[vertebral and spinal cord tumors associated with scoliosis and kyphosis].

Eighteen cases of vertebral and spinal cord tumors associated with spine deformity are included in this study. Thoracic spine was involved most frequently. Neurological deficits were found in 14 patients. Surgical management was carried out in 14 cases. In 4 patients with complete transversal spine lesion percutaneous needle biopsy was done to establish the diagnosis. Surgery was supplemented by radiotherapy or radiotherapy and chemotherapy in 10 cases. Neurological improvement was found in 15 cases.

[Injuries of the spinal cord and vertebral column in children and adolescents].

A review of 127 pediatric patients admitted to the Department of Neurosurgery between 1965 and 1993 because of spinal cord and vertebral column injuries is presented. The aim of this paper is to relate clinical appearance and age of the patient. Patients were subdivided into two age groups on the grounds of skeletal development features. The first included 22 patients (3-14 years of age), the second one included 105 patients (15-18 years of age). Accident type, injury mechanism, level of the spinal injury and type of treatment differed in these groups as was found clinically and statistically. Younger group was mostly treated conservatively and the results of treatment were better.

[Clinical analysis of 126 patients with spinal metastases].

A series of 126 patients with spinal metastases and spinal cord lesion treated between 1963 and 1992 at Neurosurgery Department in Poznań has been presented. Neurological condition of the patient, neuroradiological and histological examinations are presented; modalities of management and results are discussed. Treatment was depended on the patient's conditions. Surgery was undertaken if spinal metastasis was the first symptom of the disease. Sixty-one per cent of patients died within 6 months after being diagnosed. Survival was longer in patients treated surgically. Fifty-six percent of them became ambulatory and in majority free of radicular pain. The longest survival was in prosthetic metastases, the shortest in pulmonary ones.

[Disorders of the cranio-spinal junction].

Seventy-three patients were treated due to cranio-cerebral junction disorders between 1980 and 1993. All patients were categorized into three groups depending on the disorder type: congenital (38%), traumatic (47%), or neoplastic (15%). In every group character of the lesion, neurological symptoms and indications for surgical or conservative management were analyzed. The outcome was following: neurological status was improved in 24 cases, in 30 cases the condition was stabilized, progressed in 18 cases and 1 patient died.

[Diagnostic significance of percutaneous needle biopsy in metastatic neoplasms of the spine].

A retrospective review of percutaneous needle biopsy due to the vertebral metastatic neoplasms performed in 92 patients treated from 1975 to 1992 at Department of Neurosurgery, K. Marcinkowski University of Medical Sciences. Thoracic biopsies constituted 45 per cent, lumbar-32 per cent, cervical-18 per cent and sacral ones 5 per cent of all procedures. An accurate diagnosis was made in 70 per cent of all cases. The highest accuracy rate was noted in lesions compromising the entire vertebral body (84 per cent) and posterior elements of the vertebrae (89 per cent). Tomographic X-rays, CT and MRI were helpful in visualization of concomitant paravertebral mass. In these cases accuracy rate of the biopsy was 82 per cent. The technique of percutaneous biopsy and histological results were also discussed.

[Brain neoplasms in pregnancy. Two case histories].

The diagnosis of brain tumor in a pregnant women is a dramatic event. The nature and timing of therapy at intracranial neoplasm are affected by the existence of the pregnancy. The authors report a two cases of cerebral neoplasm in pregnancy. Clinical syndromes, diagnosis, neurosurgical and obstetrical management are discussed.

[Injuries of the spine and spinal cord as isolated and multiple injuries].

A series of 511 patients after vertebral column and spinal trauma admitted to Department of Neurosurgery of University of Medical Sciences, Poznań from 1979 to 1992 has been analyzed. Isolated vertebral and spinal injuries in 49 per cent. In 51 per cent they were associated with multiple injuries. Traffic accidents constituted the main cause (50 per cent of multiple injuries). The severity of the injury was assessed on the base of AIS, ISS rating systems and spinal symptoms. The concomitant multiple injuries were no cause of delay to surgical intervention within vertebral column. The short-term results of the treatment were better in isolated spinal injuries, with 5 per cent mortality rate in both groups.

[The management of subarachnoid hemorrhage in pregnancy].

Subarachnoid haemorrhage (SAH) from an intracranial aneurysm or arteriovenous malformation is a grave complication of pregnancy. Authors critically analyzed 12 cases of verified SAH during pregnancy from an 11 identified and 1 unidentified cerebral vascular malformation. Aneurysms were responsible for SAH in 7, and arteriovenous malformations in 4 patients. Subarachnoid haemorrhage occurred during I trimester in 2, II trimester in 3, III trimester in 6 patients and after cesarean delivery in 1 case. We conclude that the decision to operate after SAH during pregnancy should be based upon neurosurgical principles, whereas the cesarean delivery should be done in every pregnant patient with SAH.

[Spontaneous subarachnoid hemorrhage in pregnancy].

Subarachnoid haemorrhage from an intracranial aneurysm or arteriovenous malformation is a grave complication of pregnancy. The authors analyzed 11 cases of verified subarachnoid haemorrhage during pregnancy from 10 identified and 1 unidentified cerebral vascular malformation. The aneurysms were responsible for subarachnoid haemorrhage in 6, and arteriovenous malformation in 4 patients. We conclude that the decision to operate after subarachnoid haemorrhage during pregnancy should be based upon neurosurgical principles, whereas cesarean delivery should be done in every pregnant patient with subarachnoid haemorrhage.

[Conservative operative procedure in surgery of the spine].

An operative treatment within the spine due to the traumatic, neoplastic, vascular or other indications frequently result in complications as disturbance of the shape, flexibility, strength, mobility of the spine and injury of neural structures of a various degree. These complications may be prevented by minimizing surgical injury to the bone, ligaments, nerves and vessels and by adequate intra- and postoperative stabilization of the spine. The management will be effective only if the level, range and type of injury as well as spine tolerance against mechanical loads is diagnosed accurately. The authors included case reports.

[Vertebral column primary neoplasms].

Clinical analysis was carried out in 53 cases treated surgically because of primary vertebral column neoplasms in Department of Neurosurgery in Poznań between 1976-1995. Plasmocytoma solitaire (9 cases), angioma (5 cases), osteoblastoma (4 cases) and aneurysmal bone cysts (4 cases) were observed most frequently. Neoplasms localization was as follows: cranio-vertebral junction (4 cases), cervical segment (14 cases), thoracic (23 cases) and lumbo-sacral segments (12 cases). Severe neoplastic vertebral column destruction was disclosed in 34 (64%) patients, but complete transversal spinal syndrome was noted only in 7 (13%) cases. Surgical procedures: posterior and latero-posterior approaches were applied in most cases while anterior and lateral approaches--were rarely used. In 3 cases two stage operations were performed. Intervertebral fusion or posterior autogenous graft was necessary in 39 cases.

[Lumbar disk herniation treated by microsurgery].

Microsurgical technique was applied in 46 cases of lumbar disc herniation in the years 1993-1994. The group comprised 17 women and 29 men aged 17-46 years (mean age 39.54 +/- 8.72). Sciatic pain was present in 39 (85%) cases, in the remaining ones low back pain was the main symptom. Neurological deficit signs were noted in 21 (46%) cases. The authors discuss the indications to microdiscectomy, the microsurgical technique and outcomes. Microdiscectomy was carried out in cases selected strictly according to the result of neurological examination and radiological findings X-ray of lumbosacral region in 46 cases (100%), myelography in 11 (24%), CT in 4 (9%) and MRI in 31 (67%). MRI served for evaluation of signal intensity, site and site of disc hernia. In 31 cases the operation was done in patients with recognized unilateral and one-level disc hernia. Full remission of complaints was noted in 45 patients during follow-up over 2 years.

[8-oxoguanosine as a marker of neoplastic process in brain].

Reactive oxygen species are toxic and cancerogenic factors to living organisms. They are suggested to cause DNA damage (modification) that triggers cancer development. It seems that oxidative damage product 8-oxo-deoxyguanosine (8-oxo-dG) which induces transversion of G to T could be a good chemical marker for cancerogenesis. The aim of our studies was to use 8-oxo-dG as a probe for brain tumour in 17 patients operated on for intracranial neoplasm. Among the patients there were 7 female and 11 male aged from 14 to 60 year. Mean age was 42.88 +/- 16.14 yrs. Several types of tumours were selected histopathologically: from neuroepithelial tissue--6 cases, meningeomas--4, metastases--3, lymphomas--2, neurinoma--1 and chondrosarcoma--1. The tumour tissue was collected from removed material and stored at -20 degrees C. DNA from the neoplastic tissues was isolated by salt method. After hydrolysis of DNA with nuclease P1 and dephosphorylation with bacterial alkaline phosphatase, the mixture of nucleosides was analysed by liquid chromatography method connected with electrochemical detector (HPLC-ECD) working at potential 400 mV. We found higher level of 8-oxo-dG in DNA of patients with malignant tumour (glioblastoma). However, at the present stage of these studies there was no proportional correlation between the level of 8-oxo-dG in DNA in tumour tissue and its malignancy.

[Lipid image in glioblastoma multiforme].

Changes in the content and composition of lipids in brain tumours of different degree of malignancy are still the subject of numerous scientific studies. It is known that in developing brain tumours structural and functional changes of its cells, take place, in which lipids play a crucial role. The examination of the lipid spectrum was conducted in a material extracted from tumour adjacent areas (by means of fenestration) as well as from tumours themselves in 11 patients (mean age 56.8 yrs). Based on histological studies all the tumours were qualified as glioblastoma 4th degree of malignancy. Lipids were isolated from the tumours as well as from their adjacent areas. Later on they were separated, using column chromatography and thin-layer chromatography, into three classes: phospholipids, galactolipids and neutral lipids. Separated lipids were subjected to quantitative analysis by spectrophotocolorimetric method. In comparison to adjacent areas, in tumours distinct changes of lipid levels were noticed in all analysed lipid classes. Statistical analysis showed significant decrease of phospholipids (elements stabilizing cell membranes) in tumours in comparison to their adjacent areas (Wilcoxon's test: p < 0.05). At the same time in tumours an increase of level of plasmalogens took place: phosphatidylcholine (PC) (PPC), phosphatidylethanolamine (PE) (PPE) typical elements of malignant tumours, responsible for cross cell membrane transportation processes. This phenomenon was accompanied by changes of levels of PC/PE and PC/SM (sphingomyelin) ratios connected with adhesiveness and other membrane features. Changes in the level of lipids, and phospholipids in particular, in glioblastoma 4th degree of malignancy in comparison to adjacent areas can indicate the pathological processes in cells of these tumours.

[Dysontogenetic tumors of the vertebral column].

13 patients were managed surgically for dysontogenetic tumours of vertebral column between 1970 and 1997. 6 males and 7 females aged from 3 to 45 years (average age = 28.46 +/- SD 12.81 years). At the cranio-vertebral junction one tumour was localised. Other tumours were in different segments of vertebral column as follows: 3 cases in thoracic segment, 6--in lumbar and 2--in lumbo-sacral region. X-ray examination revealed coexistence of dysraphic or other spinal defects in 10 cases. Surgery was carried out in all cases using various technique. Histological examination disclosed: epidermoid cyst 6, dermoid cyst 3 and teratoma 4. Good results of treatment were achieved in 11 patients, while in 2 cases neurological deficit symptoms were exacerbated. In patients after teratoma removal post-operative radiotherapy was indicated.

[Acrylic bone cement use for internal spondylodesis in neurosurgical operations].

Vertebral column stabilization relying on performing of surgical internal spondylodesis with acrylic bone cement (methylmethacrylate, MMA) was carried out in 27 cases treated surgically in Department and Clinic of Neurosurgery in Poznań from 1996 to 1999. The age of patients was 16-69 (mean age 50.29 +/- 15.29) years. Male patients predominated (2:1). The vertebral lesion was caused by neoplasm (55% of cases), degenerative disease (26%), trauma (19%). Pathological changes were found in cervical region in 16 (59%), in thoracic region in 6 (22%) and in lumbar region in 5 (19%) patients. The anterior surgical approach to vertebral column was chosen in 17 (63%), posterolateral approach in 8 (30%) and posterior approach in 2 (7%) patients. Bone acrylic cement was used for reconstruction of vertebral body damaged by neoplasm or trauma, whereas it substituted the removed intervertebral disc or vertebral body in degenerative diseases. The acrylic cement graft in all cases maintained the proper anterior and medial vertebral column alignment. Semiliquid acrylic cement was used for filling of defects after tumour removal in thoracic and lumbar vertebral bodies. Acrylic cement graft was properly prepared in cuboid form and was inserted into the intervertebral spaces in patients with trauma or degenerative diseases. In all cases involving bone acrylic cement, stabilization of the vertebral segment was completed by metallic implants (cervical plates, "Z" plates, intrapedicular screws and rods). No postoperative complications (wound infections or implant dislocation) were observed. Postoperative, neurological signs were intensified in 3 cases (12%) and were regressive. Mortality rate was 4%. This type of internal spondylodesis makes possible to restore the anterior and medial columns and immobilisation of the proper vertebral segments.

[Diplomyelia and diastematomyelia. Case reports].

Diplomyelia and diastematomyelia. Case report. Three cases of diplomyelia [1] and diastematomyelia [2] were studied and treated between 1980 and 1990 years. All patients had symptomatic onset in adulthood. Pathological features were caused by localization on the lesion in cervical, thoracic and lumbar segment of the spinal cord. Plain X-rays showed a variety of osseous malformations associated with spinal dysraphism. The most usefull radiographic examination was computerized tomography (CT). Indications to surgical treatment are discussed.

[Clinical assessment of the severity of craniocerebral injury].

Assessment of overall patient responsiveness (coma grading) is essential in the characterization of craniocerebral injuries and grading of their severity. The combination of psychological and anatomical scores and additional examinations has yielded patient characterization which more accurately relates to outcome than when those scores are used alone. The aim of this study was to survey the methods of craniocerebral injury evaluation in neurosurgical practice.
