[Biological aspects of endodontics in advanced old age].

The traditional rules of endodontics are basing on scientific results and clinical experiences, collected in the first half of our century investigating the teeth of teens and young adults with respect to the epidemiologic situation. Prevention, preserving vitality and teeth have caused a transfer of endodontics in elder persons, but there are other biological conditions, as post-eruptive maturation of enamel, regressive metamorphosis of the pulp, transparency of the dentine of the root, apical apposition of cementum. They are of influence for diagnostic and therapy. Proposals are made.

Adult

[Pulpitis chronica clausa, diagnosis and therapy].

Resulting from clinical observation the relations between acute and chronical pulpitis and pulpitis chronica clausa, regressive metamorphoses and pulpal biomorphose are examined by histological findings and the literature. The etiological, nosological and pathohistological differentiation of this process is discussed. With the rising up of endodontological therapy needs into the middle and higher age the chronical inflammatorical and biomorphological events are more important. Proposals for diagnostics, differentiation and in the role wainting up instead of a early therapy are made.

Chronic Disease

[Examination of laser-treated tooth surfaces after exposure to acid].

In principle it is possible to homogenize the enamel surface by melting structural elements with the continuous wave CO2 laser. An experimental caries model was used for testing the acid resistance of the laser exposed tooth surfaces. Laser-treatment and measured exposure to acid produced zones of homogeneous smelting with microcracks and disintegration symptoms. Underneath the melted region the heat leakage obviously causes photo-thermic++ effects determined by increased resistance to acid.

[Genetic aspects of the susceptibility for caries and marginal periodontitis].

The main employment of stomatology with curative matters and material problems caused a neglect of genetic aspects in the aetiology of dental caries and marginal periodontitis. A critical review of literature is the base of proposals for consideration of genetic aspects in the fields of stomatologic epidemiology, dental health care programs and dental health care education.

[Black's rules for cavity preparations--anachronism or adequate plumb line?].

On the basic of a literature review Black's rules of cavity preparations are discussed. In the summary it suggested that the rules are valuable to discuss, but new perceptions had modified them. The contemporary cavity preparations saved the teeth structures and payed regard to the properties of filling materials.

[Stomatological identification after air disasters].

The paper describes experiences of stomatological identification after two air disasters and shows potentialities and limits of an odontologic(al) comparison. The application of a 16-bit-computer is pointed out as the essential improvement of the work.

[The significance of endogenous peptides for dental pulp pain].

The increase of knowledges in the field of endorphines and other peptides causes new aspects of development and transmission of dental pain. Methionin-enkephalin (ME) and substance P (SP) are found also in the dental pulp and the dentine. The concentration of SP is higher in the pulp, than in some other tissues. The concentration of both, ME and SP, in the endodont is different with respect to different functional situations, e.g. it is lower in case of pain. We suppose, there is a peptiderg reception and transmission of pain in dentine. Interaction with this new system opens new ways for opposing pulpal pain.

[Examination of the pH of saliva in children and juveniles in relation to caries, gingivitis and oral hygiene].

The pH rate of unstimulated fasting saliva in the morning in vivo and the following DMF/T, DMF/S, OH indexes and the PIe had been measured or ascertained in 70 subjects aged 10 to 17 years. The analyses stated that there does not exist a direct relationship between incidence of caries and the degree of inflammation of the gingiva and the concentration of saliva hydrogen ions as well. The pH rate of saliva is stated to be also a favourable factor for plaque formation.

Adolescent

[Physiological and psychological specifics of the stomatological care in patients suffering from connatal troubles of the haemodynamics].

Four groups of patients are subdivided related to the degree of severity of the trouble of haemodynamics. Main reasons for the decreased loadability are the lack in oxygen in the capillary range, increased infections of the respiratory systems and an altered situation of defense. The multitude of patients of the groups I and II can be treated in child's stomatology. The endangering of the patient acc. to its loadability, the application of local anaesthetics without supplement of means affecting the circulation and a protection by antibiotics with all the surgical interventions are prerequisites.

Dental Care

[Caries profunda problems in past and at present].

The development of profound caries therapy in relation to soft carious dentin and carious changed dentin is summed up. Because of its pulpitis preventing meaning profound carious therapy demands the scientific disputation in presence and future.
