the mortality rates from breast cancer showed a 35% increase in Italy. In the city of Torino, an increase occurred between 1950-54 and 1960-64 but not in the following 10-year period. These trends were confirmed by the analysis of rates by cohorts of birth. In the province of Torino, between 1960-64 and 1970-74 the increase in breast cancer death rates was far lower than in the other provinces of Piedmont. It is suggested that the peculiar patterns in the city and in the province of Torino reflect qualitative changes of lifestyle brought about by the conspicuous immigration from southern Italy during the sixties.
